Introduction
This application was developed to help high school seniors be most efficient and effective with the use of their time. In
addition, and more importantly, utilizing this application will allow seniors to spend meaningful time writing essays for
scholarships and providing other more specific information, in order to spotlight themselves as applicable for the
individual scholarships. Seniors, and for any parents reviewing this application, it is an outstanding initiative on the part
of the student to take the necessary steps to apply for any of these scholarships.
Before beginning the Uniform Application for Community Scholarships Application process, please read through the
specific Scholarship Eligibility Requirements found in Section C, beginning on Page 4.

Section A – Application submittal Instructions
1. Each Scholarship Application should be submitted in a 9 x 12 envelope.
2. A copy of the Scholarship Application along with the Scholarship Eligibility
Requirements page is required for each scholarship submitted.
3. If a scholarship requires additional information, include each of these items in the 9 x12
envelope as well.
4. Required items left out of your envelope may disqualify you for the scholarship.
5. Please thoroughly read the essay questions before composing your essay.
6.
Be sure all writing is typed, or legible if handwritten.
7. Double check your contents! It is the student’s responsibility to mail/email/deliver each
scholarship by the due date. The due dates vary and are indicated both on the Available
Scholarships List on the next page and in the Eligibility Requirements for each scholarship.
8.
Please remember, TRANSCRIPTS ARE REQUIRED for most scholarships. Please give the
Guidance Department two weeks to prepare an official transcript for your application(s).

Jessica Adkins Memorial Scholarship

Jessica Adkins was a 2006 graduate of Hamilton Southeastern High School and a 2011 graduate of Indiana
University. She died in a tragic accident on July 24, 2012. Her passion in life was to help others and be
accepting of all people, regardless of their social or economic status. Prior to her accident, Jessica had
started plans to continue her education in Social Work. To keep her legacy alive, Jessica’s family has
established a scholarship to aid an HSEHS senior with characteristics and goals similar to Jessica’s.
Seniors wishing to apply should have the following:
Characteristics: compassion, empathy, supportiveness, strong leadership and a genuine interest in working
with a variety of people from all backgrounds.
Skills: organization and attention to detail, good communication, clear and effective writing skills, ability to
work and relate to others in a group/professional setting.
The scholarship will be for $500-$1000 each year.
This scholarship can be reapplied for in subsequent years as long as the student remains committed to their
area of focus.
Required: Two Letters of Recommendation ( include the evaluation form with each letter)
• One from a teacher or administrator who knows you well
• One from a person in the community who knows you well

This scholarship is for a Hamilton County high school senior who is planning to attend
college in the fall and major in theater, drama, and/or performing arts.
In the past, this award has been from $1,000 to $1,250.
Two recommendation letters are required; include the evaluation form with each one:

One from your drama teacher
•
One from a community member, clergymen, civic leader, or employer, etc.
Write an essay about your dramatic experience and include the reasons why you are
requesting this scholarship.

A $1000 scholarship will be awarded to one graduating senior
from each Hamilton County public high school.
Criteria:
•
Plan to pursue a career in the health care field (associate or bachelor’s degree in a
medical field, such as nursing, physical therapy, lab technology, etc.) Dental hygiene
does not qualify.
•
Have demonstrated service to the community
•
Be in good academic standing (B or above GPA)
•
Financial need may be a consideration.
Essay & statements required:
•
Essay: In 250 words or less, describe what types of contribution you plan to make in
society following completion of your college/vocational training.
•

Brooks School Elementary/Luke Butler Senior Scholarship
Due 4-10-15
Program Summary: In memory of Luke Butler, Brooks School Elementary will present
two scholarships to graduating seniors from either Hamilton Southeastern High School
and/or Fishers High School who attended Brooks School Elementary for a minimum of one
full school year.
Scholarship Award: Minimum of $500.00 per scholarship.
Qualifications: Student applicants will be evaluated on the following criteria:
• Completed application
• Written explanation of post-high school plans
• Written explanation of how the scholarship will be used (200 word maximum)
• Written essay on a life lesson learned while attending BSE that impacted
the applicant’s life (1-2 page maximum-minimum font size of 10 point)
• Letter of recommendation from a current counselor, teacher or employer who can
attest to student’s character and academic potential.
Selection Process: A selection committee which consists of BSE staff members will
evaluate student applications and select scholarship recipients. The winners will be
honored at the High School Senior Awards Night. The scholarship winners will be
notified.

