OFFICIAL KSNF/KODE-TV CONTEST RULES FOR 4” DICE ROLL
1. How to Enter the Contest:
a. Subject to Section 3(a) below, this contest will run for eight weeks, from May 7th
2015 through June 25th 2015. Actual dice rolls will be held on the following
Thursdays at the following Severe Weather Tour locations:
May 7, 2015: Schuber Mitchell Homes – Webb City, MO
May 14, 2015: Windows 4 Less – Joplin, MO
May 21, 2015: Gaslight Restaurant – Joplin, MO
May 28, 2015: Griffith Motors - Neosho, MO
June 4, 2015: City of Galena – Galena, KS
June 11, 2015: Joplin Metro Credit Union – Joplin, MO
June 18, 2015: Carthage Chrysler – Carthage, MO
June 25, 2015: Cook Portable Warehouses – Joplin, MO
b. This contest will terminate immediately upon the occurrence of a Grand Prize
winning roll/throw. If the Grand Prize has not been awarded at a previous Severe
Weather Tour location, the first 25 eligible contestants to sign up at each Severe
Weather Tour location will have an opportunity to roll the dice. Sign-ups will be
held at each Severe Weather Tour location starting at 4:30pm on the Thursday of
each event.
c. Limit one (1) roll per person for the duration of the contest. Entrants may be
disqualified at the sole discretion of KSNF/KODE-TV.
d. By entering this contest, contestants agree to abide by these Official Rules and the
decisions of KSNF/KODE-TV and Odds On Promotion which decisions will be
final and binding.
2. Eligibility Restrictions:
a. The contest is open to legal U.S. citizens who are 18 years of age or older and
who reside in one of the following counties; Missouri- McDonald, Newton,
Jasper, Barton, Vernon, Barry, Lawrence; Kansas-Cherokee, Crawford, Bourbon,
Allen, Neosho, Labette, Wilson, Woodson, Montgomery; Oklahoma; Ottawa,
Delaware, Craig. Current or former employees, officers, and directors of Nexstar
Broadcasting, Mission Broadcasting, Atlas Safe Room, Hideout Harley, The
Insurance Center, any Weather Tour Location Sponsors, Odds On Promotions and
Incentive Plus Network, their respective agents, successors, assignees, affiliated
companies, subsidiaries, distributors, dealers, retailers, printers, advertising, and
promotional agencies, any and all other companies associated with this contest,
and the members of the family members or household members of any of the
above are not eligible to participate.
b. Only one (1) winner per household is permitted in any contest.
3. Prize:
a. Grand Prize: USD: $25,000. Only one Grand Prize will be awarded. If the Grand
Prize is won in accordance with these rules, the contest will officially end at the
time the Grand Prize is won. If the contest officially ends before any of the dates
above, KSNF/KODE-TV will announce it on
www.fourstateshomepage.com/home. Participants are encouraged to the check the
website prior to arriving at the Severe Weather Tour locations. If no contestant
successfully rolls the (5) dice with the letters “S-T-O-R-M” in any order face up
in accordance with these rules, no Grand Prize will be awarded. The odds of
winning the Grand Prize are 7.776 to 1 on each eligible roll.
b. Grand Prize is provided courtesy of Odds On Promotions/Incentive Plus Network.
c. The Grand Prize must be picked up in person at the office of KSNF/KODE-TV,
located at 1502 South Cleveland Street, Joplin, MO 64801 or at a location to be
designated by Odds On Promotion. The Grand Prize must claimed within the
earlier of (i) the time period designated by Odds On Promotion or (ii) 30 days of
confirmation of the award of the prize. Failure to pick up the prize in the time
period specified will result in forfeiture of the prize by the winner.
d. Grand Prize winner will receive 1099 IRS Tax form for the value of the Grand
Prize as stated in these contest rules.
e. Prizes are limited solely to those items explicitly set forth herein and shall not
include any and all other expenses, incidentals or taxes that may arise out of the
winner’s receipt of this prize.
4. Selection and Notification of Winner:
a. Decisions of station management with respect to the contest are final and binding.
b. A maximum of 200 contestants (25 per Weather Tour Location) may be eligible
to roll the dice.
c. The first 25 eligible contestants at each of the eligible eight Severe Weather Tour
locations may be given the opportunity to attempt to win a Grand Prize by
providing all requested information in the official registration binder and rolling a
set of 5 pre-printed dice after receiving instructions to do so from the Contest
Administrator. In order to be a valid attempt, the Grand Prize cannot have been
previously won and each eligible contestant must:
i. Print and sign their full name and address in the official registration binder
on the next unoccupied line. All information must be printed clearly to be
eligible.
ii. After completing this information, and upon direction of the Contest
Administrator, the contestant must roll the provided set of 5 dice in
accordance with the requirements outlined herein. Contestants must roll
the dice immediately after signing the official registration binder.
Contestant may roll the dice only one time (i.e., there will be no practice
rolls).
iii. A contestant may participate at only one Severe Weather Tour location.
d. In order for a Grand Prize to be awarded, all 5 dice must be rolled/thrown at once
by the contestant onto a smooth playing surface with the skyward side coming to
a rest as follows: S-T-O-R-M. The dice must be rolled/thrown from within a
container and come to rest a minimum of six (6) feet from where the roll/throw
was initiated. Each die must make a minimum of two (2) complete rotations while
in the air and land with the letters S-T-O-R-M face up in accordance with these
rules. Each die must be totally flat on the playing surface and no die may be
stacked or leaning on another die in any manner. The contestant must be the first
contestant to have achieved this result.
