Job Purpose
The post holder will work as part of an integrated treatment recovery system across the
borough of Lewisham to encourage and facilitate participation in the Primary Care
Recovery Service (PCRS); and provide health interventions including service user
assessment, BBV screening and immunisation, detoxification (including prescribing) and
on-going contribution to recovery planning.
The post holder will also provide nursing support for the Primary Care Recovery Service
and support the development and provision of enhanced primary care provision for
community alcohol detoxification within a recovery model.
Main Duties and Responsibilities
1.
Primary Care Liaison and Engagement
1.1 Coordinate the participation and engagement of GPs and community pharmacies in the
primary care recovery service.
1.2 Proactively visit all participating and interested surgeries and pharmacies to build
relationships, instil confidence in the new service and troubleshoot any issues.
1.3 Support GP shared care arrangements, advising health care professionals in relation to
opiate substitute treatment, alcohol and other drugs.
1.4 Comply with NMP protocols, NICE and Clinical Management (2010) guidelines to
facilitate community alcohol detoxifications in primary care settings.
1.5 Provide direct nursing expertise and support /advice to the alcohol community detox
hubs.
1.6 Undertake comprehensive assessments, risk assessments, provide alcohol brief
interventions, and detox and recovery care planning to service users within a primary
care setting.
1.7 Offer testing, pre and post-test information for BBVs including onward referrals for
further interventions and treatment when necessary.
1.8 Support GP Shared Care Workers with client work where required.
1.9 Ensure high standards of patient care are maintained in line with shared care practice
standards.
1.10 Contribute to the development of formalised pathways to wraparound and physical
and mental health services.
1.11 To attend GP meetings and GP clinic review meetings where appropriate.
1.12 Maintain professional links with related organisations to ensure elements of continuity
of care, up to date profiling of the service, referral links and general information
sharing.
1.13 To work in partnership with other voluntary agencies and statutory services and
support the Joint Providers Forum.
1.14 Maintain effective working relationships with local lead GPwSIs.
1.15 To attend external meetings as directed by the Operational/Clinical leads.
1.16 To attend and contribute in Organisational clinical forums.
2. Training and Development
2.1. Develop training programme on alcohol brief interventions and deliver to GPs and
pharmacies across the partnership.
2.2. Develop training programme on BBV interventions and deliver to GPs and pharmacies
across the partnership.
2.3. Develop and coordinate protocols on the delivery of dried blood spot testing and
referral.
2.4. Provide support for the development of enhanced primary care provision within GP
surgeries, including the development of community alcohol detox programmes in
targeted clinics.
2.5. Provide advice and training to partnership teams on health and wellbeing, clinical
issues, resolving conflict, as well as supporting staff who are working with service
users with more complex needs.
2.6. Work in partnership with PCSR Workers to co-ordinate service user pathways and to
maintain positive developmental relationships with service users, GPs and
pharmacies.

1. Recording, Monitoring and Evaluating
3.1. To co-ordinate the maintenance of client records in such a way that conforms to
Blenheim policies and procedures and complies with medical and legal requirements.
3.2. Support the effective recording of interventions and activity on BOMIC and relevant
data and case management systems.
3.3. To assist the Manager and administrator in the provision of information that enable
data collection, input and retrieval.
3.4. To ensure that all interventions are underpinned by sound theoretical methods to
ensure that outcomes are measurable.
3.5. To ensure that all communications take account of information governance policies to
ensure appropriate levels of confidentiality.
2. General Duties and Responsibilities
4.1. To maintain all policies and procedures in place in Blenheim.
4.2. To attend supervision, appraisal and other meetings as directed by line Manager.
4.3. To represent Blenheim in a professional manner at all times.
4.4. To treat all colleagues and service users with respect and dignity.
4.5. To contribute to the strategic development of Blenheim by identifying skill sharing
opportunities, training provision and the exchange of information.
4.6. To undertake any task in line with role and responsibilities as directed by the line
manager.

Learning & Development Requirements
The post holder is expected to be a registered nurse with at least three years post
qualification experience and a current PIN.

training identified on your training plan and other learning events such as conferences etc.

accessed or referred into.


