Instructions: A letter that includes the text below should be printed on the End User’s
company letterhead and completed with the appropriate information. Please note: The End
User must sign the Letter of Destruction. The ALC, Distributor or Reseller cannot sign on the
End User’s behalf.

Dear Adobe,
This letter is to advise you that _______________________________ (“Company” or “we”)
has decided to return the licensing order with the details set forth below (the “Licenses”) to our
selected Reseller or Adobe License Center (ALC) Amos Software, spol. s r. o.
We have not used the Licenses and do not plan to use the Licenses. As such, we hereby
ask that you amend your records upon receipt of this letter to reflect that we are no longer the
licensee of the Licenses. We acknowledge that any request for a return of the Licenses by the
ALC or Reseller is subject to Adobe’s approval and receipt of this letter duly executed by us, and
we will not be credited by the ALC or Reseller with respect to the Licenses unless such approval is
given. As a condition of this return, we represent to you that we have not installed or used the
Licenses or any upgrades to the Licenses. We certify that all copies of the media and
documentation associated with the order(s) below have been destroyed.

The form and contents of this Letter of Destruction must not be changed, unless prior written
approval is obtained from Adobe. Adobe reserves the right to reject any Letter of Destruction
and require a new one to be submitted if such Letter of Destruction does not comply with the
requirements set forth in this Appendix.
