LOUIS WAGGONER
Our PTL family was saddened by the death of Louis Waggoner on December
30th. Louis worked in our Safety Department as a Driver Manager. He
started as a Company Driver with PTL in February 2001. Louis received
many honors including several Driver of the Month awards and Safety
awards. Louis was well liked by all and he will be missed.
I remember stopping to visit Louis at his home and
while we were sitting at the kitchen table he asked me
if I would like some dessert. I said sure. He then got
a mason jar from his cabinet and a small bowl from
the cupboard. He spooned out a brown powder into
the bowl and added a spoon of water. He then mixed it
up and set it in the microwave for a minute. When he
removed it, there was a perfect small cake or brownie.
He said it was simple, just cake mix and water in correct
amounts. It was very good. Louis was a heck of a cook.

Every time I saw him he always had a smile and a
hello. He was a very good person with a wonderful

Louis Waggoner was a sweet, funny, and honorable
man. He was a great cook, and he enjoyed sharing
recipes of all his favorite dishes (and there were a bunch
of them!). Thanks to him, I have learned to enjoy wine
a different way, as Red Wine jelly. I could always count
on Louis to be honest in any situation, even if I didn’t
particularly like what he had to say. He loved animals
and became a foster “dad” to several strays along the
way. His sweet smile and humorous personality will
be truly missed by many here at PTL. Kelita Dunn,

I remember many humorous conversations with Louis
that made my day a little better. He always took the

Remembering Louis Waggoner
There are so many things that I will forever miss
about Louis. He called himself, ‘a wealth of useless
information’. He knew a little about a lot, but not a lot
about anything. Louis was such a debater…whether he
really did agree with you or not, he made a point always
to disagree LOL! And with his useless plethora of
knowledge, somehow he would always win! I still catch
myself coming in to work in the mornings, seeing his
van, and, for a second, looking forward to seeing him.
As mean and heartless as he could be at times, deep
down inside he was one of the kindest, caring people I
knew. Although he would choose to disagree, he truly
cared about all of his friends here at PTL. He could
make a new friend in nothing flat and was a person you
just couldn’t help but love. And it cannot go unsaid, he
was the ONLY man I knew that could say exactly what
he was thinking, ALWAYS bad (HAHA), and not get
slapped. He got away with so much, just because he
was “Louis”. I know he’s in a better place now and not
hurting anymore, but for such selfish reasons, I so, so
badly wish he were still here with us. RIP buddy. Love,

When I started working here Louis was always picking
on me about something. I was told about the birthday
cake that was given to him one year. It was a foot that
said someone “toed” me it was your birthday. Well that
just cracked me up and I found out about how he had
lost his toes. He had such a good attitude about this and
everything that went on with him. One day I came in
and he was saying something about me having on flip
flops so I fired back at him that he was only jealous that
he couldn’t hold a flip flop on. Well he went on and on
and on about the flip flops so I told him he had better
steer clear of the ocean cause they might mistake him
for flipper with his one toe. From that day on he was
not Louis, he was Flipper...I did not know him as long
as everyone else but I loved him dearly and will miss

Louis always knew what to say to put a smile on your
face, even on the worst of days. He definitely had a way
with words. I’ll miss him, that is for sure and I am so

When Louis started driving for PTL, I was his Fleet
Manager and he was indeed one of my best drivers.
A driver who would do anything you asked of him
without a bit of complaint. I remember saying “I wish I
had a whole fleet of Louis’s” and I sure did mean it!! He

about many things but mostly cooking and our dogs.

Fuel Bonus Winners - 4th Quarter 2013

FUEL BONUS PROGRAM REQUIREMENTS
At the end of each quarter fifty of our top drivers will receive a $1,000 fuel bonus. The requirements of this program
are straight forward and are designed to improve PTL’s fleet miles per gallon and equipment utilization.
To qualify a driver must:
1. Achieve a miles per gallon ranking in the top 20% for the truck/engine combination that he/she is driving.
2. Have at least 24,000 “paid” miles for the quarter. “Paid” miles are your payroll miles and are reported on your weekly
check stubs.
3. Remain in the same assigned truck for the 24,000 miles. (Exceptions will be made to this requirement if a driver is
forced to change trucks through no fault of his/her own.)
4. Have an out-of-route percentage of 10% or less.
5. Be employed at the time the fuel bonus is paid.
This program is designed so that all drivers will have an equal chance of earning the fuel bonus. Each drivers’ miles
per gallon will be ranked against other drivers driving the same type of equipment. Those of you driving older equipment
will be competing only against other drivers driving the same type of equipment. Those of you driving our new tractors
will be competing only against other drivers driving the same type of new tractor.