Section C – Scholarship Eligibility Requirements
Carrie Colglazier Memorial Scholarship
Due 4-17-15
Hamilton Southeastern High School, in cooperation with the Colglazier family, has established a memorial
scholarship in honor of Carrie Colglazier who was tragically killed by a drunk driver on June 6th, 2003. She
was an HSEHS two sport, All State student- athlete in soccer and track. She was an active member of the FFA
program and a member of National Honor Society. She was extremely passionate about her family, friends,
teammates, teachers and coaches, and took great pride in being a Royal!
The $12,000, four year scholarship is funded by an endowment through the Hamilton Southeastern Schools
Foundation and by the Carrie Colglazier Community Service Workday. The workday, which will be held in the
spring ( the Saturday before the first Saturday of spring break, March 28, 2015), includes workers from the
Girls’ Soccer and Track teams, along with members of FFA and National Honor Society. Workers are asked to
work up to an 8 hour day (8:00am-4:00pm) in the HSE community for a minimum $25.00 donation per
worker. All proceeds go toward the scholarship.

Citizen’s Police Academy Alumni Association Scholarship
Due 3-27-15
This scholarship is available to a high school senior with a minimum 2.5 GPA who
expresses a strong interest in the criminal justice field. Must be accepted to a university,
seeking to attend full-time, and possess school leadership, community commitment and
desire to contribute to society. One $1,500 scholarship will be awarded to either a Fishers
High School or Hamilton Southeastern High School senior.
Submit a typed or handwritten 500 word (or more) essay indicating why you have chosen a
law enforcement related career and how you specifically plan to meet that goal.
Two letters of recommendation (include the evaluation form with each one) are required:
o One from a school teacher, and
o One from either a school counselor, coach or community leader
The envelopes containing the recommendations must be sealed and the back of the
envelope signed by the author of the recommendation.
After the Alumni Board reviews all applications and essays, you will be notified of your
status and may be contacted to set up an interview time if you are selected. Do not make
inquiries regarding your status. You will be informed if you have been selected to continue
in the process.

For Fishers High School Seniors Only

Cumberland Road Elementary school will present two scholarships to graduating seniors
from Fishers High School who attended Cumberland Road Elementary School for a
minimum of one full school year.
A selection committee which consists of CRES staff members will evaluate student
applications and select our scholarship recipient. The winners will be honored at the
Fishers High School Senior Awards night.
Provide a letter (include the evaluation form) from a current counselor, teacher or employer
who can attest to your character and academic potential.
Qualifications: Student applicants will be evaluated on the following criteria:
Completed application
Written explanation of post-high school plans (In a sentence or two)
Written explanation of how the scholarship will be used (200 word maximum on a
separate piece of paper.
Written essay (1-2 page maximum - minimum font size of 10 point) to answer the
following question: What life lesson did you learn while a student at Cumberland

to this point?

Fishers Junior High School National Junior Honor Society Alumni Scholarship
Due 4-17-15
In 2003, Fishers Junior High School chartered the district’s first National Junior Honor
Society based on the principles of Scholarship, Leadership, Service, Citizenship, and
Character. As a chapter, we emphasize building leaders through service. The faculty
council has made it a goal for the chapter to reward members that went on to put this area
of emphasis into action at the high school level. Through the monies raised with our
chapter’s recycling program and through a portion of our chapter dues, we will be able to
make this reward a reality. The chapter will award at least two scholarships up to $500.
Eligibility:
• Must be a senior in high school
• Applicant must have been an inducted member in the Fishers Junior High School
chapter of the National Junior Honor Society
• Applicant must have been an active member of a service organization during his or
her high school years. Examples of school service organizations are: National Honor
Society, Student Council, Class Officer, Best Buddies, and Key Club. Service
involvement is not limited to school organizations.
• Applicant must have had a cumulative GPA of 3.0 or a B average
Letter(s) of recommendation (include the evaluation form with each one) from the service
organization sponsor(s). Maximum of three. Multiple letters are not as important as the
commitment and service reflected in a single letter. The letters are meant to show the depth
of your service, not the breadth.
Write a letter to the scholarship selection board explaining how your high school career
represented our chapter’s values of Scholarship, Leadership, Service, Citizenship, and
Character. The most important part of your letter will address how service helped you
become a leader in the school and/or community. Please include a current photo.

