Position Description:
Children’s Place, Inc., a non-profit in Aiken, SC, currently has three Family Service Worker Positions open. Family Services
Home Visitor position is full-time 40 hours a week and offers benefits after a probationary period. The FSW position works
with families referred to a community- based Child and Family Development Center by the Department of Social Services for
parenting support to prevent child abuse and neglect or working with behavior health services for families whose children are
demonstrating behavior problems. The FSW must work with families to develop treatment plans, make referrals for services,

Description of Duties:
Under general supervision, provides evaluation and therapeutic services to mental health consumers in an outpatient clinic.
Responsibilities include completion of complex psychosocial assessments, treatment team participation, treatment
planning/monitoring and discharge planning. Provides individual, family and group counseling to consumers and their families.
Work in community/school environment may be required.
Minimum Qualifications:
LCSW, LMSW, LPC, LAPC, LMFT or LAMFT with 2 years of experience OR Master’s degree in social work, psychology, or
counseling which must include a supervised clinical practicum and must be registered with the Georgia Composite Board of
LPCs, LCSWs and LMFTs to seek associate license. Two years of full time experience involving direct client contact such as
therapy, case management, intake assessments and crisis screening.
A criminal record background check and drug testing is required upon appointment. If appointed, applicant will be
subjected to a Fingerprint Based Background Check of the Georgia and Federal Criminal History Record Databases.

Job Description: Responsible for providing environmental supports and care coordination considered essential to assist a
person with improving his/her functioning, gaining access to necessary services, and creating an environment that promotes
recovery as identified in his/her Individual Recovery Plan (IRP). The focus of the interventions include assisting the individual
with: 1) developing natural supports to promote community integration; 2) identifying service needs; 3) referring and linking to
services and resources identified through the service planning process; 4) coordinating services identified on the IRP to
maximize service integration and minimize service gaps; and 5) ensuring continued adequacy of the IRP to meet his/her
ongoing and changing needs.