West Memphis Terminal
Greetings from the West Memphis Terminal!
I just want to welcome everyone back from the
holidays. It was a much needed break. The wifi here
at the West Memphis terminal is operational now.
If you have any problem logging on to the training
website, get with someone in the Operations trailer.
On a different front, there is construction on I-240. It
is located on the south loop between exit 16 and exit
11 for I-40. Use the north loop if possible. Remember
the speed limit on the loop around Memphis is 55
MPH. Welcome back from the holidays, let’s get
some miles and do it safely!

The Operations trailer is on the north side of the inspection lane. We are
here Monday through Friday 0700 CST until 1700 CST. There is a person
here Saturday and Sunday 0700 until 1600. We have replacement fuel cards
and camera accident kits available. We also have a fax to send in safety
paperwork or bol for loads if needed. Monday through Friday we can get
you to the clinic for physicals. We have a dog kennel on the premises.
Our Inspection Lane is open 24/7. You can get CSA inspections done right
in the lane. We can replace spare tires. We have Transflo sheets, trailer
seals, log books and blank Comcheks.
Our shop is open Monday through Friday 0600 CST until 2359 CST. It is
open on Saturday and Sunday 0600 CST until 1800 CST. We can handle
most repairs on trucks and trailers. We service APU’s as well.
Directions: I-40 east or west. Take exit 275 go north on College Blvd. We
are the 2nd building on the left. Loaded trailers go on the center line on the
yard. The empty trailers go on the west fence line. Out of service trailers go
on the north fence line. Bobtails need to park in front of the shop on the
east fence line. There are 3 dumpsters on site. You can dispose of trash in
any of these receptacles. We have a break room with a TV, recliners and a
bathroom. We have a courtesy car to use in West Memphis if you need it
to go get supplies or showers.

The cold weather is just about done with around here and as much as I dislike the warm/hot weather, this time around
it’s going to be a nice welcome.
We have been testing the wifi for Vertical Alliance online training just about everyday now. We want to make sure we
have it working to perfection so all of our drivers can test without any problems. Any drivers who may still be a little confused
on how to log onto and get to the wifi on your Qualcomm keyboard don’t hesitate to stop by our office and ask for help. It just
takes about 10 minutes and we will go step by step until you feel comfortable that you can do it on your own. This way you
can stop at any terminal and test when needed.
We have great news, about 6 months ago we lost 2 lanes with good miles from El Paso, one going to North Carolina
and the other to Indiana. Well, we have them back again and it’s been great. Both are drops at the destinations and on some
occasions are a reload back out from there. Great loads to keep you busy for the week and/or weekend.
I want to give a shout-out to Henry Hernandez in truck 19576 for leaving out a few hours early from his hometime to
cover a load for us to make delivery on time. Even though it was just a few hours early, it tells me that we can count on him
when called upon. Thanks Henry!! Well that will be all for now, until next time, hasta la vista!!

