• Confidential – HFM gets aggregate results only

etc.
• Utilizes a web-based tracking program from
Wellsource
• Accumulating 600 keys earns $100, 1200 keys
earns $250, with a requirement of 300 keys
pertaining to physical activity

(ACO) in July 2011
• Invited employees and spouses on our health
plan with diagnoses of diabetes, high blood
pressure or high cholesterol
• The goal is to better manage chronic illness to
avoid acute care
