Welcome to Annual Enrollment for 2015
benefits with Sage!
At Sage, we are committed to providing a competitive and comprehensive benefits package
that encourages overall health and well-being. As you’re a new Sage employee, we’re
pleased to offer you the opportunity to enroll in Sage benefits effective January 1, 2015.
Eligibility for benefits
As part of this transition, you will need to actively enroll in
Sage benefits between November 10 and November 21,
2014. The benefit elections you make will become effective
January 1, 2015.
You are eligible for benefits if you are a full-time or modified
full-time* employee who is scheduled to work at least 30 hours
per week, subject to the terms and conditions of the individual
plans. Employees who work less than 30 hours per week are
classified as part time and may only participate in the Sage
401(k) plan.
This newsletter provides a basic overview of the 2015 Sage
benefit options and what might be improving or different for
you this year. You’ll also find enrollment instructions and where
to go for more information. If you have questions about

This is a positive change from current eligibility requirements
under PayChoice plans. We hope this will allow more of you to
take advantage of the comprehensive benefits package
provided by Sage.
*A “modified full-time employee” is an employee who is not assigned to
a temporary status and who is regularly scheduled to work less than the
full-time work schedule, but at least 30 hours per week.

What this means for you*
You have the choice between
two medical plan options,
both administered by Anthem:
• Depending on your plan and coverage level, your premiums may be lower
and your coverage may be higher.
• You can lower your premiums by certifying that you are tobacco free and
participating in the everwell wellness program.
• Your deductible will be lower.
­ – 85/15 PPO: $400/$1,200 (individual/family)
­ – HSA Plan: $1,500/$3,000 (individual/family)
• Your coinsurance may be lower.
­ – 85/15 PPO: 15% coinsurance, after the deductible
­ – HSA Plan: 20% coinsurance, after the deductible
• You will have a new provider network through Anthem.
Note: Because the carrier network through Anthem overlaps with the Aetna
network and has many of the same providers, most employees will not have
to find a new provider.
• Transition of Care will be offered through Anthem for employees who need
to find a new provider.
• The way services are covered by the plan may be different (copay vs.
coinsurance).
• Sage medical coverage includes prescription drug coverage through CVS
Caremark; you will have lower prescription drug copays or coinsurance.
• Sage medical coverage includes these additional programs: Lifestyle/
Disease Management, Future Moms, and a 24-Hour Nurse Phone Line.
• 85/15 PPO: a traditional

• HSA Plan: a High
Deductible Health Plan
(HDHP) with a health
savings account (HSA)
Dental
You have one dental plan
option through MetLife.
• Your premiums will be lower.
• You will have a new provider network through MetLife.
Note: Because the carrier network through MetLife overlaps with the Cigna
network and has many of the same providers, most employees will not have
to find a new dentist.
• Your deductible will be lower or equal to what you currently have: $50/$100
(individual/family).
• Your annual and orthodontia lifetime maximums will be higher.
Vision
You have one vision plan
option through VSP.
• Your out-of-network reimbursement will be higher for most services.
• In-network lens options will be covered in full.
• Premiums will be higher as the Sage vision plan provides richer benefits.
Life and
disability
You have several life and
disability coverage options
through Reliance Standard:
• Short-term disability
• Long-term disability
• Basic life and basic
accidental death and
dismemberment (AD&D)
• Optional AD&D
• Voluntary life
• You are eligible if you work at least 30 hours per week.
• You are eligible immediately for basic life, AD&D, and disability coverage.
• Short-term disability coverage provides a higher percentage benefit
(100%-80% of your salary), for up to 26 weeks.
• Long-term disability coverage has a longer (180-day) waiting period and a
higher maximum benefit ($15,000/month).
Health
savings
account
(HSA)
Sage HSA plan participants
can open a health savings
account through Health
Equity.
• Eligible expenses will be the same.
• Contribution limits will increase for 2015: $3,350 for individual coverage;
$6,650 for family coverage.
• Health Equity will be your new HSA administrator. To learn more about the
transition to Health Equity, read the Frequently Asked Questions (FAQ) on
the Sage Benefits site.
• Sage contributes $500 for a covered individual, $1,000 for a family in 2015;
made in two installments, half in January and half in July 2015.

