those with long term conditions,
the vulnerable & elderly.
What do we offer?
 Support and advice for the
elderly, frail, vulnerable,
lonely, isolated and those
with multiple long term
conditions.
 Education to help patients
recognise symptoms and help
avoid unnecessary hospital
admissions.
 Visits to house bound
patients who suffer from
recurrent infections, such as
chest or urinary tract &
treating them accordingly.
 Health & Social care
assessments
 Referrals to social services,
rehab team, falls clinic,
occupational therapists,
day centres and more.
 Information on local services.
 Memory screening / Support
& advice.
 Blood pressure monitoring
 Support for patients with long
term conditions such as
COPD, heart failure, diabetes
or dementia.
 Support for those bereaved
in the last 6 months.
 Review of patients post
discharge.
