There will not be any dispensary staff members to help with any prescription queries. Immediately put the burn or scald under running cold water to reduce the heat in the skin. Do this for at least 1 minute. If running water is not possible, immerse the burn or scald in cold water. 2. Cover the burn or scald with a clean, non-fluffy cloth, like a linen tea towel. This cuts down the danger of infection. 3. If clothes are stuck to the skin, don't try to take them off. 4. Call an ambulance or take the patient to hospital. You should take a child to hospital for anything other than a small burn or scald. Do not put butter, oil or ointment on a burn or scald. It only has to be taken off again before treatment can begin. You can also visit NHS Direct online at www.nhsdirect.nhs.uk SOLUBLE Aspirin 300mg tablets (not for those with a history of stomach ulcers or asthma) Paracetamol mixture (Calpol or Disprol) good for pain and reduces fever Menthol crystals or Friar’s Balsam for steam inhalations Antiseptic solution for cleaning cuts and grazes Calamine lotion for dabbing insect bites, sunburn or chicken pox Thermometer - Feverscan forehead thermometers are very quick and easy for children Cotton wool for cleaning cuts and grazes An assortment of dressings Tweezers for removing splinters Remember - The Pharmacist can give on-the-spot advice on your medication and possible side effects. DIARRHOEA Diarrhoea is unpleasant but rarely dangerous. It may be due to a change in diet, food poisoning or travelling abroad. It is often accompanied by colicky (cramp-like) pains and usually begins to get better in 48 hours. TREATMENT: Rest. Eat nothing for 24 hours. Drink small sips of water regularly. As the stomach settles, try boiled rice, plain mashed potato or clear soup before gradually returning to a normal diet. Children and Infants: Try water, juice or Dioralyte sachets made up with boiled water to replace body fluids. Make an appointment to see Advanced Nurse Practitioner if it does not settle after 48 hours, if an attack comes shortly after a visit abroad, or if there is continuous pain. URGENT CARE CAMBRIDGESHRIE If you require medical assistance when we are closed you can contact Urgent Care Cambridgeshire who provide an out of hours service. Call 03301239131 and a duty Doctor will phone you back. Alternatively contact the practice to be transferred. NEW PATIENT REGISTRATION We welcome new patients who live within our Practice area to join us. We accept patients from Wisbech and the surrounding areas. You may register with us if you are living in any of the following villages: STUFFY or RUNNY NOSE The common cold usually starts with a hot feeling in the back of the throat. The patient feels unwell and aches, and the nose starts running with a clear liquid with becomes a thick and yellow discharge after 3 to 4 days. ANTIBIOTICS DO NOT HELP. TREATMENT: Adults add menthol or Friar’s Balsam to the steam inhalations. Inhale for 1 minutes three times a day. Babies and Children often have repeated colds . This helps them build up resistance to infection on the chest. They cough because of the discharge running from the nose down the back of the throat. Do not give linctuses. Try normal saline nose drops from the chemist, two drops in each nostril before bed. FEVERISH CHILD Children often develop feverish illness which settle spontaneously in 24-48 hours. Give liquid paracetamol (Calpol or Disprol). Remove the child's clothing and offer sips of cool water. Sponging with tepid water may also help reduce the temperature. Do not give aspirin to children under 12 years. Make an appointment with Advance Nurse Practitioner if the temperature does not fall after paracetamol (although it may rise again later) or if the child becomes drowsy or obviously unwell. If you are unsure about these or any other medical matters do not hesitate to contact us for advice. Attend the Practice and complete in person with assistance from a member of our Patient Services Team Collect the Registration Pack from reception, one for each family member, fill out all relevant areas and return the form to reception. Most sore throats are caused by a virus infection which antibiotics CANNOT cure. With simple treatment the patient normally gets better in 4-5 days. Tonsillitis usually starts with a sore throat, which causes pain on swallowing. There may be a fever, it might be possible to see white spots on the tonsils, and glands in the neck may be swollen and painful. A hoarse voice, dry cough and sore throat indicates a viral laryngitis. TREATMENT: Adults: Soluble Aspirin - 2x300mg, gargle and swallow four times a day. Drink plenty and take steam inhalations. Children: Paracetamol liquid (Calpol or Disprol) and plenty of drinks. Make an appointment to see Advanced Nurse Practitioner if the sore throat is getting worse after 2 days, or if the patient complains of earache. Visit the website and complete the forms online WHAT HAPPENS NEXT Once