Let’s pretend for a minute –
You are walking down stairs and you place your foot on the next step. It is wet and you
slip, even though you are maintaining 3 points of contact. When you slip, your ankle
twists and you experience some pretty bad pain. What do you do????
Each of us has responsibilities associated with injuries. So let’s go over them
and make sure we understand the path that needs to be taken if an injury
happens.
You, as an employee – the person who was injured
Employees are responsible for the timely reporting of incidents to their supervisor. In the event
the employee is unable to contact their supervisor, report the incident to the next person in the
chain of command. Employees are also responsible for contributing information relative to the
incident during the investigation process.
Your Managers/Supervisors – the person you report to
Managers/supervisors are responsible for ensuring that all of their employees understand the
procedures for reporting incidents. Managers are also responsible for investigating all reportable
(vehicle), recordable (OSHA/OH&S) incidents and spill/release accidents. All supervisor level
and up, or their designees, will be required to complete CS IMPACT training, with the
expectation they will enter all incidents with follow-up as required by KM EHS Policies.
Our KM EHS Representative
The EHS Representative is responsible for maintaining accurate documentation of incidents and
for the maintenance and administration of the OSHA 300 Log (US)/Hazardous Occurrence
Investigation Report (CAN). Documentation and 300 Log (US)/ Hazardous Occurrence
Investigation Report (CAN) shall be maintained in CS IMPACT. The EHS Representative is
also responsible to act as a liaison with Terminal Management to assure that incidents and
corrective actions are appropriately entered into the CS IMPACT system.
What if you are a contractor and are working on a project - Project Manager
The KM Project Manager shall ensure that all KM incidents involving contractors providing
services for Kinder Morgan shall be reported in accordance with this procedure and
documented in CS IMPACT. Form CSM-001, Contractor Accident/Injury Report or equivalent
must be completed for all contractor incidents.
It seems pretty logical, right. We are expected to report all incidents, including near
misses, accidents, first aids, equipment damage, contaminations, spills, etc. This is
part of Drive to Zero and how we are accomplishing the really good record we are
enjoying in the Gulf Area!
