Orientation will last until 5:00 PM, and attendance is required. Austin-based students should monitor their e-mail for additional information. Seton ID badge Bring your driver’s license and $10 cash or check only to orientation for issuance of a Seton badge. All student badges are coded to allow 24-hour access to the Medical Library (CEC Room C-2.303), the student lounge in the ORME suite, the Professional Office Building, and the Brackenridge Annex Building. For additional access, please contact the Office of Regional Medical Education. Parking Students working at Brackenridge can pay $10 cash or check only per month to park at the Brackenridge main garage. Your badge will be coded to allow entry to the garage. This fee will also cover parking at Seton Medical Center, but you will need to ask the Parking/Security Office to code your badge for that parking lot {Wabash Garage}. Students working at Dell Children’s Medical Center, Seton Shoal Creek, and Seton Southwest & Northwest may park for free. Pager – Cell Phones UTMB no longer issues or requires pagers in Austin. However, Trauma service may issue a pager to you when you are on call. If you need batteries, they can be obtained in the ORME office as needed. All students will provide cell numbers to each clerkship coordinator in order to receive text pages or calls for urgent or mandatory issues. HIPAA HIPAA is the acronym for the Health Insurance Portability and Accountability Act of 1996. The Administrative Simplification provisions of HIPAA require the Department of Health and Human Services to establish national standards for electronic health care transactions and national identifiers for providers, health plans, and employers. It also addresses the security and privacy of health data. Austin students will be trained on HIPAA rules and policies specific to the Seton Healthcare system. All students will be expected to abide by the policies to the fullest extent of the requirements laid out in Seton HIPAA Communications and Office Security Guidelines. Computer Accounts You will be provided a username and password for the following accounts: Seton Network Account (Windows) – All computers are on the Seton Network and require a Seton username and password to use them. Compass Account (to view medical records) and PACS (to view radiology records). For Seton password resets, call 324-1675 (X41675 within the hospital). For verification purposes, they will ask you for the last four digits of your SSN; give them the last 5 four digits of your PID number instead. We do not share your social security number ever. All UTMB Medical Students are responsible for adhering to the SETON IS Security Policy A-2100: The intent of this policy is to provide a basis for an information security program that would restrict electronic access to authorized individuals only, and provide for patient privacy and confidentiality of clinical and business information. Specifically, this policy is intended to: Protect the Seton Family of Hospital’s (Seton) applications, computer systems, networks, and electronic data by ensuring adherence to all Seton policies. Protect the confidentiality, integrity, and availability of Seton’s Electronic Protected Health Information (EPHI) in compliance with the Security regulations developed under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Minimize the potential exposure to Seton from losses, which may result from unauthorized use of Seton resources. Potential damages include, but are not limited to, the loss and/or unauthorized modification of Protected Health Information (PHI), confidential Seton data or intellectual property, damage to public image, damage to critical Seton internal systems, fines, civil monetary penalties, and criminal penalties. {To review entire policy please visit the Seton intranet under the “policy and procedures” tab IS-HIPAA Security A-2100}} For UTMB password resets, call 409-772-5200. Textbook Information Year 3 textbooks are available at the UTMB bookstore (see the required and Recommended Textbook section at the end of this manual). You may wish to purchase all of your books in Galveston before moving to Austin. Books can be sent by the UTMB Bookstore through UPS. The University of Texas Medical Branch (UTMB) and The University of Texas at Austin have an arrangement to provide UTMB medical students access to the services indicated below at UT-Austin. Before accessing services at UT-Austin, you need to have a guest EID # and a UT card. Contact the ORME for your EID #. Please remember the guest Identification Number (EID) you receive as you will need it to access other services. Once your EID has been obtained you must purchase a photo ID card, at your expense, for $10 from the UT ID Center located on the first floor of the Flawn Academic Center (FAC 102), 2400 Inner Campus Drive Austin, TX 78705 - (512) 495-4444. UT Counseling and Mental Health Services (512) 471-3515 or (512) 471-2255 UT CMHC offers mental health assessments for all current students. In general, services that are provided at CMHC include short-term individual therapy, group therapy, psychiatric services, and assistance with referrals to the community. To receive services, call (512) 471-3515 between 8:00 a.m. -4:00 p.m. to schedule an appointment. Services are provided Monday through Friday between 8:00 a.m. – 5:00 p.m. CMHC is located in the Student Services Building, 100 W. Dean Keeton Street, 5th Floor. In addition, counseling and referral services are available 24 hours a day, 365 days a year through UT Telephone Counseling. Telephone Counseling is a confidential service of CMHC, and is staffed by mental health professionals who are specifically trained to deal with concerns relevant to university students. You must present your UT EID, your identification card, and provide your date of birth and permanent address. For each appointment you will need to present your UT ID Card and EID. UHS does not assess an “Office Visit Fee” for visits to UHS with the exception of: Women’s Annual Exams Initial Travel Immunization Appointments Routine Physical Exams Visits to Urgent Care after hours or on weekends and Holidays For current hours of operations, go to www.healthyhorns.utexas.edu Charges do apply for all tests, services, procedures, supplies, medications used to diagnose