Forum Credit Union Scholarship

Program Summary: The Forum Credit Union Foundation is dedicated to supporting
education in our community. Graduating seniors who have an account at FORUM Credit
Union, or who have a parent or legal guardian with an account at FORUM, are eligible to
receive a $20,000 collegiate scholarship (paid in $5000 renewable, annual disbursements).
Applicant Eligibility: Each applicant must:

Be a FORUM Credit Union member in good standing with your FORUM Credit Union
account opening date no less than one-year prior to the time of application or have a
parent/legal guardian who is a member in good standing with their FORUM Credit
Union account opening date no less than one-year prior to the time of application.
Be intending to study full-time (minimum of twelve (12) credit hours at an accredited
college or university in a two or four year undergraduate degree program, for the fall
semester.
Have a weighted cumulative GPA of 3.0 or higher on a 4.0 scale.
Be a U.S. citizen
Be between the ages of 17 and 24.
Complete the scholarship application form and all requested supporting documents,
including the video (if applicable) by the entry deadline.
Required Supporting Documents: A copy of your most recent report card (including
cumulative GPA), a copy of your college or university acceptance letter (if available), the
signed consent form, and the link to your video (optional). The Foundation will not be
responsible for any lost, delayed, misdirected, illegible, incomplete, damaged or
undeliverable application entries, or any delay or failure to transmit, process, receive
or consider entries.
*Relatives of FORUM Credit Union officers (anyone holding the position of Assistant Vice President,
Vice President, and Chief Executive), board of directors, and supervisory committee members are
not eligible to apply for collegiate scholarships. Relatives of non-officer FORUM credit union
employees may apply for collegiate scholarships. No more than one collegiate scholarship may be
awarded each year to the dependent of a FORUM Credit Union, non-officer, employee.
Selection of Scholarship Recipients:
•
Scholarship recipients will be awarded based on a combination of factors, including the quality of the student’s application,
extracurricular activities, work experience, and future aspirations.
•
Preference will be given to allocating scholarships equally among FORUM Credit Union’s areas of service. Based on
applications received, this distribution may be adjusted.
•
No more than one collegiate scholarship may be awarded each year to the dependent of a FORUM Credit Union, non-officer,
employee.
•
Scholarship applications will be reviewed by the Foundation Board of Directors. The decisions of the Board will be final.

Announcing the Winners: The scholarship winners will be contacted directly by telephone and/or email and will then be posted on the
Foundation website. Potential recipients who cannot be contacted within ten business days following the selection process, may be
disqualified and another recipient may be selected. Scholarships will be publicly announced and awarded annually at a presentation by
the Foundation.
Disbursement of Scholarships:
•
The scholarship recipients will receive proceeds for the Fall Semester.
•
A check will be issued directly to the appropriate department/office at the educational institution once the Foundation has been
notified that registration has been completed (made payable to the institution for the benefit of the recipient).
•
Students must show proof that they are registered for at least twelve (12) credits during the fall semester (a letter of acceptance
is required)
•
The funds may be used for any educational related expenses provided these expenses can be paid directly through the financial
aid office.
•
If the total amount of the scholarship received exceeds the total expenses incurred by the recipient in completing his/her
university/college education, the remaining funds will be applied to the following semester, or forfeited.
License: By applying for this scholarship, you release and agree to indemnify and hold harmless FORUM Credit Union and the
Foundation and their employees, directors, officers, affiliates, agents, and advertising and promotional agencies from any and all
damages, injuries, claims, causes of actions, or losses of any kind resulting from your scholarship application or receipt or use of any
scholarship.
Conduct: The foundation reserves the right, in its sole discretion, to disqualify any entrant found to be violating the Official Scholarship
Rules.
Disclaimers, Liability, and Indemnification: By applying for this scholarship, you release and agree to indemnify and hold harmless
FORUM Credit Union and the Foundation and their employees, directors, officers, affiliates, agents, and advertising and promotional
agencies from any and all damages, injuries, claims and causes of actions, or losses of any kind resulting from your scholarship
application or receipt of use of any scholarship.
The foundation shall have no liability for any loss or damage of any videos submitted during the application process.

