2015 Research Analyst as Independent Contractor REQUEST FOR QUALIFICATIONS CHOICE REGIONAL HEALTH NETWORK | 1217 4th Ave E, Suite 200; Olympia, WA 98506|www.crhn.org Request for Qualifications 1 BACKGROUND INFORMATION In the spring of 2014, stakeholders from different community sectors came together to launch the Cascade Pacific Action Alliance (CPAA), a regional association of independent stakeholders focused on improving individual and community safety and wellbeing through aligned action. The Alliance built on years of collaborative efforts across county lines to improve the health of our communities. CHOICE Regional Health Network, a nonprofit collaborative organization has supported the formation of the Accountable Community of Health as the backbone organization. CHOICE is a nonprofit collaborative of health care leaders in a five-county region that includes Grays Harbor, Lewis, Mason, Pacific and Thurston counties. The organization was founded in 1995 with the mission to improve community health in Central Western Washington through collective planning and action of health care leaders. The organization’s board of directors consists of CEO-level executives from hospitals, community health centers, public health, and behavioral health agencies, and the region’s leading medical home. We have partnered for years with the region’s health care providers, local governments and a large number of social service agencies and community organizations on a wide range of community health improvement initiatives. In the development of the seven-county Accountable Community of Health, the CPAA region has consciously built upon existing community-based health improvement coalitions and forged new crosssector community relationships. The bottom-up design principle of the Alliance has led to the authentic engagement of hundreds of residents and organizations across multiple sectors in the seven-county region. It has also informed the selection of the following shared regional health priority areas. REGIONAL HEALTH PRIORITY AREAS: Increase Access to Health Care Services/Provider Capacity Integration and Coordination of Care Improve Chronic Disease Prevention and Management Mitigation of Adverse Childhood Experiences Enhancing Economic and Educational Opportunities The CPAA has explored each of these five priority areas which has led to the identification of a preliminary set of strategies and associated actions to achieve these aims. To dive deeper into these priority areas, and to inform decision making, the CPAA has identified a need to develop a regional health data assessment, asset map, and a gap analysis. It is also likely that the CPAA may identify new regional strategies as data is analyzed and discussions evolve. Shared action across community sectors and county lines necessarily requires agreement on shared performance measures in order to monitor progress toward the achievement of common goals. A metric dashboard and shared measurement system will be essential to monitoring the progress of the region. 1|Page 2 SCOPE OF WORK CHOICE Regional Health Network, the backbone organization for the Cascade Pacific Action Alliance is soliciting qualifications for a Research Analyst on an Independent Contractor basis. The chosen applicant shall provide professional research analyst services to help develop a regional health improvement plan for a seven-county region. 2.1 TIMELINE April 2015 through June 2015. 2.2 GEOGRAPHIC AREA In responding to this RFQ, applicants will provide research analytic services to the Cascade Pacific Action Alliance seven-county region in Washington State, to include: Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston, and Wahkiakum counties. 2.3 SCOPE OF WORK The Research Analyst will work to help develop a regional health improvement plan by providing the following services: Provide research analytic expertise in applicable health disciplines to collect, abstract, analyze or interpret qualitative and quantitative data contained within systems and databases related to the identified priority areas. Develop a regional health assessment based on metrics and measurements relative to the region. Investigate, research and collect data through personal interviews, community networks, databases, and organizations from seven counties to develop an asset map for CPAA’s 5 priority areas listed above. Develop and write a gap analysis to present in a PowerPoint to the CPAA Council. Assist with and provide guidance toward the development of a shared measurement system. Present all findings in a finished product to various audiences who have different levels of understanding related to data and metric information. 2.4 COMPENSATION Maximum not-to-exceed $15,000. 3 PROPOSAL CONTENT Potential candidates shall submit the following items in the Statements of Qualifications (SOQ) as part of their response to this RFQ. 2|Page 3.1 COVER LETTER Potential candidates shall provide the following information within a cover letter: Trade name of candidate firm Name of person and title completing application Mailing address Telephone number and fax numbers Email address An authorized official must sign cover letter 3.2 STATEMENT OF QUALIFICATIONS (SOQ) Potential candidates shall demonstrate their qualifications by providing: 1. Overview of organization 2. Resumes of professionals reflecting qualifications assigned to the project 3. Recent and relevant experiences/projects a. Include experience working with community projects in the seven-county region: Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston, and Wahkiakum counties, if applicable. 4. Submit an hourly rate and expected hours to complete Scope of Work. 4 SUBMISSION PROTOCOL 4.1 SUBMISSION CONTENTS SOQs should include the following information in the designated order: 1.