We are open from 5 am through 6 pm, Monday-Friday and Saturday from 730 am until noon. El Paso is in the Mountain
time zone. We are closed on Sunday.
We have supplies available including white and yellow Transflo cover sheets, trailer seals, log books, blank Comcheks,
camera accident kits and replacement fuel cards.
We can fax your signed bills upon request
Parking is very minimal. We only have 20 spots reserved so most drivers head out to the truck stop.
We share our dumpster with A.N. Webber and they have requested that we only throw small amounts of trash in it. Do
not clean your trailer out into the dumpster.
Directions: From I-10 east/west – exit 34 (Americas Ave.) - go north to Rojas Dr. and make a right – go up to a 4 way
stop and make a left on Mercantile Avenue. We are on the left hand side, A.N. Webber/PTL.
After 6 p.m.: A.N. Webber will close its gates but the shop will still be open. You will need to honk your horn so they can
come out and open the gate for you. When the shop closes at 9 pm (Monday-Friday) the security guard will take over and
will be by the gate to open it, if he is away from the gate you will need to honk your horn. On Saturday the gate is open
from 7 am until noon. When the gate is closed you must honk your horn. We have a lot of honking here.
Watch for a lot of road construction when you are headed to the terminal. We do have a Pilot Truck Stop about 3 miles
east from I-10 (exit Horizon) and there are plenty of motels near the terminal.
A.N. Webber does our maintenance. We swap tires, do trailer repairs and very minimal truck repairs. If you need repairs,
contact Road Maintenance.

entrance on right. There is a PTL red white and blue sign, WW Rowland Trucking sign and a Drive sign as well.
• There is a small truck stop behind the office which you can walk to or there is a Flying J, Pilot and TA at mile
marker 13 off of 35. If you are going north on 35 exit on 12b and stay on the service road.
• Restrooms and showers are open here 24/7. It is small green building to the right of the office.
• We do some minor truck repair, along with APU’s, Monday-Friday from 8 a.m. -6 p.m.

Employee Stock Ownership Plan (“ESOP”)
Questions & Answers
As new participants in the Paschall Truck Lines, Inc. Employee Stock Ownership Plan, I am
sure that you have many questions that you would like to have answers to. We are going to
attempt to answer a few of these types of questions in our newsletter on a regular basis. If you
have a question, please send it to Chuck Wilson ([email protected]) or Russell Long
([email protected]) so that we can address any specific question that you, as participants
in this new retirement plan, might have in the next issue. If you have a question, I am sure
many others do as well so your questions are welcome and helpful.
1.
What is an Employee Stock Ownership Plan? It is a Qualified Retirement Benefit
Plan that is sponsored by the employer and its primary (or sole) investment is Company
Stock/Employer Securities. The Plan is a stock bonus plan that enables employees to share in
the growth and prosperity of the Company and allows them to accumulate capital for their
future retirement.
2.
Are employees allowed to contribute to the Plan from payroll funds or from
outside personal investment accounts? No. All contributions to the Plan are provided by
the employer only.
3.
Who is eligible to participate in the ESOP? You must be at least 21 years of age, be
credited at least 1000 hours of service in a year/12 consecutive month period (“eligibility
period”) and not be a party to a collective bargaining agreement, a leased employee, an alien,
and independent contractor or a temporary or technical employee. If you satisfied the above
requirements as of the effective date, 01/01/2013, you became a participant of the ESOP as of
that date.
4.
How do participants in the Plan receive stock? Stock was purchased by the ESOP
Trustee in 2013 and a loan was taken out by the ESOP Trustee in which to purchase the stock.
As the loan is paid, of which a payment is due annually, a calculated number of shares are
then released from collateral and allocated to the eligible participants in the ESOP. This loan
is paid with funds contributed by the employer on the Plans behalf and can be classified, at the
Company’s discretion, as a regular ESOP contribution or a Matching 401(k) contribution.
5.
How are shares of stock allocated to my participant account? Allocations are
made to eligible participants as of the last day of the Plan Year (12/31). Contributions to
the Plan and Trust can be made in one of two ways: Matching Contributions or Regular
ESOP Contributions. If the contributions made by the Employer to the ESOP Trust are from
Matching Contributions, then your allocation is based upon your compensation as it relates
to all eligible participants compensation. For Example: If you make $40,000 a year and total
compensation for all eligible participants in the Plan is $400,000, then you would receive

10% of the assets allocated in that Plan Year. If the contributions made by the Employer to
the ESOP Trust are from Regular ESOP Contributions, then 50% of your allocation is based
upon your compensation as it relates to all eligible participants compensation and the other
50% of your allocation is based upon Years of Employment as it relates to the Total Years of
Employment of all eligible participants.
6.
What is the vesting schedule for the ESOP? If a participants employment terminates
prior to their Normal Retirement Age or for any reason other than Death or Disability, a
participants will be entitled to receive the “Vested Percentage” of their account based on their
“Years of Service” and will be determined as follows:

