Non-Profit Shelter / Rescue Registration Contract

This Registration and Contract is made and entered into between 4 Precious Paws Low
Cost Spay/Neuter Clinic, and Non-Profit Rescue or Shelter.

 $15 donation for non-profit organizations. A copy of your 501(c)3 paperwork will be
required. If your organization would like to donate a small item into our gift bags,
the donation for the booth will be $10. Booth will not be reserved until appropriate
paperwork is received.
 A 10’x10’ space will be provided. You will need to bring your own tables and
chairs. If a larger space is needed we can discuss this at time of set up.
 Booth set up begins at 9:00am and tear down can begin at 4:45pm. Check in at the
information/registration booth to receive your booth location.
 Vendors will be allowed to drive their vehicle down to their area to unload/load, but
then we would ask that you park up by the Senior Center.
 Rescue/Shelter dogs/cats are to be kept in crates or on leashes at all times.
 All leashed dogs must be handled by an adult at all times. One dog per adult.
 Sufficient water must be provided for your animals.
 All animals should have their distemper vaccine at least 2 weeks prior to the
event.
 Please keep your area clean and orderly.
 No females in heat please.
HOLD HARMLESS AGREEMENT
Participant (indemnifying Party) agrees to protect, defend, indemnify, save and hold
harmless 4 Precious Paws and their officers, directors, agents, employees, and
representatives and The Parks Department and their officers, directors, agents,
employees, and representatives from and against any and all claims, losses, penalties,
damages, settlements, costs, charges, professional fees (including attorney fees), or
expenses are cause by any negligent act error or omission, or willful or want on act, of
the Indemnifying Party, its officers, employees, representatives or agents. The
indemnifying Party further agrees to investigate, handle respond to, and provide a
defense for any and all claims at its sole expense. 4 Precious Paws and The Parks
Department, at its sole and absolute discretion, choose to defend any claim and the
Indemnifying Party agrees to indemnify and reimburse 4 Precious Paws and The Parks
Department for all costs and expenses, including attorney fees, incurred by 4 Precious
Paws and The Parks Department in defending claim. Your participation in the Dog
Olympic Games event also provides permission to use your image and that of your
animal for future promotional materials.
Signing this form signifies that I have read and agree to the Hold Harmless Agreement
stated here and agree to event rules.
