PARENTAL CONSENT FORM
To the Parent or Guardian:
Your child would like to apply to take part in a CBBC series. The show is currently
scheduled to be transmitted on the CBBC Channel sometime between December
2015 and April 2016.
For the application to be considered your child must be aged between 8 and 14
years old and we need to receive both a completed questionnaire and signed
parental consent form. These should all be returned as soon as possible for us to
consider the application.
The applicants we wish to consider for the programme will then be contacted by a
member of the production team. If you have not heard from us by October 2015 then
unfortunately it means that your child has not been selected. The closing date for
applicants is Friday 28th August 2015.
All information supplied in this application form will be treated as confidential. It will
be stored securely and will be used only in relation to this production and will not be
used for any other purpose without your permission.
Involvement in this programme will depend on obtaining the necessary permissions
or licence as required. Applicants must not be employees of, or close relatives of
employees of, the BBC. All applicants must be UK residents.

If successful we will call you and not your child for selection to the next stage. The