“Years of Service” is the sum of the Plan Years beginning on or after the effective
date in which the participant has attained age 18 and has been credited with at
least a 1000 hours of service.
7.
What are forfeitures? Forfeitures are when a participant leaves prior to reaching their
5 Years of Service. For example, if a participant has 100 shares in their account but only has
3 Years of Service, then the participant will only be 60% vested. They will only be entitled to
receive payment on 60 shares. The other 40 shares will be allocated, at the end of the Plan
Year, to active and eligible participants.
8.
Many Plan Sponsors that adopt an Employee Stock Ownership Plan have employees
that begin to question that now that they are beneficial shareholders in this ESOP and
have beneficial ownership in the form of Company Stock in their account, what changes?
The first change that typically takes place is that an employee ownership culture takes
hold. Although every company and its respective culture can be unique, certain practices
can contribute to the success of a strong employee culture, which in turn creates motivated
employees, business success and increased value for both customers and shareholders. A
phrase that is great to remember as an Employee of an Employee Owned Company is “THINK
like a customer, ACT like an owner!”

Online Training
ATTENTION DRIVERS: Paschall Truck Lines, Inc. utilizes an online training program to help PTL
drivers remain among the best and safest drivers on the road today. Hopefully these training tools will
help reduce the potential for accidents and make everyone safer.
How to Get Started
Step 1 - Getting to the Training Website:
You may access the training website on any device that has internet access with a web browser; this
includes PC’s, laptops or a mobile device. You can even connect via your Qualcomm’s web browser option.
The Qualcomm option works when you have connection to “OPEN WIFI”. You will know this when the
WIFI antenna icon is lit with no lock attached.
If no open wifi is available you will get a message that reads “TERRESTRIAL BROWSING HAS BEEN
DISABLED”. You will be unable to use the Qualcomm if you receive this message.
• Once you have established internet connection enter http://paschall.infinit-i.net in your website
browser. After the website loads, click on the “Training Center” link. Follow Steps 2 and 3 to complete
the required training.
• To access via Qualcomm, insure you are in an “open WIFI” area.
• Click on the Web Browser, this takes you to PTL home page.
• Click on Favorites which is the star icon and go to Paschall Truck Lines Inc. Online Safety Training.
Step 2 - Enter your User Name and Password:
Type in your user name (your driver code) and password
(Paschall).Then click on “Login”. (Answer yes to any
pop up boxes you see during the log in process.)
Step 3 - Select Topic in the Classroom Section:
There will be an identity check. Check the box when you
see your name on the screen. Select “Go To Classroom”.
Click on the title you want to view. The video will load
and then begin playing.

A Test option will appear after the video.
Answer all questions to complete training.
Note: To re-take the test you must watch the video again.
We will be sending one video per month to the entire
fleet. You will have 30 days to complete the required course. The videos run anywhere from 2 minutes to
as much as 18 minutes depending on the topic.

is available at our terminals in Murray KY, West Memphis AR, Indianapolis IN,
El Paso TX, Laredo TX and Brownsville TX. Note: If you have trouble with Wi-Fi
at our terminals, contact the Terminal Manager or your Fleet Manager.