and treat illnesses or injuries. These charges are generally lower than those for comparable services at other healthcare facilities. UHS is considered “In-network” with: Student Health Insurance Plan (UnitedHealth Student Resources) selected by UT Blue Cross Blue Shield UT Select (UT Employee Health Plan) UHS will file insurance claims for students with these insurances. Students with private health insurance can request documentation of payment for services at UHS and can file a claim directly with their insurance. If the student prefers, as a courtesy, UHS will file claims for students with private health insurance, however, it is the decision of the insurance company what they will reimburse to UHS or the student. Any charges not paid to UHS will be the student’s responsibility to pay. Two units at UTMB Ferry Road Apartments, 810 Ferry Road in Galveston are available for the exclusive use of Year 3 and Year 4 Austin-based and Houston-based students who need housing during a period of enrollment in a for-credit Galveston-area SOM course. Take the elevators to the third floor. You will either start on the inpatient service at Brackenridge, or at an outpatient site. Ver. 3-1-13 21 Section 6 Student Absence Policy Absence Definitions Students are expected to attend all required activities. Each course and clerkship (“course”) publishes its required activities. An absence is any instance when a student is not physically present at an activity. Students should not assume they are allowed any absences at their discretion or for their personal convenience. Absences are considered acceptable only when unavoidable, which include two types of circumstances: 1. 2. When unavoidable and anticipated, as in a residency interview or presentation at a professional meeting. Students are strongly encouraged to schedule interviews during vacation periods to avoid conflict with scheduled courses. When unavoidable and unanticipated, as in personal illness or family tragedy. Actions Required by the Student In the event of any of these absences, students must (in advance when anticipated, and as soon as possible when unanticipated): 1. Notify their supervising faculty (e.g. facilitator, clinical attending, or preceptor); and 2. Notify their course/clerkship director; and 3. Notify their course/clerkship coordinator Consequences of Absences 1. Absences (as identified above) that total no more than three days in a single course are a matter between the student and the course. For the Practice of Medicine courses, this is defined as three days of absences for the entire academic year. In all cases, the student is responsible for the material missed while absent. At the discretion of the course director, the student may be required to provide documentation of the reason for the absence, and be required to complete supplementary assignments to make up for missed activities, but the course is not required to provide repeat or make-up opportunities for missed assignments. 2. Absences for reasons other than those identified above, or failure to report an absence as described above is considered unprofessional behavior and will be reflected in the student’s evaluation and may be grounds for failure of the course. 3. Absences (as identified above) in any course that exceed three days are a matter that requires consultation with the Associate Dean for Student Affairs (ADSA). In the event of excess absences, the ADSA will either a. determine that a student must withdraw from the course, which will result in the course assigning a permanent transcript grade of “Withdraw”, “Withdraw Passing” or “Withdraw Failing” (any of which requires the student to repeat the course in its entirety); or b. collaborate with the course director who will resolve all excess absences by either i. requiring the student to make-up the missed time through a suitable equivalent or substitute activity prior to the end of the course; or ii. assigning a temporary grade of “Incomplete” if the student, for reasons beyond his or her control, cannot complete all required work by the end of the course (which requires a plan for the student to complete remaining course requirements later), or iii. assigning a grade of “F” (Fail) for the course. A course failure based on excessive absences results in a requirement that the student complete a Professionalism Remediation Program in addition to repeating the course. iv. for Year 3 and Year 4 clerkships, recommend to the Assistant Dean for Educational Affairs that the student receive credit for a single four-week elective in the same field as the interrupted clerkship, if the student has completed at least four weeks of coursework in a satisfactory manner. The transcript will reflect a Withdrawal grade for the clerkship. The student will then repeat the clerkship in its entirely after returning to the curriculum. This option is reserved for students who develop an illness or personal circumstance during the clerkship that will require a prolonged absence. This option is not available for students who could potentially make up missed time within the clerkship period, or for students whose absences warrant a failing grade. 4. Student absences will be tracked longitudinally. The name of any student demonstrating a pattern of multiple absences per course will be forwarded to the Office of the ADSA, who will contact the student and schedule an appointment to discuss the nature of the recurring absences. Approved by the SOM Curriculum Committee 6-1-2012 22 Holiday Guidelines The UTMB Student Holidays for 2014-15 are listed in the Academic Calendar. As patient care activities continue during the holiday periods, medical students with clinical responsibilities are expected to maintain their clinical activity during holiday periods. For Monday holidays, students will be guaranteed at least one full day off during the 3 day weekend. For the thanksgiving holiday, students will be given Thursday through Sunday off. Thanksgiving is the only guaranteed holiday for medical students. If the Fourth of July is on a weekday, students will be given that day off. Students choosing to complete electives during Period 13 (over the winter holiday) should be prepared to work through the holidays; generally schedules can be arranged to permit 3 days off over either Christmas or the New year, or other holiday period as desired. If students are assigned to an outpatient clinical rotation during the holiday period, they will follow the clinic holiday schedule. Inclement Weather The Austin regional campus follows emergency weather delays and closures issued by the University of Texas at Austin. It is advised that you visit the link below and register for text alerts from UT Austin in order to stay current with changing weather situations that may affect your ability to travel to and from clinical duties. http://www.utexas.edu/emergency/ Inclement Weather Determination and Notification 1. When impending inclement weather changes are forecasted, the Office of Regional Medical Education (ORME) will issue an email to students, faculty, and staff to monitor official communications and websites (e.g., UT Austin and UTMB) for possible advisories. 