SUNCORP ORIGIN STAR TREATMENT COMPETITION CONDITIONS PARTICIPATION 1. This competition ("Competition") is conducted by Suncorp Corporate Services Pty Ltd ABN 69 074 966 466 (“the Promoter”), of Level 28 Brisbane Square, 266 George Street, Brisbane, Queensland 4000. 2. All entry instructions and prize information published by the Promoter form part of these conditions. By entering, entrants accept these conditions. 3. The Competition is only open to individuals who: (i) are residents of Queensland aged 18 years or older at time of entry; and (ii) are Suncorp Customers who hold at least one (1) product issued by either Suncorp Bank (Suncorp-‐
Metway Limited ABN 66 010 831 722), Suncorp Insurance (AAI Limited ABN 48 005 297 807) or Suncorp Life (Suncorp Life and Superannuation Limited ABN 87 073 979 530) (collectively “Suncorp”) ("Eligible Entrant"). 4. Competition entry into the various prize draws open and close at various times, on various dates as outlined in clause 13 below. However the Suncorp Origin Star Treatment website will be open for registration at 9.00am on 8 April 2015, and will close at 5.00pm on 15 June 2015 ("Entry Period"). Except as otherwise stated, all times are AEST for QLD. ENTRY 5. To enter the Competition, an Eligible Entrant must, during the Entry Period: a. visit www.suncorporiginstar.com.au (“Competition Website”) and enter their unique identifier (“Player Number”) provided via email or direct mail from Suncorp. Once the Entrant has submitted their “Player Number” they are required to follow the directions to fully complete and submit the site registration form, including providing their full name, valid email address, phone number, date of birth and residential address; b. tick the box to opt in to receive communication from Suncorp, relating to the ‘Origin Star Treatment Campaign’ communications; and c. tick the box agreeing to these Competition conditions. 6. Entrants may only enter the Competition in their own name. Limit of one entry per draw, per person. Any subsequent entry in the same draw from the same person after the first valid entry is received will be deemed invalid. Each entry must comply with these conditions. 7. Entries must be received by the Promoter during the Entry Period for that specific draw – refer to clause 13 below. Entries will be deemed received at the time that the Promoter is satisfied that all necessary requirements under clause 5 have been met and not at the time of submission by the Eligible Entrant. The Promoter takes no responsibility for late, lost, incomplete, incorrectly submitted, illegible or misdirected entries or delays or failures in any equipment or telecommunications service used to administer this Competition. 8. Cost of entry via the internet is free, but internet connection and usage rates may apply. Eligible Entrants should obtain details of such costs from their service providers. 9. Eligible Entrants must submit their internet entry manually. The Promoter may reject internet entries if it reasonably forms the opinion that they have been entered using automated entry means or by use of a competition entry service. 10. The Promoter may, in its absolute discretion, declare an entry invalid or disqualify any entrant that tampers with the entry process or benefits from such tampering, submits an entry that is incomplete or not in accordance with these conditions, or otherwise acts in a manner that is disruptive, dishonest or calculated to jeopardise the fair and proper conduct of this Competition. 11. The Promoter reserves the right to require an entrant to verify the validity of their entry within the requested time, including but not limited to an entrant’s identity, eligibility, age and address and to disqualify any entrant who does not satisfy any entry criteria in these conditions. Errors and omissions may be accepted at the Promoter’s discretion. Proof considered suitable for verification is at the Promoter’s discretion. DRAWS AND PRIZES 12. For all prize draws, all valid entries received during the period starting when entry opens and ending when entry closes for that draw (as set out in condition 13) will be entered into the relevant prize draw. . 13. All prize draws will take place at Suncorp Brisbane Square, Level 33, 266 George Street, Brisbane, Queensland 4000 on the relevant prize draw date. Winners will be drawn randomly in each prize draw from all valid entries entered into each respective draw, and prizes will be allocated until the number of prizes for each respective draw are exhausted. 