The prerequisites are:  Overall and science GPA of 2.8 or above  BMS 208  BIO 120  CHM 109  Completion of 45 hours Phase II ~ Application Requirements Applicants must submit all materials to the Director of the Medical Laboratory Science program (see address below). th The application consists of the following components all of which must be completed and submitted by February 15 prior to the intended winter entry. Late applications will be considered assuming requirements are met and space is available in the program. All forms are available at www.gvsu.edu/mls  Completed MLS application  Resume  Unofficial transcripts from all colleges/universities attended  1 – 2 page statement of professional philosophy (Why have you chosen MLS as a profession?)  Three completed MLS recommendation forms One from a chemistry faculty One from any science faculty One of applicant’s choice NOTE: Recommendations are to be returned by the respondent (not applicant) directly to the MLS program office noted below  When the application packet is complete, prospective students will be invited for an interview with MLS Program Director and/or faculty – this interview may include a handwritten essay. Phase III ~ Official Admission Notification Upon completion of Phase I and II, students will be notified of admission into the program and will be asked to set up an advising appointment with a Medical Laboratory Science faculty member.
