Centegra is pleased to present My Centegra
Hospital Chart, your online medical records
resource.
In order to create a My Centegra Hospital Chart
account you can choose to receive a personal
email invitation by providing your email address at
registration. Or, you can click on the “Access

to access information from your hospital visit
including a summary of care, radiology results,
allergies, vitals and more—all in one place from
the convenience of home.
Now, when you need your test results from your
hospital stay for follow-up visits, you can securely
access and print your results free of charge.
Please note: If you add any information to your personal
health record, Centegra Health System cannot access
those changes. All questions regarding test results
should be directed to your primary care or ordering
physician.

To begin, choose one of our easy ways to register.
1. If you provided Centegra an email address,
you will receive an email invitation. Click on
the link in the invitation to go to our partner
site, RelayHealth. Steps 1-5 of this brochure
will walk you through the registration process.
2. Or, visit centegra.org and click on the “Access
My Centegra Chart” button. Next, click on the
“My Centegra Hospital Chart” and then the
“Register” tab. Select that you are a “Patient.”

Once your account has been created, click on
“Link to a New Provider” to add a provider. In the
“Provider’s Last Name” field, search for Centegra
and select “Centegra Health System.”
*You may add a member to your account if
you are his or her legal guardian and a written
authorization has been signed. Centegra requires
that the added member had a recent hospital visit
and a completed hospital authorization form on file
before adding him or her to your account. Please
wait to add a member until he or she has a recent
hospital visit and authorization is signed.
