SCHEDULE OF SCHOOL VISITS FOR RE/CERTIFICATION SY 2014 – 2015
(November 24 - 28, 2014)
Please send the following to FAPE office as soon as possible:
1. Latest Audited Financial Statement
2. Class Schedule for SY 2014 – 15
2. Updated Faculty Profile
The following documents must be prepared for the visit:
1. Approved DepEd Tuition/Miscellaneous Fees for SY 2014 – 15
2. Other updated documents for exhibit
To facilitate the recertification process, kindly prepare the following for the school visit:
1. Certifiers’ headquarters which will serve as their work area (2 certifiers and 2 FAPE staff).
It must be located within the school premises. Please ensure that there is privacy and that
the certifiers will not be disturbed. Exhibits and other documents may also be set up here.
2. Since there will be classroom observations, please make sure that no tests will be
administered, no reviews or other activities that will disrupt the regular lessons.
3. Teachers’ class records will serve as basis for the selection of students for the group
interview.