2. If official agencies (e.g., UT Austin) in Austin issue a closure or open delay advisory on the day of inclement weather, ORME will follow the advisory of the official agencies in Austin and relay a similar message to our students, faculty, and staff: a. If students and staff are already on site by the time of the ORME communication, they are to stay on site and to complete their work shift. They are also to monitor official communications for further advisories and to keep ORME and supervising faculty updated on their status. b. If students and staff are not already on site by the time of the ORME communication, they are to remain in place (e.g., at home) and monitor official communications for further advisories. c. If students are to be on night call during inclement weather advisories, d. If they are already on site prior to the start of the night call, they should remain on site to start their night call and continue to monitor official communications. e. If they are not already on site prior to the start of the night call, they should remain in place (e.g., at home) and monitor official communications. 3. If inclement weather is impending and official agencies (e.g., UT Austin) have not issued facility closure or delays overnight or by the early morning of the inclement weather, ORME will make a determination, with the available information, at 6am on the day of the inclement weather, and notify students, faculty, and staff of a local inclement weather advisory. Preeminent in such determinations is the safety of the students, faculty, and staff. 4. ORME will issue inclement weather advisories via email to students, faculty, and staff. The Office of Clinical Education and Drs. Seton continues the Catholic tradition of service established by our founders: Vincent de Paul, Louise de Marillac and Elizabeth Ann Seton. 24 N-95 N95 Mask Fit Testing Instructions Students are not permitted to perform clinical duties or participate in clinical rounding with airborne isolation patients without prior fit test certification. Fit testing certifications from other facilities are not accepted by Seton Family of Hospitals. Therefore, all UTMB students must obtain fit tests within the timeframes outlined below. Days of operations: Occupational health provides fit testing Tuesday-Wednesday-Thursday of each week. Times: Fit testing is available on a walk –in no appointment necessary basis from 8:00 am to 3:00 pm. 512-324-5650 is the Occupational med dept. phone if you have questions about times. Facial Hygiene instructions: Facial hair such as a mustache, or long sideburns should not prevent a successful fit test of an N-95 mask. However a full beard, hair on the sides of face and under the chin will not allow for a complete seal of the mask. This may disrupt a successful fit test and thus disqualify the participant from certification. Dietary restrictions: No sugar or artificial sweeteners, food or gum are allowed up to 30 minutes prior to coming in for testing. Testing normally takes 15-20 minutes; Students should be prepared to spend as much as 1 hour at the testing facility in the event a successful fit test is difficult to obtain. Once fit tested Occupational Medicine will issue a PDF copy of the certificate to you. PLEASE BRING YOUR FIT TEST CERTIFICATE BACK TO THE ORME OFFICE OR SCAN IN AND EMAIL TO: [email protected] as soon as testing is completed or you are able to get by the office. Ideally within 2 hours of exposure. IF EXPOSURE OCCURS ON AN OUTPATIENT ROTATION: The student should request the off-site facility (where they are working) test the source patient at the site where the exposure occurred. This frequently fails to happen or reporting occurs so late the patient source has already left the facility. Students DO NOT provide personal medical insurance information at the time of registration – this is considered Seton Worker’s Compensation based on our contract with Seton. Students should sign a release of medical information to facilitate follow-up care After the initial ED visit follow-up care will be directed by the UTMB Student Wellness office. Local care can be arranged with UT Austin Health Services (as contracted) or students may seek follow up care from their own primary care provider. However, direction from the UTMB Student Wellness office is required. If you are required to travel back to Galveston for follow up care a travel scholarship can be arranged to defray travel costs. See ORME staff for assistance Students will be contacted by Seton Occupational Med Nurse to complete an accident report Students must notify the ORME office as soon as possible after an exposure If exposure DOES NOT occur in a Seton facility AND is out of the city of Austin The student should go to the nearest emergency room and seek care. The student should sign medical release forms so that follow up care and test results can be provided. The student should also request that the off-site facility (where they are working) test the source patient at the site where the exposure occurred. However, you may not eat or take physician’s lounge food at any time. We must strive to be good citizens and guests of Seton. Reported infractions will result in Early Concern Note reports and further actions. Please advise your attending and resident physicians of your lack of badge access to the lounge and that you may not enter the lounge unescorted, even if you are doing so on their behalf or request.
