APPLICATION FORM FRONT PAGE
Business details
If your name and address have been preprinted check the details, make any necessary amendments and notify your
SGRPID area office. If the form is blank fill in your details.
SECTION 1
LMO continuing commitments
Question 1 – We have listed your current commitments for LMO land based options. Some amendments may not have
been captured prior to this information being turned around so check that the commitments are correct and amend if
necessary.
Question 2 – We have listed your specific actions for the LMO Animal Welfare Management Programme. Please check
that the specific actions are correct and amend if necessary. Any amendments to your list of specific actions must have
been agreed with your vet and documented in your Management Plan. You must provide your SGRPID area office with
written confirmation of this agreement.
SECTION 2
Calculating your allowance for LMO
Question 3 - Calculate your area
You have a maximum yearly allowance (the most you can claim in that year) for LMO based on the total area of land
you declared in your SAF including any allocation of common grazings. Enter the total area of land declared on your
SAF Data Sheets. If you have common grazings, use the previous year’s allocation unless you know it has changed.
Question 4 – Calculating your allowance available for new LMO options
Use the figure for total land from question 3 to work out your total allowance. Deduct the amounts used for existing
LMO commitments (these figures will be printed). This gives the allowance available to you to undertake new LMO
commitments.
SECTION 3
Question 5 – Choosing new Land Managers Options
The LMO Animal Welfare option is closed to new applicants. Complete your choice of remaining options using the codes
and payment rates detailed on pages 3 and 4. We will accept this choice of options as a contract to carry out these
commitments in full. If you do not meet all the requirements of your selected options, that will be treated as a breach
and financial penalties may apply.
