“YOUTH IN CHRIST” SUMMER PROGRAM
SUMMARY
What We Have to Offer
Our ‘Youth In Christ” Summer program is designed for children ages 5-16. Children are engaged in devotion and bible lessons
each morning, followed by Body Gospel (exercise your faith) and educational instruction in the areas of reading, language arts and
math. The afternoon sessions consists of speakers, arts and crafts, baking activities, as well as educational and fun field trips.
It is held Monday through Friday each week during the month of July, beginning Monday, June 29, 2015 and ending Friday, July
24, 2015. Doors will open at 7:30 am, and all children should be picked up by 4:30 pm.
Breakfast and lunch will be provided for your child on Monday, Tuesday, Wednesday, and Thursday. However, on Fridays, please
provide your child with funds for breakfast and lunch, because we will be traveling out of town.
Our volunteer staff does an exceptional job of working and caring for our youth as well as chaperoning them on field trips. We
are truly blessed to have volunteers who are committed and dedicated to our Youth Ministry. We also would like to thank our
True Light Family for your prayers and continued support.

Bring your bible each day.
Appropriate clothing – “Youth In Christ” t-shirt each day with knee length shorts, jeans or khaki pants, and clean
tennis shoes and socks (No sandals).
Participate in all activities.
Be respectful to adult leaders, chaperones and other children.
Profanity, swearing or any abusive language is prohibited.
NO electronic devices are allowed (e.g. cell phones, media players, cameras etc.). Children will be allowed to contact
their parents or guardian if there is a need.
Remain with your assigned group at all times.
Emergency contact person must show picture ID. (i.e. drivers license)

By signing below, I am giving consent that my child may participate in any True Light Baptist Church “Youth In Christ” summer weekly
programs and camps (ages 5–16). In consideration of my child’s participation in this programs, I, and our heirs and assigns, hereby
forever release, discharge and hold harmless the True Light Baptist Church directors, officers, employees, representatives, agents, and
volunteers from any liability for an injury, loss or damage sustained by our child, howsoever caused, arising out of or in connection
with our child’s participation in the above mentioned program.
Transportation Consent:
I understand that some of True Light Baptist Church “Youth In Christ” summer programs will require that my child be transported
away from the base site. I understand that all safety precautions will be taken when transporting my child off site and I give permission
for this to occur.
Emergency Treatment:
In the case of illness or accident of my child, and I cannot be reached by phone, I hereby authorize True Light Baptist Church “Youth
In Christ” summer program staff to send for or seek medical assistance. I agree that in the case of an EMERGENCY, True Light Baptist
Church “Youth In Christ” summer program staff may call upon the local hospital or ambulance. All costs incurred are the responsibility
of the parent or guardian. This waiver and all information included on the attached forms which includes medical and contact
information will be valid until December 31, 2015. It is my responsibility to inform True Light Baptist Church “Youth In Christ” summer
staff in writing with any changes to this information in the one year term (January 1, 2015 to December 31, 2015).

The True Light Baptist Church “Youth-In-Christ” Summer Program will focus on skills in the areas of Math, Reading and Language
Arts.
Our academic program has been designed to help students maintain and improve skills in Math, Reading and Language Arts. It is
our desire that the Summer Youth Program is successful in helping our children return to elementary, middle and high school grades.
