Statement of Cooperation and Consent 2015
I consent for my student, __________________________________________________________________, to take part in all Arma Dei Academy activities, including field trips, sports activities, and school sponsored trips away from the school premises. I acknowledge that an approved driver will transport my student in a private vehicle or bus. I also acknowledge and confirm that the medical release information and plan related to Arma Dei Academy sanctioned events apply to this activity. I acknowledge the dress code for this field trip to be an ADA uniform shirt, jeans, jacket, and good walking shoes and agree to have my student follow it. Should legal action, for any reason, be taken against Arma Dei Academy or any employee or agent thereof, on my student’s behalf, and the school or agent not be found at fault, I agree to pay any attorney fees, court fees, damages, or other costs that Arma Dei Academy or its agents should incur to defend itself against such action. I also understand by signing below that $25.00 will be withdrawn from my account via FACTS on or after May 14th to cover the cost of the upcoming field trip. _______________________________________________________________________________ _______________ Legal Guardian/Parent Signature Date FOURTH QUARTER FIELD TRIP All School Georgetown Mining and Railroad Field Trip Monday, May 18th, 8:30-‐3:20 Cost -‐ $25.00/Student Price includes chartered transportation to and from Georgetown, silver mining tour, Georgetown Loop train ride, and lunch at Beau Jo’s Pizza. In order for your student to attend this final all-‐school field trip, this signed permission slip and FACTS agreement of withdrawal must be returned to the front office by Friday, May 8th, 2015. Return to school by Friday, May 8th. Payment withdrawn from your FACTS account on or after Monday, May 14th, 2015.
