Define medication errors, and common types that occur in pharmacies. Discuss the common causes of medication errors. Review the potential source of errors such as drug names, abbreviations, packaging and communication. Discuss the National Assessment of Adult Literacy and the effect on medication errors from prescription labeling. List steps by the pharmacist to avoid errors during dispensing. Discuss the protocol a pharmacist should follow in mitigating a dispensing error. Recognize common errors in electronic prescriptions and identify the appropriate interventions needed. Identify reference sources available to pharmacists for more information.