you have completed and returned your registration form you will be fully registered with us. You can book to see a member of our clinical team, as soon as necessary. COUGH WHAT DOCUMENTS YOU WILL RECEIVE You will receive a welcome letter and online appointment booking information through the post, shortly after you register. Summary Care Record information is also included and an opting out form. You may have already received the SCR form and opt out form from your previous surgery, if you have completed the form you do not need to fill out another. If you wish to have your core data on the National Spine, you do not need to do anything, if you DO NOT wish to have your core data on the National Spine, please complete the opt out form and return it to our reception. After a couple of months you should received your new health authority letter with your NHS number/address and your new Doctors name and practice address, from the local Health Authority. If dust or food enters the air passages, coughing acts as a protector. It also prevents mucus from causing infection in the chest. To suppress this action with cough mixtures can cause more harm than good. TREATMENT: The best treatment is steam inhalation and avoiding cigarette smoke. Make an appointment to see Advanced Nurse Practitioner if coughing continues for more than a week or two after a common cold has cleared up, if coughing produces yellow or green sputum - it may mean the lower air passages are infected, if breathing is accompanied by a pain in the chest or shortness of breath, or if coughing produces blood. SELF-HELP AND MINOR ILLNESS NHS Choose Well WHO YOU WILL BE REGISTERED WITH You will be registered with an allocated Doctor depending on the Doctors patient list size. You have the right to express a preference of a Doctor to be registered with, if at the time of registration there is sufficient space within your preferred Doctors list you will be allocated to that Doctor. NEW PATIENT MEDICAL It is recommended that if you are on a number of medications that you book to see our Health Care Assistant for a new patient medical. If you wish to have a medical, speak to our Patient Services Team who will be able to book this appointment for you. A Lifestyle & Past Medical History questionnaire form will need to be completed prior to an appointment. When you attend for your new patient medical , you will need to bring with you, your completed Lifestyle & Past Medical History questionnaire, any medications you are taking and a urine sample for testing. VOMITING Vomiting may be caused by a virus infection of the stomach, food poisoning or by eating and drinking too much. It normally stops within 24 hours and may be followed by diarrhoea. Some children will vomit when they have a temperature. TREATMENT: Rest. Eat nothing for 24 hours. Drink small sips of water regularly. As the stomach settles, try boiled rice or clear soup before gradually returning to a normal diet. Children and Infants: Try water, juice or Dioralyte sachets made up with boiled water to replace body fluids. Make an appointment to see Advanced Nurse Practitioner if vomiting is accompanied by continuous stomach pain, if vomiting lasts longer than 24 hours, or if the child is vomiting clear fluid or has a high temperature. The appointment will take around fifteen minutes and the Health Care Assistant will go through your medications with you. It is preferred if you need to see a Doctor, to book with the Health Care Assistant first for your new patient medical. Appointments are by appointment times only. In person - Come in to the practice and speak to a member of our patient services team, who book appointments for all Doctors, Nurse Practitioners and Nurses at the Reception. KEEPING YOUR APPOINTMENT It is important to arrive in good time for your appointment to enable surgeries to run smoothly. If you arrive more than 10 minutes late the Patient Services Team will ask the clinician if it is still possible for you to be seen, if so the clinician will see you as an extra but there will be a wait. CANCELLING APPOINTMENTS Please cancel appointments you no longer need or are unable to attend. Give us as much notice as possible so we can offer your appointment to someone else. Appointments can be cancelled, on-line, via our cancellation option on the telephone 24 hours a day, via text (number quoted in appointment confirmation text) or by telephone, during opening hours. Violence in this context includes actual or threatening physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify you in writing of the removal from the list and record in your medical records the fact of the removal and the circumstances leading to it. ACCIDENT AND EMERGENCY We are not equipped as a casualty unit. If you think your injury may require stitches or if you think you have broken something, please go to the Accident & Emergency Department or your nearest hospital. If the incident is more urgent, please