MKSAP 16 QUESTION
A 68-year-old woman underwent a right hemicolectomy 2 years ago for stage III colon
cancer. She received 6 months of chemotherapy with 5-fluorouracil, leucovorin, and
oxaliplatin (FOLFOX) after surgery. On a recent routine follow-up visit, a serum CEA
level was found to be elevated to 43 ng/mL. She has no known medical comorbidities
and takes no medications. She works full time and recently returned from a ski vacation.
On physical examination, temperature is 37.1 °C (98.8 °F), blood pressure is 125/85 mm
Hg, pulse rate is 80/min, and respiration rate is 14/min. The physical examination is
notable for a palpable liver edge just below the right costal margin. The abdomen is soft
with no distention or masses, and bowel sounds are normal. No supraclavicular lymph
nodes are palpable. Laboratory studies indicate a hemoglobin level of 13.5 g/dL (135
g/L), a leukocyte count of 9000/µL (9.0 × 109/L), and a platelet count of 288,000/µL (288
× 109/L). Contrast-enhanced CT scan of the chest, abdomen, and pelvis demonstrates
three hypodense lesions on the right lobe of the liver ranging in diameter from 1.5 to 4.0
cm. No other abnormalities are seen on the scan.
Which of the following is the most appropriate management?
A. CT-guided fine-needle aspiration of liver lesion
B. Hepatic arterial embolization
C. Palliative systemic chemotherapy
D. Radiation therapy to the liver
E. Right hepatectomy
NEJM IMAGE CHALLENGE

Welcome to Fundamentals of
Clinical Medicine!
"FCM is the highlight of intern year!"
“My favorite part of FCM was doing the hand
dancing!”
“FCM was super fun, and I really enjoyed
bonding with my co-interns over beautiful (and
sometimes not so beautiful) art!”
“Also, our black fleeces are a wildly unifying
force, as you can see in the pic!”
