Therapy in Osteoarthritis: Are the Effects Molecular
Weight Dependent?

Background:Hyaluronan,or hyaluronicacid (HA),is the maior hydrodynamic
nonprotein component of joint synovial fluid (SF). lts unique viscoelastic
propertiesconfer remarkableshock absorbingand lubricatingabilitiesto SF,
sizeand hydrophilicityserveto retainfluid
while its enormousmacromolecular
in the joint cavity during articulation.HA restrictsthe entry of large plasma
proteinsand cellsinto SF but facilitatessolute exchangebetweenthe synovial
capillariesand cartilageand other joint tissues. In addition, HA can form a
pericellularcoat around cells,interact with proinflammatorymediators,and
bind to cell receptors,such as cluster determinant (CD)44and receptor for
hyaluronate-mediated
motility (RHAMM),where it modulates cell proliferation, migration,and gene expression.All these physicochemicaland biologic
propertiesof HA have been shown to be molecularweight (MW) dependent.
Objective:Intra-articular(lA) HA therapy has been used for the treatment of
knee osteoarthritis(OA) for more than 30 years. However,the mechanisms
responsiblefor the reported beneficialclinical effects of this form of treatment
remain contentious. Furthermore,there are a variety of pharmaceuticHA
preparationsof different MW available for the treatment of OA, but the
significanceof their MWs with respectto their pharmacologicactivitieshave
not been reviewed previously.The objective of the present review is to redress
this deficiency.
Methods: We reviewed in vitro and in vivo reports to identify those pharmacologicactivitiesof HA that were consideredrelevantto the ability of this
agent to relievesymptoms and protect joint tissues in OA. Where possible,
reports were selectedfor inclusionwhen the pharmacologiceffects of HA had
been studied in relationto its MW. In many studies,only a single HA preparation had been investigated.In these instances,the experimentaloutcomes
reportedwere comparedwith similarstudiesundertakenwith HAs of different
MWs.
Results:Although in vitro studieshave generallyindicatedthat high MW-HA
preparationswere more biologicallyactivethan HAs of lower MW, this finding
was not confirmedusing animal models of OA. The discrepancymay be partly
explained by the enhanced penetration of the lower MW HA preparation
through the extracellularmatrix of the synovium, thereby maximizing its
concentrationand facilitatingits interactionwith target synovial cells. However,there is accumulatingexperimentalevidenceto show that the binding of
HAs to their cellularreceptorsis dependenton their molecularsize;the smaller

OA have shown that HAs with MWs within the range of 0.5 x 106-1.0x 106Da
were generallymore effectivein reducingindicesof synovialinflammationand
restoring the rheologicalproperties of SF (visco-induction)than HAs with
MW > 2.3 x 106Da. Theseexperimentalfindings were consistentwith light
and electron microscopicstudies of synovial membrane and cartilagebiopsy
specimensobtainedfrom OA patientsadministered5 weekly lA injectionsof
HA of MW = 0.5 x 106-0.73
x 106Da in which evidenceof partialrestorationof
normal joint tissue metabolismwas obtained.
Conclusions;By mitigating the activities of proinflammatory mediators and
pain producing neuropeptidesreleasedby activated synovial cells, HA may
improve the symptoms of OA. In addition,HAs within the MW range of 0.5 x
106-1.0x 106 Da partially restore SF rheological properties and synovial
fibroblastmetabolismin animal models.Thesepharmacologicactivitiesof HA
could accountfor the reportedlong-termclinicalbenefitsof this OA therapy.
However, clinical evidence has yet to be describedto support the animal
studiesthat indicatedthat HAs with MW > 2.3 x 106Da may be lesseffective
in restoringSF rheologythan HAs of half this size.
