Oral-B Health Program: Our Focus Area In its inaugural year, the Oral-B Health Program is focused on dedicating this support to children, particularly those living in regional areas in disadvantaged circumstances, to help improve oral health outcomes. During the 2010/ 2011 financial year, donations from Procter & Gamble Australia Pty Ltd ABN 91 008 396 245’ ("Procter & Gamble") for the Oral-B Health Program will be used to fund a grants program for children in regional areas to access much needed dental services. This is being operated through Barnardos Australia ABN 18 068 55 7906 ("Barnardos") with the assistance of volunteer paediatric dentists. Oral-B Health Program: Grants Criteria The Oral-B Health Program provides small grants (up to $3000.00 per grant) ("OralB Health Program Grant" or "Grant") to Australian based community groups who apply on behalf of a disadvantaged child requiring access to dental services, with the assistance of Barnardos and volunteer paediatric dentists. There is no set criteria against the type of dental work that can be funded by an OralB Health Program Grant. It is up to the discretion of a qualified dentist as to what the child requires. Those children in the greatest need will be considered first; and we will be guided by the following: • • • • Children who have a history of infection or swelling (one or multiple teeth) Children with dental anomalies eg. supernumerary tooth Special needs and medically compromised patients Children whose treatment can only be adequately completed under general anaesthesia The decision on who receives an Oral-B Health Program Grant rests solely with Barnardos. Only children between the ages of 0-16 living in regional areas are eligible to be nominated for a Grant. Regional areas are defined as outside of major capital cities. All Grant nominations for dental work must be accompanied by: • • A referral from a qualified dentist outlining the work required A cost estimate on the funding required to undertake the work (less any dentist professional fees) Page 7 of 11 • • • In the event that the cost estimate of the dental work to be conducted exceeds $3,000 ("Grant Limit"), evidence to the satisfaction of Procter & Gamble and Barnardos that all amounts exceeding the Grant Limit have been obtained. A copy of the nominating community group’s public liability insurance and approved Not For Profit status, and All other information reasonably requested by Procter & Gamble or Barnardos. The Oral-B Health Program Grants are operated and administered by Barnardos. Paediatric dentists are volunteering their time to perform the dental work. The Grant will cover the costs of materials and other costs involved in having the dental work done (excluding the usual costs of dental fees that would normally be charged by the participating dentists). If in the event that a paediatric dentist does not volunteer in time, the grant will also be used to cover the cost of the dentist. Organisations applying for an Oral-B Health Program Grant must be a not-for-profit community group, and hold current public liability insurance cover of minimum $10m. Individuals can not apply. They must apply through a community group (who satisfies the requirements as stated in the above paragraph). Applications open on 20 January 2011 and close on last receipted online application at midnight on 31 March 2011 ("Grant Period"). Applications will only be accepted online at www.oralb.com.au during the Grant Period. Once approved, a Grant Agreement is to be signed by the Community Group’s authorised signatories and returned to Barnardos prior to monies being EFT’d to the group’s bank account. General Terms and Conditions: Information on how to apply & information in relation to the Grant form part of these Terms & Conditions. Participating in this Grant is deemed to be acceptance of these Terms & Conditions and the Grant will be provided in accordance with and subject to these Terms & Conditions. While we would like to be able to assist all disadvantaged children with their dental health needs, we have a limited pool of funds available and as such need to apply the following limitations and terms and conditions. Page 8 of 11 Exclusions: • • • General fundraising project or appeals Any retrospective funding Children of employees of Oral-B®, Procter & Gamble, Barnardos Australia, Referring dentist's practice or the applying organisation. Terms and Conditions 1. No applications will be considered for an Organisation with outstanding acquittals for Grants from previous periods. 2. No applications will be accepted by mail. All applications must be through the official online application form on the website. 3. Only children aged between 0 and 16 years living in regional areas are eligible to be nominated for the grant. Regional areas are defined as outside of major capital cities. 4. All successful Applicant Organisations will be required to sign a Grant Agreement supplied by Barnardos before any part of the grant payment is made. 5. All dental work as a result of the grant must be undertaken by a qualified volunteer dentist. 