What is Case Management?
Case Management is the process of consulting, planning, implementing, delivering,
monitoring and reviewing support for a client. It’s like having a tertiary qualified ‘go to’
person to help you get the most out of life, however you define it.
What happens at my initial consultation?
At your initial consultation, your Case Manager will discuss with you your current situation,
your current areas of need and any concerns you have and what support you may require.
Together, we will develop a Care Plan that is individually tailored to your situation. Your Care
Plan will detail what supports and activities you would like to undertake so as to achieve
your goals.
What does it cost?
If you are interested in any services CCNB can provide, please make contact with us and we
will discuss relevant costs at that time.
I don’t live on the Northern Beaches, can I still access your private service?
We already provide support beyond the Northern Beaches. In fact, we work right across
Northern Sydney, which includes the Local Government Areas of Manly, Warringah,
Pittwater, Ryde, Hunters Hill, Hornsby, Kur-ring-gai, Willoughby, Lane Cove, North Sydney
and Mosman.
However, should you live outside Northern Sydney, please do not hesitate to contact us to
discuss providing support in your area.
