Future Pro Five Star Combine/Skills Camp and Future Pro Youth Football Clinic
WAIVER AND RELEASE OF LIABILITY
PLEASE READ THE FOLLOWING CAREFULLY AND SIGN BELOW:
In return for being allowed to participate in the Future Pro Five Star Combine/Skills Camp and/or Future Pro Youth
Football Clinic, I release, and agree not to sue, Future Pro Sports Marketing, LLC, and its member clubs, the Official
Sponsor, the Official Testing Partner, the Official Recruiting Partner, and/or any other Official Future Pro Sponsor/
Vendor at the Future Pro Five Star Combine/Skills Camp and/or Future Pro Youth Football Clinic, their respective
employees, contractors, officers, owners, sub-contractors, sponsors, agents, and affiliates (collectively the
“Released Parties”) from all present and future claims that may be made by me, my family, estate, heirs, or assigns
for property damage, personal injury, or wrongful death arising as a result of my participation in the Future Pro
Five Star Combine/Skills Camp and/or Future Pro Youth Football Clinic including, without limitation, claims caused
by the ordinary negligence of any of the Released Parties, wherever, whenever, or however the damage, injury, or
death may occur. I understand and agree that the Released Parties are not responsible for any injury or property
damage arising out of or in any way related to the Future Pro Five Star Combine/Skills Camp and/or Future Pro
Youth Football Clinic, even if caused by their ordinary negligence. I further agree to indemnify the Released Parties
for any claims made by, or on behalf of, me for any claim of property damage, personal injury, or wrongful death
arising as a result of my participation in the Future Pro Five Star Combine/Skills Camp and/or Future Pro Youth
Football Clinic. Such indemnity extends to the costs of defending or settling a claim made against a Released Party
(including, without limitation, reasonable attorney’s fees). I understand that participation in Future Pro Five Star
Combine/Skills Camp and/or Future Pro Youth Football Clinic involves certain risks, including, but not limited to,
those of serious injury or death. I am voluntarily participating in Future Pro Five Star Combine/Skills Camp and/or
Future Pro Youth Football Clinic with knowledge of the danger involved and agree to accept and assume all risks of
participation in the Future Pro Five Star Combine/Skills Camp and/or Future Pro Youth Football Clinic and all
related activities. Future Pro Sports Marketing has not undertaken to determine whether I have any medical
condition that can cause me to be unfit to participate, and I acknowledge that Future Pro Sports Marketing has no
duty to do so. I agree to let Future Pro Sports Marketing use my likeness, name, portrait, recorded voice, and
biographical material, free of charge, to advertise, promote, and publicize, but not as an endorsement of any
product or service of any advertiser. I further specifically agree to permit Future Pro Sports Marketing to take
measurements of my height, weight, body fat, other physical dimensions, and athletic performance and to publish
those measurements, together with my likeness, biographical information, academic information and other data in
any manner in the sole discretion of Future Pro Sports Marketing, including on one or more of the Future Pro
Sports Marketing’s web sites, which I specifically understand is part of the Future Pro Five Star Combine/Skills
Camp and/or Future Pro Youth Football Clinic. I understand that this document is intended to be as broad and
inclusive as permitted by the laws of the state of which the combine is held and agree that if any portion of this
agreement is invalid, the remainder will continue in full legal force and effect. I further agree that any legal
proceedings related to this waiver will take place in the city of which the combine is held.
I am at least eighteen years of age, am competent to sign this agreement, and am freely doing so. I have read this
form and understand that by signing this form, I am giving up legal rights and remedies.