6. Successful applicant organisations must pay the Grant and any additional funding for any work to be undertaken (if required and agreed) directly to the dentist nominated 7. The above payment to the Dentist must be cleared no later than 2 business days before the work is undertaken. 8. All Community Organisations (Grant recipients) are required to provide an acquittal of the grant, including a feedback report as to how the grant was utilised, the difference it made to the life of the Nominee and an authorised financial report within a month of the work being undertaken, and at the latest by 15 July 2011. 9. The decision on who receives an Oral-B Health Program Grant rests solely with Barnardos. 10. All decisions on the application are final. 11. Applications will not be returned to Applicants. Barnardos reserve the right to delete/destroy part or all of applications received within a reasonable time after the assessment period to which the application applies. This will be done to ensure privacy and confidentiality. 12. Unsuccessful applications will NOT be automatically considered in a subsequent grant round. You must re-apply for consideration of another grant. 13. Closing dates for applications are listed on the site and may change from time to time without notice. 14. All applications are deemed to be received at the time of receipt into the database via the website, NOT the time of transmission by the Applicant Organisation. 15. Applicants will be notified by email of the result of their application. 16. Incomplete, indecipherable, non english and/or illegible applications will be deemed invalid. Page 9 of 11 17. Any costs associated with accessing the website are the Applicant Organisations responsibility and are dependent on their Internet service provider used. 18. One (1) single payment for the total of the Grant awarded will be distributed to successful Applicant Organisations within 30 days of signed Grant Agreement being accepted by Barnardos following approval notification. 19. Grants will be paid by Barnardos by direct deposit into a nominated bank account of the successful applicant organisation. 20. Funds must be spent on the agreed dental work. If for any reason the dental work does not proceed or funds are not fully expended, then the unexpended funds must be returned to Barnardos immediately with the Dental Work Statement. 21. The successful applicant organisation must upload a statement of expenditure certified by your organisation’s Treasurer with a Dental Work Statement upon completion of the dental work as part of the acquittal process. 22. The applicant organisation must acknowledge the funding provided by Barnardos and the Oral-B Health Program in any media coverage or promotional activities related to the dental work for which the funding has been received. Copies to be uploaded as part of the acquittal process. 23. Procter & Gamble, Barnardos and its associated agencies and companies will not be liable for any loss (including, without limitation, loss of profits or other indirect or consequential loss), personal injury or death which is suffered or sustained (whether or not arising from any person's negligence) in connection with this Grant, including but not limited to, where arising out of the following: (a) any dental work conducted by the nominated dentist; (b) any technical difficulties or equipment malfunction (whether or not under Procter & Gamble's control; (c) any theft, unauthorised access or third party interference; (d) any tax liability incurred by a claimant or participant; (e) use of a Grant, except for any liability which cannot be excluded by law (in which case that liability is limited to the minimum allowable by law). 24. The applicant organisation indemnifies each of Barnardos and Procter & Gamble insofar as any activities relevant to the Grant, or the dental work. 25. The Professional Indemnity Insurance of the dentists/specialists performing the work must be utilised for the dental work. 26. The applicant organisations insurance policies must cover any travel by the nominee to and from the dental appointment, and public liability as required for the dental work. 27. The applicant organisation must provide documentation to the satisfaction of Barnardos and Procter and Gamble to demonstrate a current public liability policy, minimum $10 million, is current. 28. Barnardos and Procter and Gamble reserve the right to change these Terms and Conditions, and to make and publish interpretations of these Terms and Conditions, without prior notice. 29. Procter and Gamble reserves the right in its sole discretion to disqualify any individual or organisation who they have reason to believe has breached any of these conditions or any instructions or directions on the promotional website, or engaged in any unlawful or other improper misconduct calculated to jeopardise the fair and proper conduct of the Grant. Procter and Page 10 of 11 Gamble's legal rights to receive damages or other compensation from such an offender are reserved. 30. No changes can be made to the application once it is submitted.
