MERIT LIST OF WRITTEN ADMISSION TEST - ADMISSION TO BHM COURSE
AT AIHM&CT
ACADEMIC SESSION 2015-16 (TWENTIETH COURSE)
Dear Candidates,
Congratulations to all successful candidates!
1.
Candidates listed below have been shortlisted for admission to BHM course
at AIHM&CT for the academic session 2015-16 ( Intake is 60). Acceptance of the
offer of admission should reach AIHM&CT by a demand draft of Rs 15,000/- on
or before 09 June 2015 and based on confirmation received candidates will
be called chronologically as per Merit list. Non receipt of confirmation of
acceptance by mail & scan copy of draft, to join the course by 09 June 2015 will
render the offer of admission automatically cancelled. Further representation
will not be entertained from the candidates who have not confirmed by due
date for admission for the academic session 2015 – 2016.

DOCUMENTATION
The candidate must bring the following certificates, in original along with
“Certified True Copy” in quadruplicate (preferably photostat copies duly
attested) and submit all the certificates to this Institute at the time of
admission:(a)
Matriculation certificate & Passing certificate of X standard or
equivalent examination from a statutory board.
(b)
Statement of Marks of XII standard and Passing certificate of XII
standard or equivalent examinations from a statutory board.
(c)
Eleventh Standard Marksheet.
(d)
Caste Certificate issued by the competent authority. This
certificate is mandatory as per Bangalore University
regulations.
(e)
Birth certificate, if date of birth is not given in sub serial (a)
and (b) above.

(f)
School/ Institute leaving certificate (after qualifying examination).
(g)
Bonafide certificate from the School/ Institute last attended, mentioning
the period of attendance.
(h)
Character certificate of good moral character from last School/
Institute attended.
(i)
Migration certificate, in case you have passed qualifying examination
other than from Bangalore University.
(j)
10 copies of passport size photographs in shirt and tie.
(k)
After having passed in the qualifying examination, if there is a gap
between the period of college/ Institute last attended and joining
AIHM & CT Bangalore, you have to submit an affidavit,
executed before a Magistrate on a stamp paper of requisite
value, as applicable, stating that you have not joined
any
course of studies during the said period in any other statutory
University/ Institute.
(l)
Dependent card vide Army Order No 74 / 75, 120 / 80, should be
produced on joining date.
(m)
Serving Certificate or Discharge book and PPO of parents.
(n)
Medical Certificate. A medical examination certificate with original
ECG report duly attested by the RMO/MO of nearest MH will be
brought by the student.(Appx ‘F’)
(o)
Details of parents
as per proforma attached. (Changes in
address/telephone Nos/Mobile Nos must be intimated on priority
by the parents).
(p)
UNDERTAKING BY STUDENT & PARENTS IS COMPULSORY for
HOSTEL AND ACADEMICS.

Joining Instructions of the course will be issued on confirmation on
receipt of Rs 15,000/- as registration fee.

Candidates required to report to AIHM & CT on 18 June /19 June
2015 at 1000hrs for documentation alongwith Parents.
