Web Junior Vets On Call is going to be an amazingly, fun experience. But it won’t be a walk in the park! Can you handle pressure? Can you overcome your fears? And most importantly, are you up for the challenge of a lifetime? If the answer is yes and you want to be a Junior Vet, here’s what you need to do … 1. Check to see if you’re eligible We’re looking for volunteers who will be between the age of 9 and 14 on 23rd March 2015. You may be called to help on more than one day between 23rd March and 15th May 2015. Unfortunately we cannot accept applications for anyone below the age of 9 years old because of the nature of the veterinary work involved … sorry! 2. Get permission Make sure you have permission from your parent/guardian before sending us any personal details. They’ll need to fill in a form telling us that they’re happy for you to take part too. 3. Fill out the attached application form We need to know about you and why you think you should be a member of the Junior Vet On Call team. So fill in the application form and get your parent/guardian to fill in and sign the consent slip. You’ll also need to include a recent photo of yourself. Sorry, but we won’t be able to return the photos. 4. Post us your application form The closing date for entries is Friday March 20th 2015, so make sure you get your application forms and signed consent forms to us by then! The address to send them to is: Page 1 of 9 Web So what happens next? The Junior Vets On Call production team promise to read every application form received before the closing date. You should know that we get lots of people applying to be on the programme – and sadly, we can’t say yes to everyone. Please accept our apologies, but we’ll only be getting in touch with you if you make it through to the next round of selection. The purpose of the casting process is to find children to take part in the series. The selection panel are looking to choose a diverse mix of children from across the UK. We will be considering every application equally, regardless of race, religion or disability. The panel’s decision is final. If you haven’t heard from us by 20th April 2015, then we’re afraid your application probably hasn’t been successful this time. If you do make it to the next selection stage we’ll give you a call. We’ll need to talk to a parent or guardian first – and then we’ll want to chat to you a bit more about why you think you’d be great on the programme. Right, let’s get started... Page 7 of 9 Web PARENTAL/GUARDIAN CONSENT FORM For the parents / guardians of children applying to take part in Vet School for CBBC Your child would like to apply to take part in the new series of Junior Vets On Call, a True North production for CBBC. If successful, your child will need to be available on one or more days between between 23rd March and 15th May 2015. For the application to be considered, all children need to provide a completed questionnaire, a signed parental/guardian consent form and a recent photograph, which should be sent directly to True North Productions. Due to the large numbers of applications we expect to receive for the programme, we cannot guarantee that everyone who applies will appear on this (or any future) series - and only those who make it to the next round will be contacted by the production team. If your child’s application does make the initial shortlist, we will call you and not your child, to discuss the next stage of the selection process. Having had a chat with you, we may then ask to speak to your child. If selected, your child’s inclusion will be dependent on us being able to obtain appropriate consents (e.g. local education authority). The information provided on these forms will only be used for the purposes of selection for inclusion in a CBBC programme. All details will be held securely and will not be passed to any third parties outside of True North and CBBC. They will not be used for any other purpose without your permission.
