2015 Wellness Program Summary
Our employees’ health is very important to us. In an effort to motivate our employees to be
more aware of their health and ways that it can be improved, Roanoke City Public Schools
provides reduced insurance premium rates to those employees who elected to be “Wellness
Compliant”. It is anticipated that if employees exhibit healthful behaviors, the result will be
both a healthier workforce and reduced overall health care cost.
New In 2015:

We have a new Wellness Plan Administrator. UnitedHealthcare’s Incentives for Health
program will collect and track wellness compliance for our employees. UHC receives and
tracks Physical – Health Actions forms
 2015 information is available on the RCPS website, Employee Tab, Wellness. You will
receive a mailing from Incentives for Health in the next few weeks.
 Employees participating in the wellness program for 2015 will receive a $50 per
month reduction in their premiums or increase in premium credit (for a total of $500
in savings per year) in 2015.
 Under the Non RCPS Spouse also Compliant level, both Spouse and Employee must
meet Wellness Compliance requirements to qualify for Wellness in 2016. If either
the employee or the spouse fails to be compliant then neither are compliant for
2016.
 With 2 Employee coverage, both employees must meet Wellness Compliance
requirements to qualify for Wellness in 2016. If either employee fails to be
compliant then neither are compliant for 2016.
REQUIREMENT FOR 2015
Have an Annual Preventive Physical by 9/30/15: you must get a preventive physical
examination by September 30, 2015 and you MUST submit the Physical form to
UnitedHealthcare’s Incentives for Health program by 9/30/15 as well.

Employees are responsible to submit the physical form to UnitedHealthcare’s
Incentives for Health program by September 30, 2015 to be considered Wellness
Compliant for 2015. Employee, please return forms to:

 Please retain copies of your forms and verification that they were sent
to UHC via fax or mail by 9/30/15.

Employees are responsible to verify receipt of their physical form.
 Check with UHC that your form was received before the 9/30/15
deadline
 If after the deadline of 9/30/15, you must provide proof that you met the
deadline for your physical and submitted all other required
forms/receipts by 9/30/15. If this occurs after the 9/30/15 deadline,
employees have until 10/31/15 to notify RCPS of an issue; if this is not
done by 10/31/15, no changes to Wellness will be made. This proof
must be:
o Copies of your original paperwork
o If faxed, a fax confirmation form noting the date sent to UHC
New In 2015:


This year employees are not required to attend a Weight Management program based
on BMI or Tobacco Cessation program based on tobacco status. These programs are
optional.
We also have a new process for Reimbursement for Weight Management and Smoking
Cessation support. If your BMI is 25 or greater, you may participate in a weight
management or if you are a tobacco user, you may participate in a tobacco cessation
program.
To participate in a weight management program or smoking cessation, you must do the
following two things:
Step 1: have your provider complete the 2015 Provider Recommendation form if you haven’t
already submitted this form. Please submit this form to RCPS Human Resources.

Step 2: to receive reimbursement, complete the Wellness Reimbursement Claim Form and
attach legible copies of all receipts. Submit the form and receipts to RCPS for review and
