My child, _________________________,has my permission to be a cheerleader on the Bayou Belles
Sprit All-Star Prep Team. I understand that she must abide by the rules and regulations set forth by coaches and
directors at Bayou Belles Spirit. I understand that all practices and performances have mandatory attendance
requirements. I have read the rules and regulations and understand that the violation of any of these rules may
lead to permanent removal from the squad.
The spirit group constitution must be read and signed by the conclusion of the mandatory parent meeting
on May 1, 2015. If the necessary paperwork is not completed by this deadline, I understand that my child will
not be allowed to tryout.
I understand that my child must attend the tryout clinic session or my child will not be allowed to tryout
for the squad.
I understand that qualified judges will evaluate my daughter. Judges will evaluate candidates based only
on mastery of the skills that they see demonstrated during the tryouts. I agree to abide by the decision of the
judges.
I understand that by the very nature of the activity, cheerleading and gymnastics carry a risk of physical
injury. No matter how careful the participant, coaches, and directors are, how many spotters are used, or what
type of landing surface is used, the risk can not be eliminated. The risk of injury includes minor injuries such as
muscle pulls, dislocation, and broken bones. The risk also includes catastrophic injuries such as permanent
paralysis or even death from landing or falls on the back, neck, or head. I understand these risks and will not
hold Bayou Belles Spirit, LLC or any of its personnel responsible in the case of accident or injury at any time.

I hereby give permission for images of my child captured during regular and special activities through video,
photo and digital camera, to be used solely for the purposes of Bayou Belles Spirit, LLC promotional material
and publications, and waive any rights of compensation or ownership thereto.

The estimated cost of cheerleading for one year is outlined below. The greater part
of these expenses will be incurred during May-July of 2015. Every effort will be made to
keep these costs to a minimum. The stated costs include all fees for practice wear, cheer
uniforms and shoes, cheer bag, choreographer fees, USASF membership fees, &
competition registration fees for two competitions. The other competition fees will be
raised through required team fundraisers.
Uniform/Jacket/Bow: $225.00
Practice Wear: $20.00
Shoes: $60.00
Bag: $45.00
Choreography/Competition Fees: $190.00 (includes USASF membership)
Total: $540.00
All athletes are required to participate in fundraising activities. The fundraising
activities will help support competition functions and costs. Those not participating in
fundraisers will be required to pay for their portion from their own funds.
The total cost above is due by May 19. If you would like to participate in our
installment program, the breakdown of prices is below. Please note that an installment
fee of $30 will be applied.

Tryout Clinic
9:00 AM – 12:00 PM- @ Impact Sports
-Appropriate Dress: Solid white t-shirt and black shorts, tennis shoes, and socks.
Participants shirts must be tucked, and hair must be up & secure. NO JEWELRY!
Clinic is MANDATORY --candidates learn tryout material.
Cheerleading & Mascot Tryouts
at Impact Sports
Time: 2:00-5:00
Participants will be given a time slot to try out and will need to arrive back at
Impact Sports at least thirty minutes before their time slots. We will not wait on
late participants.
Appropriate Dress: Solid white t-shirt and black shorts, tennis shoes, and socks.
Procedures:
- Tryouts will be closed.
- No parents or friends are allowed in panel judging.
- Judging results will be kept confidential.
- Bayou Belles Spirit, LLC directors will review and approve the newly elected
squad.
- Numbers of the 2015-2016 All-Star Prep Squad will be posted on the Bayou
Belles Facebook page by 8:00 PM.