his/her name, mailing address, telephone number, and e-mail address (“Personal Information”) for the purpose of contacting and
announcing the scholarship recipients; (b) grants to the foundation the right to use his/her personal information for publicity and
promotional purposes relating to the awarding of the scholarship, in any and all media now known or hereafter devised, without
compensation unless prohibited by law; and (c) acknowledges that the Foundation may disclose his/her personal information to thirdparty agents in connection with any of the activities listed in (a) and (b) above. The Foundation will use the recipient’s personal

Hamilton County Extension Homemakers Scholarship

The Hamilton County Extension Homemakers will award four $500 scholarships to Hamilton County students majoring in
the field of Health and Human Sciences or related fields.
Eligibility: You must be a high school senior entering college or a currently enrolled undergraduate,
majoring in Health and Human Science or related field. Included would be majors in departments of Foods
and Nutrition; Family and Consumer Science Education; Interior Design; Textile and Clothing Design;
Hospitality & Tourism; Child Development and Family Studies; Consumer Science & Retailing; Nursing;
Fitness and Health; Speech, Language and Hearing; and Health Sciences.
The scholarship will be awarded at the end of the first semester provided that at least a C average or decimal equivalent

Coach Don Nagy Memorial Scholarship

Hamilton Southeastern High School Football Program in cooperation with the Nagy Family,
has established a memorial scholarship in honor of Don Nagy who was a longtime volunteer
football coach for the Royals. Coach Nagy was extremely passionate about his family, the
Michigan State Spartans and the Hamilton Southeastern Royals. Coach Nagy believed in
the life lessons that are learned on the football field. He was a tremendous asset to the
Football Program and was a role model to the football players he coached.
A scholarship of $2000 will be given to a Hamilton Southeastern High School senior football
player who best exemplifies the qualities and characteristics of Coach Nagy.
Criteria for the scholarship:

Sponsored by Geist Elementary PTO, the Claire Tatom Memorial Scholarship is a
$500 scholarship awarded to one male and one female student graduating from
either Hamilton Southeastern H.S or Fishers H.S.
Eligibility Requirements:
1. Must have attended Geist Elementary for one complete school year K-4
2. Top 1/3 of your class
3. Must be planning on attending a four-year college or university
4. Be active in a variety of activities
5. Make a statement of your plans as they relate to your educational and career
objectives and future goals
6. Write a 300-500 word essay on what attending GES meant to you in your
education

The Twists and Turns Scholarship
Hunter Law Office, P.C.
Due 4-17-15
The Twists and Turns Scholarship by Hunter Law Office, P.C. will be awarded to the
applicant who, in the judgment of the Selection Committee, appears most deserving based
on the scholarship criteria. One scholarship of $500 will be given to a student from
Hamilton County and one scholarship of $500 will be given to a student from Parke County.
Eligibility Requirements:
•
Must be at least a senior in high school (current college students may apply)
•
Must be or will be a graduate of a high school in Hamilton County or Parke County
•
Must be enrolling in a college or attending college in 2015-2016 school year
•
There is not a GPA requirement
Application Requirements:
1. Applicant must complete the Uniform Application for Community Scholarship Form
2. Applicant must submit with their application packet (1) Personal Recommendation from an
instructor, counselor, club advisor, coach, church leader, or employer
3. Applicant must submit with their application packet (1) response to the following essay
question in times new roman, double-spaced,12 pt. font:
In the past, life was simpler. In today’s changing times, life is more complex with all its
unexpected twists and turns that inevitably take us off course. In 800 words or less, please tell
us about a time that life has taken an unexpected twist or turn and how you plan on utilizing it as
a learning experience as you navigate through college?
