RELEASE AND COVENANT NOT TO SUE
As a vendor wishing to rent space in Heritage Days on the Old National
Road Festival, I agree to save, hold harmless and indemnify the City of
Martinsville, Martinsville on the Move, Heritage Days, its sponsoring agency,
members and employees, from any and all liability or responsibility pertaining
to any damages to person or property on the site leased to me from said festival,
when such damages, or liability arise out of any acts of my own, or of my
employees or associates, located at said site.

The Festivals & Events Committee of Martinsville, Illinois, invites you to participate
in the “Heritage Days on the Old National Road” festival to be held Friday, June 12
through Sunday, June 14, 2015. We have extended the festival an extra day this year to
include A Night of Bluegrass--4 Bands in 4 Hours on Friday evening.
Saturday’s festivities will begin with the Third Annual Heritage Days Equine
Parade at 11:00 a.m. In addition to vendor booths & demonstrations, other activities
being planned include storytellers, Native American impersonators, an Indian Artifact
display, a quilt show, an antique tractor/truck show, old-fashioned games for adults &
children, many food vendors and 8 great band performances. In addition, a communitywide church service will be held Sunday morning at 10:00.

Vendors will pay the lesser of $100 or 10% of sales for rental of their space
A deposit of $100 (cash, check, or money order) must accompany this
application.
Deadline for Applications is May 1, 2015. The application fee must be
received by May 1 to guarantee your spot.
No refunds will be given after June 5, 2015. NO EXCEPTIONS. THIS IS
A RAIN OR SHINE FESTIVAL.
Food vendors must be set up and ready for operation no later than 5:00 p.m
Friday.
Food vendors must have their operations open from 5:00-9:00 p.m. Friday,
10:00 to 8:30 p.m. Saturday, June 13, and 11:00 to 5:00 on Sunday, June 14.
Heritage Days reserves the right to deny any application that does not meet
event criteria.
Vendors are required to provide a Certificate of Insurance with
“Martinsville on the Move” and “The City of Martinsville” named as added
insured. NO EXCEPTIONS
FOOD VENDOR DEPOSIT - $100
If you desire to participate in Heritage Days please fill out the front & back of the
attached vendor application and mail it with your check made out to “Martinsville on the
Move” in the amount of $100. Send it to:
