ELIGIBILITY REQUIREMENTS:
• Camp purchases must be made between May 1, 2014-August 31, 2014.
• Computer-generated DSR Sales Tracking Report that includes camp name and address,
product/quantity sold, date sold and invoice number must accompany request.
• DSRs may submit multiple DSR Spiff Certificates until maximum refund is achieved.
• Copies of the DSR Spiff Certificate may be used, and must be filled out completely.
Each must be accompanied by an original Tracking Report.
• Request must be postmarked on or before September 30, 2014.
• Allow 2-3 weeks for processing.