As a Company, we need more Driver Trainers. Since we want all of our drivers to attain
the success they deserve, Kevin Fox and I sat down and discussed what can be done to attract
and retain Driver Trainers. One of the most important factors that makes a successful driver
is the Trainer. Your Trainer is where your first impression of PTL is formed. You get over the
road knowledge that cannot come out of a book. Some of our most successful drivers are
Trainers, and, in turn, their Trainees become successful drivers.
To encourage Drivers to become Trainers, we have revised our compensation plan and
there are now two options available. It is your choice.
We have some great trainers here at PTL, so we plan to bring you some “Profiles of
Success”. We are going to start with Regeil Staples in 16473.
Regeil is putting his six plus years of experience to good use. Currently a trainer here at
PTL, Staples has figured out a system that is working great for both his family and himself.
He is able to make good money while over the road and not worry about the day to day
activities going on at home. In one year as a trainer, Staples has added an additional $15,000
to his paycheck.
His family is behind him in his choice to be a truck driver. His wife understands what the job entails and does not
pressure him to be home constantly. His son, 5, thinks it’s cool that dad is a truck driver. He looks forward to helping
clean out the truck and beep the horn when he comes home. His family is not living paycheck to paycheck. His wife
works, and Staples spends every moment while over the road trying to make more money.
Staples was very hesitant to become a trainer in the beginning. He was asked if he was interested and he immediately
turned down the position. He said he didn’t want to have a rookie in the truck with him. After talking it over with his
wife, she changed his mind. She said, “Somebody took a chance on you and let you in their truck. You didn’t know
how to change gears, down shift, upshift and somebody took a chance on you as a rookie and helped you.”
Once convinced, he started viewing training as a business. “On average, it’s a $200 a week raise. I don’t do bad
solo, but the week after I get my upgrade bonus I bring home $1,800. It’s usually $1,000 to $1,300 a week after taxes.”
Staples carries that same principle into his method of training. “I’m gonna put you in my truck and treat you like
you’re not a student. You’re not a student, you’re a co-partner. Don’t treat them like a kid, they’re not kids, they’re
adults. I give them immediate access
to everything - here’s the key to the
truck.”
Staples says it has nothing to do
with power and being in charge. “I’m
in the business of making money.
It’s a job. If the truck isn’t moving,
you’re not making any money.”
He shows each of his trainees his
payroll preview. He tries to explain
to them about setting PTA’s and ETA’s
correctly to maximize drive time and
make the most money. He also tells
them about freight movement and
customers.
Staples stays in touch with his

call and ask any questions they run
across. “Some of my drivers call me and say, ‘I made $650 this week! I was working two jobs making $450 a week,
that’s a $200 increase!’ and I tell them, yeah, that’s great, and if you train, it could be more.”
Staples typically stays out in the truck for around 35 days. During this time, he is able to train two trainees. This
keeps him out when freight is good and gives him the best home time. When he has a trainee that is worried about
being away from home for so long, he tells them “Think of the financial side. You’re working to make money. You can
go from making $20,000 to a potential of $80,000 a year! Be positive!”

AUTOMATIC SLACK ADJUSTERS
Commercial Brake Systems manufacturer Bendix recently reported in a news release that the reason 49.6 percent of the
vehicles placed out of service during Roadcheck 2013 had to do with brake adjustment and other brake violations. The number of
vehicles out of service for brake issues illustrates the importance of the braking system including brake stroke measurement in any
pre-trip or wheel end inspection.
We recognize that the majority of drivers will not slide up under a trailer to check brake stroke, but awareness of what the tech
or inspector is doing when looking at your brakes can certainly be beneficial. However, some drivers
would prefer to look at their brake systems and catch defects before a DOT inspector catches them, so
a very brief primer on brake stroke might be helpful to some.
To check the brake chamber stroke on each wheel end, measure the distance from the chamber
to the pin with the brakes released and then again with full brakes applied. The difference between the
two measurements is the brake stroke. For our 30/30 chambers, a measurement of 2¼” will result in a
violation write up and 2 brakes with that measurement or less will result in an OOS citation. A defective
slack adjuster is only one potential cause of a brake stroke being out of adjustment. If a wheel end with
an automatic slack adjuster is out of adjustment, we have to figure out why the brake is over the stroke
limit and fix the problem in the foundation brake system.
Brake stroke is a function of the slack adjuster and there is a considerable amount of misunderstanding surrounding the slack adjuster. PTL’s entire fleet is equipped with automatic slack adjusters.

