Application Form for Admission
Candidates who have downloaded the Application Form for Admission from College Website
shall have to pay Application Form for Admission Fee of Rs. 1000/- through DD drawn in
favour of Lachoo Memorial College of Science & Technology, Jodhpur.
Candidates who have purchased the Application Form for Admissions from Pharmacy Office
of the College on payment of Rs. 1000/- are not required to pay through DD.
This fee is non-refundable, non-transferable and non-adjustable.
Eligibility Criteria for Admission
All residents of India fulfilling the eligibility criteria mentioned below can apply for admission
to D.Pharm., B.Pharm. and M.Pharm. courses on the basis of instructions given in this
Information Booklet.
Age: Candidate must complete 17 yrs of age on or before 31.12.2015 i.e., born on
or before 01.01.1999.

IIT-JEE (Mains)/AIPMT qualified candidates shall be given preference for admission.
Note:
1. As per Pharmacy Council of India (PCI) notification, candidates from open school are not
eligible for admission to D.Pharm. and B.Pharm. courses.
2. Candidates who are under result awaited category/ get supplementary can apply but
they are required to submit their result at the time of Deposition of fee.
M. Pharm.
At least 55% marks (aggregate of 4 years/ 3 years for lateral entry admission) in Bachelor
in Pharmacy of any University / Institution or any other examinations recognized as
equivalent thereto by the competent authority [50% in case of candidates belonging to
SC/ST/OBC (non creamy layer)/SBC (non creamy layer) of Rajasthan State].
For candidates having CGPA, after converting CGPA to equivalent percentage, at least 50%
marks (aggregate of 4 years/ 3 years for lateral entry admission) in Bachelor in Pharmacy of
any University / Institution or any other examinations recognized as equivalent thereto by
the competent authority [45% in case of candidates belonging to SC/ST/OBC (non creamy
layer)/SBC (non creamy layer) of Rajasthan State].
GPAT qualified candidates shall be given preference for admission. Such candidates are also
eligible for PG Scholarship from AICTE.

Fee Structure
The proposed fee for different Pharmacy Courses for the session 2015-16 is as follows:

Admission to D. Pharm. / B. Pharm. Course: Declaration of Merit List, Counseling
and Deposition of Fees
Admissions shall be made strictly on merit basis. Merit List shall be prepared on the basis of
score Obtained in IIT-JEE (Mains)/AIPMT and XIIth marks and shall be uploaded/ displayed
on College Website and College Notice Board.
Candidates securing equal score shall be considered for admission in the following order of
preference:
1) One who has higher IIT-JEE (Mains)/AIPMT score.
2) One who has higher marks in XIIth or an equivalent examination.
3) One who has obtained higher marks in secondary examination (Xth) or its equivalent
examination.
4) The candidate of higher age (older candidate).
For admissions, Candidates shall be called for Counseling on the notified date. The
admissions shall be made in accordance with Merit List and the Order of Choice of the
Course (D. Pharm. / B. Pharm.) filled in the Application Form by the candidates.
Selected candidates are required to deposit the Prescribed Fee and the Requisite Documents
(Original & Photocopy) on the same day / by the notified date.

Admissions shall be made strictly on merit basis. Merit List shall be prepared on the basis of
B.Pharm. aggregate marks and GPAT score (if applicable) and shall be uploaded/ displayed
on College Website and College Notice Board. Candidates having B.Pharm. final year result
awaited, may be considered for provisional admission on the basis of marks obtained upto
B.Pharm. III year.
Candidates securing equal score shall be considered for admission in the following order of
preference:
1) One who has obtained higher aggregate marks in Bachelor in Pharmacy course
2) One who has passed Bachelor in Pharmacy course earlier
3) The candidate of higher age (older candidate).
For admissions, Candidates shall be called for Counseling on the notified date. The
admissions to different M.Pharm. specializations shall be made in accordance with merit list
prepared on the basis of score of the Written Entrance Test and the order of choice of the
specialization filled in by the candidates on day of counseling.

