Boarding Requirements

All forms must be completed and submitted before services will be performed.

Pet’s food: All dry food must be pre-bagged per meal. No large bags of food will be allowed.

Toys and/or bedding (Please note that Greenies, rawhide or pig ears are not allowed in the kennel. 2-3 toys only please.)

Cancellation Policy: All reservations cancelled within 72 hours will incur a $20 pet pet cancellation fee. If you fail to check-in
on your scheduled arrival date, you will automatically forfeit the boarding reservation and incur the cancellation fee.

Holiday Policy: NO PICK-UP/DROP-OFF AVAILABLE ON THANKSGIVING DAY OR CHRISTMAS DAY.

$50 holiday fee for pick-up/drop-off on the following days: Easter Sunday, Memorial Day, July 4, Labor Day, Friday After
Thanksgiving, Christmas Eve, New Year’s Eve, and New Year’s Day. *Phones are turned off on holidays.

Nightly charges are per 24 hours of service. Additional hourly boarding rates apply.

Reservations required
Meet and Greet: First step towards getting your dog ready for camp. Done Monday thru Thursday; we ask that you have your dog
stay with us for the day from 7:30am-4:30pm. After your dog completes this process and has a good time, they must come to camp
at least once every month to stay current. If one month passes without coming to camp another scheduled meet & greet may be
necessary. RESERVATIONS REQUIRED.

PETSAFE VILLAGE AGREEMENT
I, __________________________________, do hereby entrust PetSafe Village Corporation to care for my pet(s),
____________________________________________, beginning on the effective date. This agreement will remain in effect until
further notice, and I agree to all of the terms listed below.
I hereby certify that my pet is in good health and has not been ill with any communicable condition in the last 30 days. I further
certify that my pet has not harmed or shown aggressive or threatening behavior toward any other person or any other pet.
I agree that I have notified PetSafe Village of any and all health conditions, abnormal behaviors, or issues that would be pertinent to
the care of my pet(s).
I agree to pay daily board at the PetSafe Village Corporation’s posted rates. If my pet requires special care I authorize PetSafe Village Corporation to provide that care and agree to pay for those services.
I agree to pay for any auxiliary services or products requested by me such as playtime, transportation, grooming, medication, etc. I
also agree to pay for any grooming charges that result from my pets coat condition and/or misbehavior.
I understand that PetSafe Village provides a Daycamp program where dogs socialize, play, and interact with each other from 3 to 4
hours per day. I understand that while PetSafe associates will always be present to monitor Daycamp, injuries can and do occur due
to vigorous play and exercise. I agree that I am solely responsible for any incidental injury that may occur to my dog(s) and I also
agree to be responsible for any resulting Veterinarian charges. I further understand that PetSafe Village will do its best to monitor
my pet(s) intake of food and water. If PetSafe Village notices my pets’ intake of food may not be sufficient, I understand I will be
notified.
I understand that I am solely responsible for any harm caused by my pet while my pet is in attendance at PetSafe Village. PetSafe
Village, its associates, or affiliates will not be held liable for injuries resulting from abnormal behavior (failure to eat, drink, eating
debris such as rocks or feces, allergic reactions, etc.).
I authorize PetSafe Village Corporation to do whatever is deemed necessary for the health and well-being of my pet and agree to pay
for any and all expenses relating to the same. If the pet becomes seriously ill, the owner shall be notified at once. If a pet, which is
presented for boarding, grooming, or daycamp/daycare is found to have fleas and/or ticks, it will be bathed and treated at owner’s expense.
I agree that I am solely responsible for any damages that the above mentioned pet(s) may cause through malicious or improper
conduct and that PetSafe Village Corporation is not responsible for the loss or damage of any personal items left at the facility or
with the pet(s). If the pet(s) is not picked up within ten days after departure date the pet(s) will be placed with a no kill shelter, or
private home in order to find the best home possible. Notice in writing of such intended placement shall be mailed by registered
mail to the owner of the pet at the address given hereon, not less than ten days before the date of such intended placement, and no
further notice shall be deemed necessary. Any deficiency is deemed to be due and immediately paid by the owner.

PetSafe Promise: In consideration for the payment of the fee ($5 per pet, per boarding stay), and subject to the conditions listed
below, PetSafe Village will pay up to $500.00 for any veterinary care provided by a licensed veterinarian for the care or treatment of
the pet(s) listed above for any physical injury of the pet(s). I understand that my pet(s) can become ill as the result of having contact
with other pets, similar to children in a daycare. PetSafe Village will not be held liable for any illness that my pet(s) contracts. PetSafe
Promise covers injuries that occur while the pet is present at PetSafe Village, i.e. cuts, scrapes, puncture wounds, and broken bones.
All other illnesses are considered exempt from PetSafe Promise coverage. Some examples are: * Pre-existing health conditions (such
as gastric upset/issues i.e. colitis, IBS), injury, illness or death resulting from incidents occurring before the arrival date. * Gastric
dilation-volvulus (bloat). * PetSafe Promise will not cover bordetella bronchiseptica/tracheobronchitis/parainfluenza/mycoplasma
(kennel cough), upper respiratory infection *Giardiasis (Giardia). The pet owner hereby agrees to authorize their veterinarian to
forward a copy of the diagnosis and treatment to PetSafe Village. Any and all claims under this program must be submitted to
PetSafe Village within (7) days from the pet’s departure date. Such records are required before any claim is paid. Once received and
verified by PetSafe Village, a check will be sent directly to the owner for reimbursement within 14 business days. A signed statement
by the veterinarian may be required, stating that the illness was not part of or caused by a pre-existing condition. Any controversy or
claim relating to this program shall be settled by arbitration in accordance with the rules of the American Arbitration Association.
