Documentation Needed for the 2nd Round of Pre-K Registration:
The following documentation must accompany your application in order for the student to be considered:
1.
The child’s original birth certificate or birth identification card (In order to be eligible for a 4-year old class in 2015-2016 session, a child must be 4 years of
age on or before September 30, 2015.
2.
A copy of income – Income can be verified by submitting at least one of the following:
a.
Individual income tax form 2012 1040/1040A or 2014 W-2 forms
b. Documentation showing current status as a recipient of public assistance
c.
Copies of three recent (consecutive)check stubs from working parent(s)/guardian(s)
d. Letter from the parent including the amount of money received on a weekly basis and a statement saying that they are paid cash for the work they do
and are therefore unable to provide check stubs. Letter must be dated and signed by the parent.
3.
Applicant’s driver’s license or picture I.D. or passport
4.
Proof of residency – (Must reside in Jefferson Parish) Two of the following documents are acceptable:
a.
Current utility bill, telephone bill, Direct TV bill, cable bill, car insurance bill or deposit for utilities (issued within the month of
registration)
b. Driver’s license or state approved identification card showing the current address of the parent/legal guardian
5.
Special Education documentation ~IEP/Evaluation (only when applicable)
