SHA has a conservative dress code. Business casual is the minimum standard.
Tattoos must be covered and piercings removed during business hours.
• Open toe, sling back and tennis shoes are prohibited.

SMOKING

/ TOBACCO POLICY:
SHA is a smoke free / tobacco free environment. We expect the same when on
assignment. Do not use any tobacco products on client property.
CELL PHONE POLICY:

All cell phones are to be turned off during business hours. Any phones with cameras
should not be brought on to client premises.
•
If there is a short term personal issue that requires your cell phone to be turned on
you must seek prior approval from SHA.
TIME

CARDS & PAYROLL
Employees are responsible for their own timesheets. Often a client will prefer to submit
your time to our office and others may require you to submit your own time.
In either case, IT IS YOUR RESPONSIBIUTY to be sure the hours are reported before
8 AM on Mondays. You can fax, email or call in your hours but we must have the signed copy
to release the check.
Pay day is FRIDAY ... Checks are mailed on Monday and dated for FRIDAY.
Time cards can be faxed to 440-243-3777 or scanned and emailed to
[email protected].
Failure to submit your hours or the signed time sheet will delay your paycheck by one week.

ASSIGNMENTS & UNEMPLOYMENT

We expect that you will honor your commitment to us and our Clients. In the event that you
need to leave we would expect the customary two week notice. Failure to comply with this
policy will result in all unpaid wages to be reduced to the current minimum wage of $7.70
per hour. There will be no exceptions to this policy.
Pleasenotify SHA of any time off needed 2 weeks in advance of the event. Example:
dentist or doctor appointments are generally scheduled in advance.
Failure to call off work or an unexcused absenteeism is unacceptable and can result in
immediate dismissal.
Unexcused absences are those which are not scheduled in advance and pre-approved by
SHA. Examples: excessive child care problems, transportation problems, personal problems,
unscheduled appointments, minor illnesses and/or job interviews.
TERMINATION OF YOUR EMPLOYMENT FOR ANY OF THESE REASONS WILL EXCLUDE
YOU FROM UNEMPLOYMENT COMPENSATION.
WORKERS' COMPENSATION

Workers' Compensation is provided for injuries that occur on an assignment only.
All injuries MUST be reported to SHA immediately at 440-243-3700.
You must complete an SHA accident report within 48 hours.
The Ohio Legislature has revised the Ohio Workers' Compensation law to invalidate injury
claims when they are caused by controlled substances not prescribed by a physician,
therefore mandating that upon any work related injury, you are REQUIRED to submit to a
drug test immediately. Failure to do so will result in your employment being terminated.
DRUG TESTING

Employees will be required to submit to a pre-employment drug test. The cost of this drug
test is a one-time fee of $45.00 and you will be reimbursed upon the completion of 40hours worked at your assignment.
If you fail your drug test, or do not complete 40 hours or work, you WILL NOT be
reimbursed.
