Movie Permission Form May 14, 2015 Dear parent(s) / guardian(s), I would like to show the movie “Unbroken” to the AP US History students. I feel that this movie has educational value. The movie depicts the heroism of those who fought in WW II and the character built by facing adverse circumstances and the benefits of forgiveness. The movie is based on the life of Louis Zamperini and the hardships he and others faced while fighting the Japanese in World War II. Zamperini was an Olympic athlete who joined the armed forces during WW II. He was captured by the Japanese navy after a plane crash in the Pacific. The movie “Unbroken” is Rated PG-13 for war violence including intense sequences of brutality, and for brief language. There is a fairly intense scene where two soldiers are told to undress – I will make every attempt to black out the screen for that scene. I would encourage you to see the complete parent advisory / content guide that can be found on the IMDB database. The web address is: http://www.imdb.com/title/tt1809398/ If you choose not to give your child permission to watch the movie, an alternative assignment will be given – the student will not lose any academic points for not viewing the movie. Thank you, Chris Calhoun, JDHS By signing below, you indicate that your child has your permission to watch “Unbroken” with the APUSH class. _______________________ _____ ___________________ Parent / Guardian Signature Phone Number where you can be reached Date
