How to submit a Local Service Request Fax via Syniverse’s Fax Server
NSP = Wireline Fax Trading Partner (TP)
OSP = Wireless Carrier Using Syniverse’s Service Management Gateway (SMG) Porting Application
INITIAL SCENE:
1. Wireline Fax TP (NSP) wants to submit a Port In request to “win” a Telephone Number (TN) from a
Wireless Carrier (OSP) that uses Syniverse’s SMG.
2. The Wireline Fax TP manually fills out a Syniverse -Approved non-industry standard LSOG 5
LSR fax form
3. The Wireline Fax TP faxes the LSR to Syniverse’s Fax Server*
4. Syniverse’s Fax Server forwards the image to the Optical Character Reader for Forms application.
5. Syniverse Fax Repair Rep performs verification on the LSR via OCRFF & “releases” to the SMG.
Note: Verification is identified as the process of reviewing inbound fax responses and requests to
identify fax errors, then correcting these errors using OCRFF. Fax errors include questionable
characters that OCRFF cannot recognize on the fax image (for example, OCRFF is unsure if a
mark is a numeral “1” or a lower case letter "L")
6. The SMG converts the LSR to a WPR & sends the WPR to the OSP in the regular manner.
The following screen prints illustrate the fields to fill out an LSOG 5 LSR fax form, in order to pass the
Validations (business rules) in the Syniverse SMG porting application. This example shows a singleline port request.
NOTE: These instructions do NOT include information on how to pass Business Rules for specific
Syniverse Customers.
For example, “ABC Wireless” uses the Syniverse SMG Porting Application. ABC Wireless may require
the Account Number AND the Social Security Number for ABC Wireless to “confirm” a Port Out.
However, the Syniverse SMG only requires the Account number or the SSN, not both.

Syniverse maintains two sets of inbound fax lines. One is dedicated for the receipt of machine generated
or typed forms and the other is dedicated for the use of hand-written forms.

Fields under Administrative Section:
1. PON – Fill in with Request Number (PON) chosen by the Wireline Fax TP. (Max Size = 15 digits, A/N)
2. VER – Fill in with “01” for an initial request (DO NOT USE “00”. This will NOT work.)
3. AN – Can Leave blank
4. SC – Can Leave blank
5. D/TSENT – Date and Time Sent. Fill in with MMDDYYYYHHMM, in military time. For example,
091220031830
6. DDD – Fill in with Desired Due Date using MMDDYYYY. For example 09212003
7. DFDT – Always leave this field blank
8. CHC – Can Leave blank
9. REQTYP –Fill in with “CB”
10. ACT – Fill in with “V”
11. SUP – Leave Blank on Initial Port Request. Fill in when sending a Supplemental Port Request,
because the OSP gave a Local Response with Response Type (RT) of “J”. Use appropriate type. For
example, type 1 = cancel, type 2 = change due date and/or time, or 3 = modify data.
12. RTR – Fill in with “C”
13. CC – Fill in with 4-digit SPID of New Network Service Provider (NNSP).
14. NNSP – Fill in with 4-digit SPID of NNSP.
15. ONSP – Fill in with 4-digit SPID of ONSP.
16. AGAUTH – Fill in with “Y”
17. DATED – Fill in with current date using MMDDYYYY. For example, 09102003
18. AUTHNM – Fill in with name of person who signed Authorization (This is usually the Customer.
Customer = End User, or Subscriber. Max Size = 15 digits, A/N)
19. RPON – Can Leave blank
20. RORD– Can Leave Blank on initial port request. (Should be populated on Supplemental Port Request)
21. CUST – Can Leave blank
22. NPDI – Fill in using “B”
Fields under Bill Section:
23. BILLNM – Fill in with Customer’s name. (Max Size = 15 digits, A/N)
24. STREET – Fill in with Customer’s billing address.
25. CITY – Fill in with the city on Customer’s bill.
26. STATE – Fill in with the state the Customer lives in.
27. ZIP – Fill in with Customer’s zip code. If using Zip + 4, there must be a dash. Example, 33602-3355
Fields under Contact Section:
28. INIT – Fill with name of person who originated the request (This is normally the Sales rep, etc.
Max Size = 15 digits, A/N)
29. IMPCON – Fill with name of person who is responsible for completion of this port (could be same as
INIT, or it might be “Port Center”, etc Max Size = 15 digits, A/N)
30. TEL NO – Fill in with the phone number of the IMPCON. For example, 800-111-1234 (Could be the
800 number of the Port Center for the New Service Provider, etc.)
31. REMARKS – Not a required field on an initial port. Can fill in with something like, “NSP 8859
Winning TN from OSP 8816”.
