This year’s event will be held at Midsun Community Association on May 9th 2015 Please have your
competitors fill out the attached entry sheets and return them to me at:

Rules available upon request.

WAIVER:
Please ensure all competitors have completed and signed the waiver
found on the enclosed Registration Form and submit at the tournament.
EQUIPMENT:
ENTRY FEES:
Kumite competitors must provide their own fist protectors, mouth
guards, and groin cups.
$30.00 per event, Kata or Kumite

PROCEDURE:
Photocopy one Individual Registration Form per competitor and have
them fill it out completely. Transfer information to the Master
Registration Form and total the fees due.
Please submit one cheque per club payable to CSKA
email the master Registration Form to the tournament director before the
deadline. Other forms and the cheque must be given to the tournament
director before 9:00am on the tournament day.
PLEASE NOTE ALL KUMITE WILL BE IPPON SHOBU
NO TEAM KATA ENTRIES WILL BE ACCEPTED ON TOURNAMENT DAY

and sport of karate is potentially hazardous and that a participant is exposed to risks in said activity. I further
acknowledge that in the course of doing karate I may receive injuries that may be serious and permanent.

and forever discharge the Canadian Shotokan karate Association, its officers, servants, employees,
