3. Describe methods of error reduction and prevention including
Failure Mode Effect Analysis and the Universal Protocol and
Root Cause Analysis
4. Identify risk management measures designed to reduce error
and enhance patient safety including communication and
collaboration
5. Describe and Organization with a Culture of Patient Safety

Primary Care

1. Discuss the wide prevalence of partner violence in the lives of
patients and colleagues
2. Identify how to screen for partner violence
3. Illustrate how to introduce the patient to “safety planning”, not
just restraining orders or divorce
4. Explain how to document statements and injuries for later use
in the courts
5. Describe how to activate the response system on behalf of a
victim patient and/or the children, while minimizing the risk of
increased violence in the home after discharge