You have three flexible
spending account options:
• Health care FSA
• Dependent care FSA
• Limited-purpose FSA
• Eligible expenses will remain the same.
• The annual dollar limit on employee contributions to employer-sponsored
health care FSAs will increase to $2,550 in 2015, up from $2,500 in 2014:
– Health care FSA: $2,550
– Dependent care FSA: $5,000 ($2,500 for single parent)
– Limited-purpose FSA: $2,550
• TRI-AD will be the new claims administrator. To learn more about the
transition to TRI-AD, read the FAQ on the Sage Benefits site.
Wellness
program
The Sage everwell program
offers a variety of resources
and the opportunity to pay a
lower medical premium with
the wellness rates.
• If you have completed the wellness requirements under the existing
PayChoice wellness program, you will transition to the wellness rates for
2015 under the Sage medical plan.
• If you have not completed the activities to qualify for the wellness rates
through Aetna, you will have an opportunity to qualify for 2015 wellness
rates through the Sage everwell program.
• The everwell program has four levels which you can complete throughout
the year to earn incentives.
• To be eligible for the wellness rates in 2015, you have to complete three
required everwell activities and reach Level 1.
Employee
assistance
program
(EAP)
The Employee Assistance
Program is offered to
all employees through
Carebridge.
• You will have access to more face-to-face sessions per personal issue (up
to 6 sessions per personal issue).
Paid time
off (PTO)
– effective
4/1/2015
Sage employees accrue PTO
time that can be used for
vacation, sick or personal
time.
• You will be eligible to accrue PTO based on your anniversary date and
scheduled working time.
• You will see an increase in your total amount of time off.
Holidays
Sage provides nine companypaid holidays including one
floating holiday.
• Company-paid holidays at Sage:
– New Year’s Day
– Thanksgiving Day
– Memorial Day
– Day after Thanksgiving
– Independence Day
– Christmas Day
– Labor Day
– Day after Christmas
• The half day on Christmas Eve will no longer be a company-paid holiday;
however, you will now have one floating holiday (which may be used on this
day if desired). The floating holiday will replace the birthday holiday.
Leave of
absence
(LOA)
You can take either a
continuous or intermittent
leave of absence if you need
time away from work for
personal, military, or parental
reasons.
Additional
benefits
You have several additional benefit options:
• You will have a new leave of absence (LOA) administrator, Matrix
• You will need to follow the Sage LOA policies, which you can find on the
Sage Benefits site (benefits.na.sage.com; password: sagena).
• Tuition assistance: offers up to $5,250 reimbursement for eligible courses
• Adoption assistance: offers up to $5,000 reimbursement for eligible fees
• 529 college savings plans: offers two plans to help you save money for college
• ARAG group legal: offers savings and discounts on legal services
• Health Advocate: this is the same program you currently have
• Best Doctors: offers you and your eligible family members with access to world-class medical experts to help
you make informed health care choices

How to enroll
Ready to enroll? To get started, follow these easy steps:
Step 1. Visit the Sage Benefits site (benefits.na.sage.com; password: sagena) for a comprehensive overview of your plan
offerings. You can also access decision tools through the site.
• Decision Tool: Compare your medical plan options and out-of-pocket costs to choose the plan that’s best for you
and your family.
• Anthem Coverage Advisor (password: sage): Input your plan option, coverage level, and spending account
contributions to calculate how much you might spend on health expenses during the year.
• “Understanding your HSA” video: Check out a short video that’ll help you learn about an important feature of our
HSA plan.
Step 2. Gather the names, birth dates, and Social Security numbers of anyone you are enrolling for coverage or adding as
a beneficiary for life insurance.
Step 3. Log on to the TRI-AD MyBenefits Portal (www.eAboutMe.com/sage) any time between November 10 and
November 21, 2014. Your first time enrolling, you will have to:
• Enter your Social Security number, birth date, and home ZIP code.
• Create a username/password.
• Provide your email address and set up security challenge questions.
Step 4. Once logged on, click the Health & Welfare Benefits Enrollment Account panel to begin the enrollment process.
Step 5. Certify your tobacco status and your spouse’s/domestic partner’s tobacco status. If you do not log on to the
TRI-AD MyBenefits Portal and indicate your status, your medical and life insurance premiums will automatically default
to tobacco rates.
Step 6. Make your benefit elections.
Step 7. Make sure your personal information and address are correct so you’ll receive your ID cards on time.
Step 8. Confirm your enrollment and print your statement.
Step 9. Complete the everwell Level 1 requirements to earn the 2015 wellness rates if you do not already qualify for the
PayChoice wellness discount.
You must take action!
Remember to enroll on the TRI-AD MyBenefits Portal
between November 10 and November 21. If you don’t
actively enroll by November 21, you will only have
company-paid benefits for 2015.

click the Sage Payroll Solutions panel. This page provides you
with the tools and resources you’ll need to help make informed
decisions around your 2015 benefit options.

dependent information, or navigating the TRI-AD MyBenefits