2)
Paste your recent passport size coloured photo (taken on or after 01-01-2015) with
signature on the photograph.
3)
Tick the course (D.Pharm./B.Pharm./Both) applied for.
4)
Write your Name in Block Letters as appearing in your 10th class Mark Sheet. Tick Mr./
Mrs./ Miss
5)
Write Father‟s Name in Block Letters.
6)
Write Mother‟s Name in Block Letters.
7)
Tick the Category (SC/ST/OBC/SBC) as applicable to you.
8)
Write Date of Birth as appearing in your 10th class Mark Sheet in DD MM YYYY format.
9)
Write the State of Domicile: Rajasthan/ Name of other State.
10) Write full present Address for Correspondence along with City, Pin code, and State
where the communication to you could be made.
11) Write STD code and your Residence Telephone number.
12) Write your Mobile number (without zero „0‟) i.e. only 10 digit number.
13) Write your Father‟s Mobile number (without zero „0‟) i.e. only 10 digit number.
14) Write your valid E-mail ID where necessary information may be send.
15) Write details of Demand Draft (write your name at the back of DD)/ If you are
depositing Cash in the office then write Receipt Number.
16) Write details of qualifying examination

Tick the status of 10 + 2 or an equivalent examination: Passed/ Supplementary/
Result awaited

Write Year of Passing, the Name of Board (as appearing in the Mark Sheet) from
where you have passed the 10+2 or an equivalent examination. Also tick the
number of attempts.

Write the Marks Obtained and the Maximum Marks in each of the following Science
Subjects as appearing in the Marks Sheet of 10+2 or an equivalent examination:

Physics

Chemistry and

Biology/ Mathematics (any one, whichever is higher)

Write Total Marks Obtained and Maximum Marks in Science Subjects.

Calculate and write Percentage in Science subjects.

Write the Marks Obtained and Maximum Marks in each of the following
Compulsory Subjects:
a. Hindi/ other Subject(s)
b. English

Write Total Marks Obtained and Maximum Marks in all the Subjects.

Calculate and write Aggregate Percentage of marks in 10+2 on the basis of
Marks Obtained in all Science Subjects and all Compulsory Subjects.

In case of supplementary in any Subject(s), calculate the Aggregate Percentage
of marks by considering actual pass marks obtained in supplementary
examination.

If grades are awarded, write CGPA equivalent percentage in the above Subjects.
17) Mention valid IIT-JEE (Mains)/ AIPMT score and merit number.
18) Attach the following Documents and Tick appropriately.


19) Write Name, Father‟s Name, Date and Place under Declaration Head, Sign this
declaration and get it countersigned by your Father/Guardian.
20) Fill in the required entries except Roll No. and paste Photograph on the Admit Card
given at the last page of Application Form.
M. Pharm.
1)
Download the Application Form and fill in all the entries in the Application Form.

Write Total Marks Obtained and Maximum Marks in Science Subjects

Calculate and write Aggregate Percentage of marks in B.Pharm. on the basis of
Marks Obtained in all four years/eight semesters .

In case of supplementary in any Subject(s), calculate the Aggregate Percentage of
marks by considering actual pass marks obtained in supplementary examination.


at the last page of Application Form.
General Instructions:
1)
Ensure the eligibility and other requirements for admission.
2)
Outside Rajasthan candidates are also eligible for admission.
3)
Application Form for Admission can be obtained from Pharmacy Office of the College on
payment of Rs 1000/- or it can be downloaded from College Website. Candidates who
have downloaded the Application Form for Admission are required to submit a Demand
Draft of Rs 1000/- in favour of “Lachoo Memorial College of Science and Technology,
Jodhpur” alongwith Application Form.
4)
Be careful in filling the Application Form. If any lapse is detected during the scrutiny
even at a later stage, your candidature will be rejected even if you are admitted.
5)
In case of any wrong information being furnished by the candidate, his/ her admission
would be cancelled.
6)
Incomplete Application Forms and downloaded Application Forms received without
draft shall be rejected.
7)
Application Forms received after the last date shall not be entertained.
8)

9)
The College reserves the right to reject any Application Form for proper reasons
adduced therewith.
10) On the date of counseling the admitted candidates are required to deposit prescribed
fee in the form of Cash/ Demand Draft drawn in favor of “Lachoo Memorial College of
Science & Technology, Jodhpur”, payable at Jodhpur.
11) Admitted candidates shall bring all original documents for verification at the time of
counseling.
12) Fee once deposited shall not be refunded.
13) The decision of the admission board shall be final and binding to all.
14) The jurisdiction for legal cases arising shall be limited to the competent courts at
Jodhpur only.
Admissions on the Vacant Seats and Drop-out Seats
Advertisement shall be given for admission on vacant seats and drop-out seats (if any).
Admissions on the vacant seats and drop-out seats (allotted but not reported) shall
thereafter be made by the College in order of merit in a transparent manner ensuring the
eligibility criteria and as per the details notified accordingly.

Candidates who get supplementary in the qualifying examination and pass the
supplementary examination (after the notified date) shall also be considered for admission
on the vacant seats and drop-out seats.
