mix1 Ultimate Arizona Coyotes Experience Official Rules
1. General. By participating in the mix1 Ultimate Arizona Coyotes Experience (the "Drawing"), entrants accept and agreed to
these Official Rules and the decisions of Arizona Coyotes Hockey Club ("Coyotes"). The Drawing is subject to all applicable
federal, state, and local laws and is void where prohibited.
2. Prizes. The Drawing will be for specific prizes (the "Prizes"), which are as follows:



Grand Prize (One winner): Two tickets for glass seats (Row A) with access to the BMW i Lounge for a
2015-16 Arizona Coyotes regular season home game, a VIP Experience (which includes the opportunity
to watch pre-game warmups from the Arizona Coyotes Penalty Box and Post-Game Meet and Greet
with an Arizona Coyotes Player), and a mix1 Prize Pack.
First Prize (Two winners): An Arizona Coyotes Team-signed jersey, two tickets for mix1 Club seats for a
2015-16 Arizona Coyotes regular season home game, and a mix1 Prize Pack.
Second Prize (Three winners): An Arizona Coyotes Team Signed Hockey Stick, two tickets for mix1 Club
seats for a 2015-16 Arizona Coyotes regular season home game; and a mix1 Prize Pack.
Tickets will be provided for admission to games as selected by Coyotes and may not be resold and cannot be transferred
without the approval of the Coyotes. Pre-game warm-up experience requires adherence to certain safety pre-cautions.
3. Entries. No purchase is necessary to win. Entries may be submitted from March 18, 2015 until 5:00 p.m. on April 17, 2015 via
on-line entry form or by submitting entrants name, address, email address, and phone number to: Arizona Coyotes Hockey
Club, mix1 Ultimate Arizona Coyotes Experience, 9400 West Maryland Avenue, Glendale, Arizona 85305. Only one entry per
person; persons submitted multiple entries will be disqualified and any winnings forfeited. Employees and immediate family
members of the Coyotes, Global Spectrum, LP, Levy Premium Foodservice LP, Fanatics Retail Group Fulfillment, Inc., and mix1
employeesare prohibited from entering the Drawing.
4. Selection of Winners. Winners are randomly selected during the week of April 20, 2015. All entries will be numbered and
winners will be selected by using the Random Sequence Generator provided by random.org (or similar independent random
number sequencing program.) Entries bearing the first number of the random sequence will be awarded the Grand Prize;
entries bearing the second and third numbers will be awarded the First Prize; and entries bearing the next three numbers will
be awarded the Second Prize. In the event of the selection of an ineligible winner, the entry bearing the next random
sequenced number will be declared the winner. Winners will be notified using the contact information provided and will
publicly announced on the Arizona Coyotes official website, http://coyotes.nhl.com. Odds of winning are dependent on the
number of entries.
5. Claiming the Prizes. Winners are notified by using the contact information provided and must claim their prize within 14 days
of the Drawing. Claiming the Prizes requires winners to provide written indicia of acceptance to the Coyotes. The actual prizes
will be made available at a later date. Winners must provide all identification and other information to claim the Prizes,
including tax identification information. A winner forfeits the Prize the event he or she does not accept the Prize within the
required period, refuses to provide the required information, is ruled ineligible, or refuses the prize.
6. Taxes and Costs. Each winner is responsible for any and all federal, state, and local taxes, fees, assessments and like charges.
Withholding may be required by law. Each winner is responsible for travel and other costs when utilization awarded tickets.
7. Interpretation and Amendment of the Rules. All decision of the Coyotes are final. The right to modify these Rules or the
Prizes at any time, or to cancel or suspend the Drawing is reserved.
8. Release of Liability. By entering into the Drawing, all entrants agree to release IceArizona Hockey Co LLC, the National
Hockey League and their member teams, Mix 1 Life, Inc., the City of Glendale, and each of their respective parent entities,
subsidiaries, and affiliates, together with their owners, partners, employees, officers, directors, sponsors, and agents, from any
and all liability with respect to any injury, damage, or loss, directly or indirectly, in whole or in part, from the participation in the
Drawing, or from any activity related thereto.
9. Publicity Release. By accepting the Prize, each Winner grants to the Coyotes permission to use such winner's name, address
(city and state only), photograph, voice, and other likenesses, and prize information for advertising, trade, and promotional
purposes without further compensation, notice, review, or approval in all media now known or hereafter created, worldwide,
and in perpetuity. Each winner must execute all requested documents Coyotes in connection with this release.
10. Winners List. A Winners List will be provided upon receipt of a request and a self-address, stamped envelope that is send to
Arizona Coyotes Hockey Club, Attn: mix1 Ultimate Arizona Coyotes Experience, 9400 W Maryland Ave., Glendale, AZ 85305.
11. Governing Law. The Drawing and these Rules are governed by the laws of the State of Arizona. If a court of competent
jurisdiction finds that any provision of these Rules or their application to a particular persons or circumstances to be invalid or
unenforceable the remainder of these Rules remain enforceable to the fullest extent permitted by law. Venue for any cause of
action pertaining hereto is solely Maricopa County.