dial 999 for an ambulance. FREEDOM OF INFORMATION Information about the Doctors and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager. ACCESS TO RECORDS In accordance with the Data Protection Act 1998 and Access to Health Records Act, you may request to see your medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without your consent unless we are legally obliged to do so. SUGGESTIONS AND COMPLAINTS HOME VISIT We make every effort to give the best service possible to everyone who attend the practice. If you are housebound or too ill to attend the Practice please telephone us on 0844 477 3121 from 8am to request a home visit. However, we are aware that things can go wrong, resulting in patients feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. Your Doctor will phone you back to assess your needs and if necessary, will agree a home visit for you during the course of the day. To pursue a complaint please contact the practice manager who will deal with your concerns appropriately. Further written information is available regarding the complaints procedure from our reception. Please be aware patients on the home visit list for the day will be seen in priority order. PATIENT ACCESS The Practice is accessible to people with disabilities. We have a rear parking area for disabled patients to park and enter the Practice from the rear entrance, some of our rooms are upstairs if patients are not able to manage the stairs, the Clinicians will come and use a room on ground level. Toilets are available on ground level. There is no need to feel embarrassed about this and our students understand and respect your decision at all times. CONFIDENTIALITY & MEDICAL RECORDS The practice complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services. To help you get other services e.g. from the social work department. This requires your consent. When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff. PRACTICE TRAINING Dr. Wong, Dr. Whyman and Dr. Sira are involved in the training of other Doctors. The following groups are trained: MEDICAL STUDENTS FROM THE UNIVERSITY OF EAST ANGLIA, NORWICH These students visit in groups of 10 for one day a week, for 20 weeks per year. Dr. Sira and Dr. Whyman co-ordinate their training. Foundation year doctors (called F1 or F2 depending on which year they are post-qualification) These are qualified doctors who elect to spend 4 months in general practice as part of their 2 year rotation after qualification. Dr Wong and Dr Whyman co-ordinate their training. GP Registrars These doctors are following a 3 year training program to enable them to pursue a career in general practice. They are usually with us for 4 or 6 months. They are fully qualified doctors and are part of the King's Lynn Vocational Training Scheme (VTS). Dr. Whyman and Dr. Wong co-ordinate their training. Although there are named partners co-ordinating training, education is done very much across the whole practice and almost all other members of the practice will be involved in their training. By Phone - The facility to request a repeat prescription is available 24/7. During our opening hours your call will be transferred, when closed you will be advised to select the repeat prescriptions from the options provided. In person - By putting your repeat prescription order slip in one of the request boxes located next to the North Brink Pharmacy and outside the rear entrance. If we are closed, please post your request through the letter box on the front door. Please note all prescription requests made after 3pm will be processed the next working day. Do not book for a General Practitioner if you require: FOR PATIENTS WHO LIVE NEAR A PHARMACY Once a repeat prescription is ready for collection it is held in the Practice dispensary area for you to collect and take to the Pharmacy of your choice to be dispensed We have arrangements for prescription collection with all local pharmacies. All you need to do is sign the authorization form supplied by the pharmacy of your choice, order your repeat prescription in their usual way (phone, in person or via the internet) and the script will be ready for collection by the nominated pharmacy after 2 working days. If you live in a village around Wisbech or further than 1 mile from the nearest chemist you may collect your prescriptions from our Dispensary. FOR PATIENTS WHO LIVE WITHIN THE NORTH BRINK PRACTICE DISPENSARY AREA The same procedure for ordering repeat medication will apply if you live within the area and have your medicines dispensed from the Practice Dispensary. The dispensary team offer a full dispensing service including home delivery, an automatic ordering service (whereby regular medication can be issued 4 weekly without the need for a request by you) and advice on anti-malarial needs for travel abroad. Medication is usually ready for collection 2 working days after the request has been received and we offer a notification by text service also.