Anytime a brake system is noted as being out of adjustment, never simply adjust the slack adjuster and
do not let a technician or shop do it either. By doing so, the auto slack has been permanently damaged.
Some drivers still misunderstand the auto-slack concept and try to manually adjust their own brakes. One retired trooper with
over 19,000 roadside inspections under his belt is still surprised with this lack of understanding. He found many believed they could
still manually adjust the slack and everything would be OK. He noted that many times when he told a driver a brake was out of
adjustment, the first thing the driver did was grab a wrench and start trying to adjust the slack. That just doesn’t work.
I want to re-emphasize again, never adjust an automatic slack adjuster. The only time this might be considered is to get the
vehicle to a shop for repair by a professional technician. If this happens, always advise OTR Breakdown maintenance of the
situation and if you are aware that an auto slack has been adjusted, let your maintenance coordinator know so they open a repair
order for that wheel-end brake system to find the real problem for the out of adjustment condition.
One could question why having 2” in stroke is dangerous. The answer is brake reserve. Take the example of going down a long
mountain grade and using the brakes to maintain speed and spacing with other vehicles. Brake drums will heat and expand which
can increase the brake stroke up to ¾” inches. If that happens, the brake pedal could go to the floor and it would be very easy to
have zero brakes in such a situation.
PTL invests considerable time, energy and training to make sure your vehicle’s brakes are in top operating condition. Our
technicians are likewise committed to ensuring the brake systems on our vehicles perform their function as they are designed to
do. Properly installed and maintained slack adjusters should never need adjusting after the initial installation set up. When an out
of adjustment wheel-end brake is encountered, it is almost always a problem in the foundation brake set-up.

TIPS FOR BETTER FUEL ECONOMY
SPEED COMES AT A PRICE
• 65 MPH vs 60 MPH = 6.4% decrease in fuel economy
• 70 MPH vs 60 MPH = 10.9% decrease in fuel economy
• 60 MPH vs 45 MPH = 17.3% decrease in fuel economy
PUSHING AIR ISN’T EASY
• Between 55 and 60 MPH, 50% of the fuel burned is used to overcome air resistance; the remaining 50% of
fuel consumed is used to move the load. The percentage of fuel needed to overcome air resistance increases
dramatically as vehicle speeds increase.
DRIVER TIPS FOR IMPROVED FUEL ECONOMY
• Slow Down – Fuel economy is improved when drivers maintain 60 MPH as their cruise speed.
• Cruise Control – The use of cruise control will typically enhance fuel economy performance.
• Maintain Optimal RPM – Each manufacturer’s engine has unique torque curve. Drivers should operate at the
appropriate RPM to maintain speed and maximize fuel economy as recommended by the engine manufacturer.
This range is typically between 1,200 and 1,600 RPM’s.
• Minimize Idling – Idling consumes between 1.0 and 1.2 gallons per hour depending on your unit’s idle speed.
With our trucks having APU’s, this should not be an issue.
PROPER MAINTENANCE WILL INCREASE FUEL ECONOMY
• Maintain proper tire inflation of the tractor and trailer.
• Replace fuel filters at regular intervals.
• Keep all axles properly aligned to minimize rolling resistance.
• Use good synthetic lubricants in the transmission and axles.

Bonjour Drivers! Welcome back to NaviJoey’s Corner. Wow! Has this been the coldest winter since woolly
mammoths were roaming the interstates or what?!? I’m from Philly where we are used to the cold but this is… this
is cold enough to make Frosty take a trip to the Florida Keys. Let’s talk about a few adjustments we have to make
as drivers when temperatures reach this low, specifically regarding fuel.
For any of you who are not aware, biodiesel freezes at 32o Fahrenheit. This is
very important to know in the winter when places like Minnesota gets to minus 30o
Fahrenheit, a.k.a. “Why would anyone ever live there?” Here is what you as PTL
drivers need to know about your fuel:
First and most importantly, make sure you are fueling at the fuel stops we are
giving you on your fuel solutions. PTL’s fuel department has purposely blocked all of
the high biodiesel fuel stations that we possibly can. By drivers fueling at the PTL fuel
stops, you will decrease the amount of biodiesel that goes into your fuel tanks hence
reducing the amount of fuel that is capable of freezing. It may not seem like much of
a difference fueling at one fuel stop opposed to another, but it very well could make
the difference of your fuel filter gelling or not.
Second note to our drivers, make sure that you are adding anti-gel additive to
your tanks anytime that you are heading north of Interstate 40. Before adding any
anti-gel to your tanks or before idling your truck in the extreme low, we would like
you as drivers to check with Road Maintenance just to confirm exactly what they
would like you to do for the weather conditions that you are heading into.

