1.
A certified death certificate for the deceased minor. Check here if you want this returned.

A court-certified appointment of the administrator or executor. This document must be received
within 60 days of court certification. This is sometimes referred to as Letters Testamentary.
OR
Depending on your state laws, a small estate affidavit that is either court-certified or notarized. This
document must be received within 60 days of court certification or notarization. Please check with
your legal counsel for the requirements of your state.
3.
A notarized Affidavit of Domicile.

To transfer the assets to a new or existing ShareBuilder account, please send a notarized or
medallion signature guaranteed “Transfer Assets Between ShareBuilder Accounts” form signed by
the executor or administrator. If you don’t have a ShareBuilder account, visit us at www.sharebuilder.com
to set up a new Individual or Joint Account.
OR
To request a check, send a Letter of Instruction signed and notarized (or medallion signature
guaranteed) by the executor or administrator.