15. Winners will be able to nominate their desired number of tickets to a maximum of 4 tickets per winner. Four (4) winners will be selected to win accreditation for two (2) people to attend a behind the scenes dressing room tour, and tickets to the State of Origin Match on 8 July 2015 – to be staged at Suncorp Stadium, in Brisbane. The prize will include a QLD State of Origin Merchandise pack, and hospitality at the match including access to attend a function with a Former Origin Great seated on the winners table. Flights and accommodation will be Up to $1,075 per prize package. The prize will include a QLD State of Origin Merchandise pack, and hospitality at the match. Flights and accommodation will be provided for winners that live 100km or more outside of Brisbane City. Fifteen (15) winners will be selected to each win one (1) of either a signed football (Five (5) footballs to give away) or a signed QLD State of Origin Jersey (Five (5) Jerseys to give away) or two (2) tickets to attend the State of Origin Match to be held at Suncorp Stadium in Brisbane on 8 July 2015 (5 x double passes to give away). Up to $1,400 Up to $14,000 Up to $380 per prize $3,400 *Winners of prize 1 are required as part of the entry process to specify either a name or nickname and number that they wish to have put on their personalized jersey. By entering into the competition, entrants agree that any name, nickname or number that they specify shall not be vulgar, derogatory, demeaning or any in anyway liable to cause offense. The Promoter at its absolute discretion impose restrictions on the names or nicknames that may be placed on the jersey and may, at its absolute discretion, require the winner to specify a different name or nickname to be placed on the jersey or replace any name or nickname specified with the Winner’s first or last name. PROVISIONS APPLICABLE TO PRIZES GENERALLY 16. The total prize pool is valued at up to $111,850 AUD including GST depending on the winner’s travel and accommodation requirements as at the date of prize delivery (if applicable). A prize value is the recommended retail value as provided by the relevant supplier(s). The Promoter takes no responsibility for any variation in a prize’s value due to exchange rate changes or otherwise. 17. Any additional costs not expressly stated, but which may be incurred in acceptance and use of a prize, are the responsibility of the winner. This includes but is not limited to travel and accommodation expenses – unless otherwise stated above, taxes, insurance, spending money and any other ancillary costs. 18. No compensation will be payable if a winner is unable to take any element of a prize as stated, or does not satisfy any conditions of use for any element of the prize (including conditions imposed by event organisers or staff, or other prize providers, such as health, behavior, age and safety requirements), for whatever reason. Any part of a prize that is not taken for any reason is forfeited and no compensation will be provided. 19. If a prize (or part of a prize) is unavailable for any reason, the Promoter may, in its absolute discretion, substitute alternative goods or services of equal or higher value, subject to the approval of any relevant gaming authority, where required. The Promoter accepts no other liability or responsibility for any loss incurred by a winner or any other party if a prize (or part of prize) is unavailable for any reason. 20. Other than as expressly stated in these conditions, a prize is non-‐transferable, non-‐refundable, non-‐exchangeable, non-‐replaceable, and non-‐redeemable for another prize including cash, except at the Promoter’s discretion and subject to relevant State and Territory legislation. PRIZE WINNERS 21. Winners will be notified as per the dates listed in clause 13 of these conditions for each relevant draw by a phone call or email, using the details provided in their entry. 22. Prizes from Draw 1 will be sent by registered post to the winners at their nominated Queensland address within 28 days of the draw. 23. The prize notification for Draws 2 – 4, will include instructions as to how each winner is to claim their prize. 24. The prizes from Draw 5 will be sent by registered post to the winners at their nominated Queensland address within 5 days from the close of the draw. 25. The prizes will only be awarded following any winner validation and verification that the Promoter requires in its absolute discretion. 26. The name of the winners for draws 1and 3-‐5 will be published at www.suncorporiginstar.com.au “Competition Website” on the dates as outlined in clause 13 of these conditions and for draws 3-‐4, they will remain on the website for a minimum period of 28 days. The winners for draw 2 will be individually notified via the contact details provided by the Entrant at the time of registration. 