If you are going into temperatures that are going to be 20 degrees or below, please

make sure that your fuel is treated with anti-gel. Anti-gel can be purchased on your
Comdata fuel card as an additive. If you are going to be in temperatures less than 0, it is time to idle your truck.
Before doing this though, please reach out to Road Maintenance and have them approve the idle time and put a
message on your Qualcomm for you.
This month we lost a member of the PTL family. Mr. Louis Waggoner, a very dear member of our Safety
Department passed away. I would like to extend my condolences to his family. We will miss you my friend, R.I.P.

• Backing accidents are the most frequent accident at PTL.
• Always use a spotter. Trainers should always spot for
Trainees.
• P2 Training drivers always spot for each other.
• Do not be afraid to execute several pull ups.
• Always make sure the area you are backing into is clear
of obstacles.
Pictures are Worth a Thousand Words
If you are involved in an accident/incident, remember that pictures can protect yourself and the Company
(even when at fault).
•Always take pictures.
•Make sure you get accident cameras at terminals.
•If you find yourself in a situation with no accident camera, use your cell phone.
•Always report all accidents/incidents to the Safety Department which is staffed 24/7/365.
SUBMISSION OF CITATIONS AND INSPECTIONS
Some drivers may not be aware of the policies and requirements for submitting Citations and
Inspection Reports within 24 hours of issue.
The FMCSA states in regulation §396.9 (d) (1) that the driver of a Commercial Motor Vehicle shall
immediately mail, fax or otherwise transmit the inspection report to the carrier within 24 hours.
With that being said, always make sure that the copies that you are sending to the carrier are clear
and legible. If, during the inspection, you have received a citation for any type of violation, you are also
required to submit a copy of the ticket (front and back) with the inspection report.
There are a couple of ways that you can submit these in a timely manner:
1. Fax to 270-753-1528 (ATTN: Gene Fielder/Tabitha Jones) or
Fax to 270-753-1730 (ATTN: Dana Sills)
Either fax number will assure that the appropriate persons who handle these time sensitive
documents will receive a copy.
2. Fax or scan both sides of the citation. Be sure to sign your name on the citation where required.
3. Transflo or Trip-pak scan the documents with a yellow log cover sheet. Then send message
21 and place an ‘X’ next to the number 9 over the Qualcomm with the 16 digit confirmation code. The
code or number, will be a combination of letters and numbers i.e. LV01-2ab3-d45e-6fg7. There may or
may not be any dashes.

CSA Maintenance Score Card
Preventable CSA Maintenance Violations and Preventive Actions
Each month we study our CSA maintenance score to determine why we were
cited for a violation. We are looking for trends and repeated failures that we can
prevent by changing maintenance practices. At the same time, we evaluate each
violation as to whether the driver could have found the defect prior to DOT finding
it and prevented the violation with a thorough Pre-trip or Post-trip inspection.
During December we were cited for 43 violations of DOT vehicle safety
requirements. Our analysis of the available data shows that 35 (81%) of those
violations should or could have been prevented by a pre-trip inspection. Listed
below are the violations that a proper pre-trip inspection would more than likely
have prevented along with preventive actions.

Prevention Basics
Air lines cannot rub against any other part of the
vehicle. They cannot be kinked in any way. This is
easily seen by a visual inspection.
Cannot have any defect larger than what a dime
would cover. They also may not be glazed or
discolored in any manner.
A discharged fire extinguisher is a violation. Fire
extinguishers are secured in a metal mount with a
latching metal band attaching the fire extinguisher
to the mount. If the band is not latched, it is a DOT
violation.
Should be heard during a vehicle walk around with
the engine running.
Even a very basic pre-trip would have observed
leaks prior to being found by DOT inspectors.
The most common violation. All lighting should
be tested at least daily if not every time you take a
break. Either a light is observed to be working or
not working.
It is basic safety procedure to ensure the tandem
pins are properly seated through the slider rail holes
prior to moving the trailer after sliding the tandems.
Visual inspection would have noted questionable
tread depth, flat tires and exposed tread.

