How to Set Up your
MUSC MyChart
Account
For First Time Users
You will need your Activation Code
 MyChart activation codes can be
found on the last page of your After
Visit Summary that your doctor
would have given you at your last
appointment.
 If you can’t locate your code, please
call or email the MyChart Support
Team.

1. Go to the MUSC MyChart Website
 https://mychart.muschealth.com/
 Select Activate My Account
2. Enter Your Activation Code and
Information
 Please enter your 15 digit activation code,
the last 4 of your SSN, and your date of
birth.
 Click NEXT
3. Select a Username and Password
 Once you have completed this process, you
will log in to your MyChart account with the
username and password that you select here.
 Please write down your username and
password so that you do not forget it.
 Click NEXT
4. Enter your Email
 You must have an email account to set
up your MUSC MyChart account.
 Enter your email address and retype it.
 Click SIGN IN
5. Read and Accept the Terms and
Conditions
 Please read the terms and conditions
for using MUSC MyChart.
 Click ACCEPT.
Congratulations!
 You have successfully activated your
MUSC MyChart account.
 You can now explore a portion of
your medical information.
 REMEMBER: you will use your
username and password from here
on out to log in to your MyChart
account.
 You will no longer need your
activation code.
