Use this form when you want to rent a vehicle from Enterprise Rent-A-Car. This form is to be submitted along with your
clubs Travel Itinerary Form at least 8 business days in advance of departure date. Only the primary driver listed can pick
up the vehicles. See the Sport Clubs Policy and Procedure Guide for more information about usage of rental vehicles
while on University Business.
