Letter of Recommendation 2. Personal Letter of Recommendation (Optional) 3. Academic Transcript with Fall 2012 Grades (copies are accepted) 4. Photo 5. Résumé We will email the recommender directly to offer our letter of recommendation form. You should follow up with the recommender to confirm that the email was received and to encourage them to complete the form quickly. Alternatively, the recommender may use the PDF of the form and then fax the completed information to 610.265.3678 or scan and email to [email protected]. If you do not have your fall grades yet, submit your transcript as well as a report card or progress report when Fall grades are released.