27. Subject, where relevant, to any directions given by a relevant gaming authority, if: (i) a prize is not claimed by a winner or the Promoter has been unable (despite reasonable efforts) to successfully contact the winner within 7 days from the draw; or (ii) a winner is determined by the Promoter to be ineligible to win a prize in accordance with these conditions, the relevant prize will be forfeited and instead awarded to another Eligible Entrant in a redraw. Any such redraw will be made from a pool of the remaining Eligible Entrants and will take place at the same location as the original draws, with any unclaimed prizes awarded to the next valid entry randomly drawn. The name of any resulting winner(s) will be published via the Competition Website within 2 weeks of the redraw. Any redraw prize winner(s) will be notified within 2 days of the redraw by a phone call or email using the details provided in their entry. GENERAL 28. If for any reason this Competition is not capable of running as planned, or is interfered with or disrupted in any way (including by vandalism, power failures, natural disasters, acts of God, weather event, civil unrest, strikes, computer bugs or viruses, tampering or technical failures), the Promoter reserves the right to cancel, terminate, modify or suspend the Competition, or invalidate any affected entries, subject to the approval of any relevant gaming authorities, where required. 29. The Promoter assumes no responsibility for: (i) any error, omission, interruption, or delay in the operation or transmission of any communication sent to (or by) the Promoter or any entrant whether caused by problems with communication networks or lines, computer systems, software or internet service providers, congestion on any carrier network or otherwise; (ii) any theft, destruction or unauthorised access to, or alteration of such communications; (iii) any problem with, or technical malfunction of, any computer system or other equipment used for the conduct of the Competition; (iv) any incorrect or incomplete information which may be communicated in the course of the administering this Competition (whether as a result of one of the foregoing causes or otherwise); or (v) any delay in delivery (when not directly caused by the Promoter or its supplier), or failure of safe delivery of a prize. 30. All of the Promoter's decisions in relation to all aspects of this Competition are final and no correspondence will be entered into. Failure by the Promoter to enforce any of its rights at any stage does not constitute a waiver of those rights. 31. A winner accepts a prize at their own risk. Any tax liability arising as a result of accepting a prize is the responsibility of the winner. Independent financial advice should be sought. 32. To the full extent permitted by law, the Promoter, its associated companies and agencies and any of their personnel (the "Relevant Parties") exclude all liability for any loss (including any damage, claim, injury, cost or expense) which is suffered or incurred by any Eligible Entrant in connection with the Competition or a prize, including without limitation: (i) any indirect, special, economic or consequential loss; (ii) any loss arising from the negligence of a Relevant Party; and (iii) any liability for personal injury or death. To the extent that such liability cannot be excluded by law, it is limited to the minimum allowable by law. 33. The winners must, at the Promoter’s request, participate in all promotional activity (such as publicity and photography) surrounding winning a prize, free of charge, and consent to the Promoter using their name, likeness, image and/or voice (including any photograph, film and/or recording of them) in promotional material in any media at any time without notification or compensation, financial or otherwise. PRIVACY STATEMENT AND MARKETING CONSENT 34. Entrants’ personal information is collected from entrants by or on behalf of the Promoter to enable the Promoter and its agents to manage, administer and promote the Competition and, where applicable, publicise the winners. The personal information may also be provided to persons assisting the Promoter, including prize suppliers and deliverers, marketing agencies and IT providers, and to gaming authorities, media outlets and related companies. Personal information may also be used to communicate to Entrants about future sponsorship promotions run by the Promoter conducted by Suncorp or the Promoter and may be shared with Suncorp for this purpose. By participating in the Competition, Eligible Entrants consent to these uses and disclosures of their personal information. 35. If an Eligible Entrant does not provide the personal information the Promoter requests, the Promoter may not be able to enter them into the Competition. All personal information is handled in accordance with these conditions and the Promoter’s Privacy Policy.
