How To Enroll Patient Portal 1.Visit the link patientportal.bmcf.com/selfenroll 2. Enter your last name, first name, date of birth, medical record number (e.g. F000###) and email address. Then click SUBMIT ENROLLMENT REQUEST 3. If the self-enrollment is successful, you will receive the one-time User ID/Password email. Be sure to check your spam folder if the email does not appear in your inbox. 4. Go to patientportal.bmcf.com 5. Enter your one-time User ID/Password 6. You will be able to create your own User ID/Password 7. Log off and then click LOG ON link again 8. Sign on with your new User ID/Password 9. Explorer the Portal! How To Pay Bill via Patient Portal 1. Log on Patient Portal using your logon ID and Password. 2. Click Billing 3. Click Show all Accounts 4. Click Make a Payment 5. Enter the amount that you wish to pay and Click Continue 6. Fill out the credit card information then click Submit. 7. You will receive the receipt via email.