THINGS YOU CAN’T SEE
We have discussed the importance of seeing and being seen in the previous months. However, there are things
we can’t see that increase the possibility of being involved in an accident.
This time of the year, ice may appear in several parts of the country. The timing and location may be nearly
unpredictable. The overlapping of a cold and warm front may produce a quick layering of ice when rain in a
warm upper atmosphere falls on surfaces with a temperature below freezing. You might expect snow or sleet,
but surprise . . . suddenly the road is as slippery as a skating rink. Listen to weather forecasts and check mirror
brackets for the formation of ice. Temperatures dropping below freezing may cause icing where drainage is poor
and water has puddled on the road. Don’t forget that bridges and overpasses freeze first. Lower your speed, but do
not accelerate or brake on these surfaces.
Changing temperatures can produce fog. You’ll see the fog . . . but in many cases, nothing else! Do not drive
faster than you can get stopped in the distance you can see ahead. Realize in some instances, it is wise to get off
the road and wait for conditions to improve.
Blind spots while backing may hide objects. Get out of your truck and look before backing and get out as often
as necessary to check. Back from your sight side and avoid blind side backing whenever possible.
Blind intersections require slowing to a speed that allows you to look left, right, and left again before proceeding.
Realize railroad crossings with multiple tracks can create a blind intersection. Do not begin to cross the tracks as
soon a train clears without waiting to see if another train is entering your path from another set of tracks.
Some things you can’t see may be seen if you slow down. Others may require you to get out and look or in the
case of ice, slow down when conditions may cause icing.
DON’T KNOW ??? DON’T GO !!!
Ice
• A warm front on top of a cold front may produce freezing rain.
• Listen to weather forecasts.
• Check mirror brackets for ice.
• Poor drainage during the day may produce ice at night.
• Bridges & overpasses freeze first; slow down; don’t brake or accelerate.
Fog
• Do not drive any faster than you can stop in the distance you see ahead.
• It may be wise to wait for conditions to improve before proceeding.
Blind spots
• Get out and look before backing. Recheck as often as necessary.
• Back from your sight side; avoid blind side backing.
Blind intersections
• Slow to a speed that allows you to look left, right and left again before proceeding.
PTL U-TURN POLICY
UNDER NO CONDITION IS A “U-TURN” ACCEPTABLE
ON ANY TYPE OF HIGHWAY BY PTL DRIVERS.
Do not endanger yourself or other motorists by making a
U-Turn on any highway. Violations are subject to termination.

ODOMETER READINGS
It is very important that we get an
accurate odometer mileage reading every
time you fuel. At the pump, after you swipe
your fuel card and enter your driver code
you are asked for an odometer reading. We
need the lifetime miles on the tractor not
trip miles or other information that might be
displayed.
Below are examples of what you might
see on your International or Freightliner
truck. The smaller mileage figure with TRIP
MI or TRIP MILES is NOT the number we
are looking for. These are miles that you can
reset from time to time. In every case the
largest mileage figure you can find and the
words MILES or MI IS the number we are
asking you to record. Naturally this mileage
figure will be larger than the number you
entered at your previous fuel stop. Do not
input the decimal or tenth of a mile figure.

about employees should be addressed to Russell Long at Ext. 316 or Charlotte Cook at Ext. 344. The type of questions
include, but are not necessarily limited to, home address, telephone number, marital status, work schedule, vacation
status, work location, whether a person has life or health insurance, 401(k) information and non-driver reference
checks. We do not release most of this information without written authorization from the employee (or ex-employee)
or in response to a subpoena.
Over the telephone information is typically limited to dates of employment, full or part time status, and title.
It is important that you make sure to keep a current address with PTL. Contact Brandi McDaniel at Ext. 305 if you
are a non-driver or Betty Kell at Ext. 343 if you are a driver to update your address. If you need to clarify information
on W-2 forms this also falls into the privacy category. We do not release information or make any changes except
when speaking to the employee. Therefore, contact Brandi McDaniel for non-drivers and Betty Kell for drivers about
any W-2 problems you have.
Lending institutions trying to verify non-driver employment for loan purposes should be forwarded to Brandi
McDaniel at Ext. 305 and for drivers to Betty Kell at Ext. 343. They, too, will usually require written authorization
from the employee and a form either faxed or mailed from the lending institution.
