VENDOR INFORMATION PACKET
Dear Vendor,
Thank you for participating in the 2015 Spring Spa Day, sponsored by the Ladies of Victory. This will be
a time to meet potential clients, gain business exposure, reap a harvest and have a wonderful time as you
provide your services to participants.
The following documents are included in this packet: Vendor’s Information Form, Set-up Request Form, Do’s and
Don’ts Sheet and Vendor Ad Form. You will need to complete and return all forms along with applicable fees as soon
as possible to validate your participation at this event. All requested documents and fees must be submitted no later
than Monday, February 9, 2015, if space is still available at that time. Please note that vendor spaces are offered
on a first–come, first-served basis. The only way to guarantee your spot is to get your forms and money in quickly.
The vendor fee is $60. This fee includes a $10 refundable set up fee and a $10 refundable clean up fee. The $10
set up fee and $10 clean up fee must be paid in cash only. The set up fee will be refundable only if you are
completely set up by 9PM Friday, March 13, 2015. At the conclusion of the event, a representative from the
Vendor Committee will need to verify that your space is clean in order for you to collect your refundable clean
up fee. You must collect your refundable set up and clean up fees before leaving at the conclusion of the event
or consider it a donation. There will be no refunds for no-shows. Once you submit your forms and payment,
you will then have the opportunity to select your location within the designated area for your specialty. Any
cancellation request must be received in writing by Monday, February 16th for refund consideration.
Tickets will go on sale starting Sunday, January 25th. Vendors may begin to advertise their services with flyers,
etc. on that day. Please submit your material to the Vendor Coordinator for approval.

Spa Service Vendor DO’S & DON’TS

1. Complete your information sheet and set-up
request form. Submit them with applicable license
or certificate copies and vendor fees.
Note: Up-to-date licenses are required for
Massage Therapists, Manicurists, Pedicurists,
Persons doing Facials (except Mary Kay or
others who let clients do it themselves). If you
are performing in some other area that requires
either a license or certificate you must submit a
copy of that required document when you register.
1. Wait until the last minute to set up.
(Although you are responsible for your own
set up, assistance will be available up until 2
days before the 2015 Spring Spa Day. If you
set up on the last day, you may not have the
advantage of help from the set-up committee.)
2. Utilize any items that belong to the church
without permission.
3. Hang anything without permission.
2. Set up space between March 9th and March
13th, and be finished by 9PM Friday, March 13th.
(There will be no setting up on the day of the event.)
3. View space at least 1 week before the event.
4. Before setting up, look over the room carefully
so you can put everything back in its proper place
before you leave
5. Bring all necessary items for set up including
power or extension cords, tape, decorations,
pictures, trees, etc.
6. Attach any wall hangings only on existing display
boards. (All displays are subject to approval.)
4. Accept any clients for services without proof of
their having prepaid for the service.
5. Leave any items behind after the event. We are
not responsible for any items left on the premises.
6. Leave the room in a way other than it was
before setting up.
7. Piggy back (sell items for anyone who has not
registered as a vendor for the 2015 Spring Spa Day)
8. Leave without clearance from the Vendor
Committee.