e. All sign-ins and dice roll/throw attempts must occur in the presence of the Contest
Administrator (a representative of Sponsor(s)). Each contestant’s sign-in and
roll/throw must be recorded on videotape in its entirety. Any sign-in or dice
roll/throw attempt that is not in the presence of the Contest Administrator, or that
is not videotaped in its entirety for any reason will be disqualified and is not
eligible for a Grand Prize, regardless of the outcome. Sponsor’s footage must
contain video of each contestant’s body position in relationship to the various
demarcation lines as well as the dice coming to rest after each roll. If a contestant
begins their sign-in, attempt, or makes any sign-in or roll/throw prior to Sponsor’s
recording, or while Sponsor’s camera is not properly functioning for any reason
whatsoever, their entire attempt will be disqualified. Sponsors reserve the right to
review the film footage after any winning attempt and make a determination (in
their sole discretion) whether the footage is acceptable and qualifies with these
rules. Sponsors disclaim any responsibility for technical problems or equipment
failures that result in disqualifying or insufficient footage.
f. Contestants are not permitted to take any practice, warm-up, or qualifying rolls at
any time after his/her notification of participation in the promotion until their
actual attempt. Once any one die has been rolled, their attempt is considered to be
started.
g. Each eligible contestant must roll/throw each oversized dice all at once, unless
otherwise provided in these rules. Contestants may not attempt to change the
outcome of any rolled die by hitting the die with a subsequent roll attempt.
h. Each die must be tossed in a way that does not allow the contestant to “set” the
dice, alter or influence the random outcome of the roll.
i. Each die must be rolled a minimum of six (6) feet in distance from the contestant,
in order to prevent the contestant from altering, affecting, influencing, or
enhancing the random outcome of any roll. The Contest Administrator will
designate the demarcation line that each die must cross during the roll attempt, as
well as the line behind which the contestant must be positioned to take his/her
attempt. No part of the contestant’s body or clothing must come into contact with
the demarcation line(s) prior to releasing the roll/throw.
j. Each dice must make a minimum of two (2) complete rotations while in the air.
k. All dice must come to rest lying individually and completely flat. Any die that is
stacked or leaning in any fashion will void the roll/throw and the contestant will
be allowed a replacement roll/throw of that die only. Any such replacement
roll/throw must be in full compliance with all of the foregoing. Each contestant
will be allowed only one replacement roll/throw regardless of whether the
replacement roll/throw is successful.
l. All dice must land on and remain on the designated dice rolling surface.
m. No rolled die may come in contact with any person or object other than the
designated rolling surfaces or walls.
n. If any die comes in contact with another person or object (other than the dice roll
surface or designated walls or backstop, or other dice) for any reason or if any
person or object in any way interferes, alters, changes, influences or affects the
random outcome of the roll, then the entire attempt is disqualified and the
contestant may not receive another attempt for the duration of the contest period.
5. Conditions:
a. Payment of all federal, state and local taxes are the sole responsibility of the
winner. Winners will be required to sign an IRS Form W-9 or the equivalent.
Photo identification and social security number of the Grand Prize winner may
also be required in order to collect the prize. Failure or refusal to execute any
requested or required documentation will result in winner forfeiting the prize.
b. The winner may be requested to take a polygraph in Sponsors’ sole discretion.
Any failure to take the polygraph will result in forfeiture of the Grand Prize by
winner.
c. By participating in the contest, the winner or winners (and their guest or traveling
companion, if any) agree to have their name, voice, or likeness used in any
advertising or broadcasting material relating to this contest without additional
financial or other compensation, and to sign a publicity release prior to acceptance
of the prize.
d. Only one Grand Prize will be awarded. The Grand Prize winner must accept
delivery of prize from KSNF/KODE, or as otherwise instructed by the Sponsors.
All other costs related to prize delivery and acceptance are the responsibility of
winner including but not limited to all federal state and local taxes. Prizes are
non-transferable and no cash redemption or prize substitution allowed, except at
KSNF/KODE sole discretion or as provided herein. KODE/KSNF reserves the
right to substitute a prize of equal or greater value if an advertised prize becomes
unavailable. Contestants understand and agree that delivery or receipt of the
Grand Prize will not be immediate upon winning and may be subject to delays.
The Sponsors reserve the right to conduct a reasonable investigation into the
circumstances of a winning attempt.
e. Grand Prize winner will be required to complete and return a W-9 form, affidavit
of eligibility, and liability/publicity release, and present a valid driver’s license
and social security card before prize will be awarded. Winners of prizes greater
than $600.00 will be issued a 1099 form reflecting the value of the prize(s) and
are responsible for paying all income taxes on prize(s). Pursuant to the liability
release, the winners will agree to hold Incentive Plus/Odds On Promotions,
KSNF/KODE,their licensees, their affiliated companies, the other Sponsors,
administrators, and participants in this contest, and the officers, shareholders,
directors, employees, agents and representatives of each of them harmless against
any and all claims, causes of action, liability, judgments, damages, costs or
expenses (including reasonable attorney’s fees) arising directly or indirectly from
the prize or participation in the contest.
f. KSNF/KODE reserves the right in its sole discretion to modify the contest rules
and dates and/or terminate the contest at any time for any reason. Material
modifications shall be announced on-air, when practical. By participating in this
contest, you agree to be bound by these contest rules, any modifications thereof,
and by all other rules imposed by the KSNF/KODE.
g. No purchase is necessary to enter or win. Purchase does not increase chances of
winning. This contest is subject to local, state, and federal law and is void where
prohibited.
h. Failure to comply with the contest rules may result in a contestant's
disqualification, at the sole discretion of the KSNF/KODE or the other Sponsors.
i. KSNF/KODE is not responsible for typographical or other errors in the printing,
the offering or the administration of the contest or in the announcement of a prize.
j. Copies of the written contest rules and a list of all winners are available during
regular business hours of KSNF/KODE-TV, 1502 Cleveland Street, Joplin, MO
64801.
