Mark Menolascino, MD, MS, ABIHM, ABAARM, IFMCP is founder/director of the Meno Clinic Center for Advanced Medicine in Jackson
Hole, WY. He received his MD from the University of Nebraska, followed by Internal Medicine training at Banner Health, Barrows and Mayo

2015 Terms
& Regulations
Cancellation Policy
Cancellations for exhibition must be made in writing and are subject to the following
provisions: If exhibit space is cancelled for any reason, a $200 cancellation fee will be charged
to cover incurred administrative expenses. Full refunds (less the administrative fee) for
cancelled exhibit space will be made for all cancellations made prior to July 1, 2015.
Cancellations received by September 1, 2015 will receive a 50% refund on the price of the
exhibit table space. No refunds will be issued after September 1, 2015. No refunds will be
issued for any other sponsorships.
Fees
Exhibitors and sponsors are responsible for all hotel fees associated with delivery of products
and supplies as well as service charges required for production, distribution, or display.
Limitation of Liability
Exhibitors and sponsors assume the entire responsibility and liability for losses, damages, and
claims arising out of injury or damage to exhibitor’s display, equipment, and other property
brought upon the premises of the hotel, and shall indemnify and hold harmless Heal Thy
Practice, Holistic Primary Care and the hotel, agents, and employees from any and all such
losses, damages, and claims unless caused by the hotel’s sole negligence.
Force Majeure
If events beyond the control of Holistic Primary Care, as planners of this meeting, including
but not limited to acts of God, war, strikes and threats of strikes, government regulation
or advisory, civil disturbance, terrorism in the US, curtailment of transportation in the US,
disaster, fire, earthquakes, hurricanes, extreme inclement weather, or any other comparable
conditions, occur either in the location of the Conference, or in the countries/states of
origin of at least twenty-five (25%) of the attendees or along their routes of travel, making it
impracticable, illegal, or impossible to perform under this Agreement, the affected party may
terminate this Agreement, without liability, upon written notification.
